Pulling belt on tugger machine, hook broke causing tugger to move forward and sling chain, hitting employee on leg and ricochet into mouth.

Employees were changing a screen at VS-2. After LOTO, the screen was unbolted and secured and as employee was positioning the screen for removal, the screen shifted and pinned employees finger between the screen and the lip / edge of the chute.

Contract Highway Truck driver fell while exiting cab of truck. Fell to the ground complained of back pain contacted EMS for transport to Clinch River Medical in Richlands Va.

Descending stairway - hyperextended knee, went to the ground in sharp pain in left knee. Pain went away, knee sore went to hospital.

Reportable hearing loss left ear only of 26.

Stepping off of Pug Mill Hopper, slipped on ice sprained left knee.

Maintenance work on plant individual was being handed a tool (3 ton come-a-long) by another employee. Finger was caught between the tool and plant frame causing a cut that required 3 stitches.

Ice on stairway, slipped and twisted left knee.

Individual complained of pain in his side he felt that it was a possibly a hernia. Individual went to doctor and confirmed same.

Individual was greasing the lift arms on a 988F Caterpillar loader. He slipped and fell to the ground about 18 to 24 inches. Reported that he twisted his knee. He did refuse treatment and or to go to the doctor as noted on company sign off accident report.

Air Conditioner repairman (NOT A POUNDING MILL QUARRY EMPLOYEE) caught his left hand. Last joint of middle finger was severed.

Cleaning out a rock box, loose rock fell and broke finger on left hand.

Individual was using a 'sledge hammer' to straightened a access step on a mobile piece of equipment. When hitting the surface of metal step to be straightened, individual's left index finger came between the hammer handle and metal bracket.

Unit being repaired by dealer mechanic-unit was over heating hydraulic fluid. Dealer mechanic being instructed by manufacturer tech service to bypass thermal valve, sending fluid directly to cooler system-hydraulic line ruptured spraying fluid-flash fire burning operator (our operator) on back and arms (2nd degree). Dealer mechanic received minor burns on hand and forehead.

EE helping maintenance man hook chain up to lifting hook to break loose wedged rock in feeder a loose rock about size of football rolled down about 5' and struck him on right leg. The leg does have a cut about 11/2"" long. Supervisor sent man to emergency room.

Was getting out of water truck, left foot slippd on steps letting him fall and hit right side of body on rocker panel of truck, sore under arm around ribs.

WAS WALKING DOWN CATWALK ON #68-BELT. LOST FOOTING FROM ICED CONDITION. FELL DOWN ON RIGHT SIDE ONTO CATWALK. RIGHT ARM GETTING NUMB AND TINGLING IN NECK ON RIGHT SIDE.

Was climbing off truck bed; had been putting block in bed to spread with. Fell off top step of side of bed, hitting on back and head.

EE WAS CLEANING CHUTES WHEN HIS FOOT SLIPPED. HE STRADDLED THE HANDRAIL AND SIDE OF GUARD AND FELL DOWN ABOUT 4 FEET.

WAS SLINGING SLEDGE HAMMER, AND IT SLIPPED CAUSING FINGER TO HIT SIDE OF STONE BIN.

EMPLOYEE WAS USING A HAMMER TO BEAT MUD OFF ROLLERS. THE HAMMER BECAME LODGED BETWEEN THE MOVING BELT AND ROLLER. BEFORE OUR EMPLOYEE COULD LET GO OF THE HAMMER HIS RIGHT THUMB AND INDEX FINGE R BECAME CAUGHT BY THE BELT.

SLIPPED ON ICE.

SLIPPING TIRE ON 35 TON TRUCK,, FELT SHARP PAIN IN THE LOWER RIGHT ABDOMEN NEAR GROIN AREA

WAS DIGGING OUT CRUSHER RUN CHUTE AND MATERIAL FELL IN ON LEFT FOOT.
