Over-exertion in lifting objects

Employee was helping lift a 2 1/2"" inch woven wire screen on the top screen deck . When had lifted the screen approximately 12"" to 18"" off the floor, he felt something pop in his left arm. He was taken to the doctor that day, who diagnosed it as a strain. He was returned to regular duty.

Employee was moving the gasoline de-watering pump, shifting it down hill. Reached to get gas can and felt a twinge in lower back.

Employee working on 990 loader bucket removing wear plates between teeth. Had impact gun on nut. When trigger was hit, bolt dropped, wrench came off and bolt cut employee's left hand.
