EE was hosing off pad under feeder when a rock fell out of a hole and struck EE in the shoulder.

EE was changing a turbo on a haul truck. EE was using a scraper to remove the gaskets from the turbo and needed to change the blade. After changing the razor blade on the scraper, the employee was trying to tighten the set screw with an O-ring pick. EE's hand slipped and caused the razor blade to cut EE's middle finger on EE's right hand just above the knuckle on EE's right hand.

Employee was leaning against handrail and the rail broke, when the rail broke employee was struck in the lip by the water hose nozzle causing a small cut to upper lip and a chipped right front tooth. Employee did not seek medical treatment until 7/29/2019.

While in the office an employee experienced a personal medical condition unrelated to work activities. The employee fell to the floor which resulted in a fractured nose.

Employee was changing bucket teeth on a PC 300 excavator bucket on October 19th 2016. The employee states EE felt a pain in EE's right shoulder while swinging sledge hammer and needed to stop for a minute. EE requested medical treatment on Nov. 4th. EE was diagnosed with sprained right shoulder.

While removing a zip tie with a pocket knife, the knife slipped and cut the employee's finger.

EE was installing wear plates on 988H loader when he felt a pain in his back.

EE crushed left thumb with a hammer used to loosen a bolt on the bed of the haul truck, requiring two stitches.

TWO EMPLOYEES WERE INVOLVED IN HORSEPLAY WHICH RESULTED IN ONE RECEIVING A FIRST AND SECOND DEGREE BURNS TO HIS LEG.

EE WAS LYING ON HIS BACK DRILLING HOLES IN THE FLOOR BOARD OF A WELDING TRUCK TO MOUNT A HEATERUNIT WHEN A PIECE OF METAL FELL BEHIND HIS SAFETY GLASSES & ENTERED HIS RIGHT EYE.
