Employees were changing a screen at VS-2. After LOTO, the screen was unbolted and secured and as employee was positioning the screen for removal, the screen shifted and pinned employees finger between the screen and the lip / edge of the chute.

Individual was using a 'sledge hammer' to straightened a access step on a mobile piece of equipment. When hitting the surface of metal step to be straightened, individual's left index finger came between the hammer handle and metal bracket.

Unit being repaired by dealer mechanic-unit was over heating hydraulic fluid. Dealer mechanic being instructed by manufacturer tech service to bypass thermal valve, sending fluid directly to cooler system-hydraulic line ruptured spraying fluid-flash fire burning operator (our operator) on back and arms (2nd degree). Dealer mechanic received minor burns on hand and forehead.
EE helping maintenance man hook chain up to lifting hook to break loose wedged rock in feeder a loose rock about size of football rolled down about 5' and struck him on right leg. The leg does have a cut about 11/2"" long. Supervisor sent man to emergency room.
