Over-exertion (Not Elsewhere Classified)

Employee was operating a 992K loader while loading customer trucks when EE heard a ""pop"" in EE's right wrist while using the control handles.

Struck by flying object

Employee for C & L Machine and Welding and foreman for Martin Marietta were standing on-site looking at a tooth on a bucket to see which one had to be removed. Employee tapped the tooth with a hammer and a metal splinter went into EE's groin area.

Fall onto or against objects

Employee was climbing a ladder to complete a task on the crusher. Employee went to step on to the third rung of the ladder and slipped. Employee landed on the handrail to EE's right. When employee fell EE's finger/arm caught a piece of metal plate resulting in a scratch on EE's arm and a cut on EE's finger resulting in the nail falling off.

While performing maintenance on a crusher, employee stepped to swing a hammer and dislocated right knee. This injury became reportable on 7/30/2024 when physical therapy was prescribed.

A miner was involved in an accident while operating a CAT 777D haul truck that resulted in the truck being turned on it's side. No injuries were reported.

Employee stated while using a battery operated grinder to cut a bolt off of a belt roller bracket, the grinder recoiled back striking EE's left knee, resulting in a small laceration requiring 6 sutures

Employee was preparing the crusher. When stepping backwards they lost balance contacting the handrail on thier right ribs.

Employee was changing the manganese on a 300 cone crusher. While loosening a bolt, employee's left ring finger and left middle fingers got caught between the wrench on the bolt and the ram that was being used to remove the bolt. Finger tips of listed fingers were sliced.

Employee had hooked a pipe wrench to the hydraulic cylinder to hold the drill steel while he attempted to loosen the bit. The pipe wrench twisted and dropped, striking and breaking the tip of his finger on his left hand.

Employee claims he hurt his back lifting a seat assembly out of a 992D FE Ldr. He went to the dr on 2/14/2013 and the doctor returned him to FD and told him to take Tylenol. He rtn'd to the dr on 2/21/13 still complaining of numbness in leg and doctor gave him 2 prescriptions, felt it was a strained hip and told him to go to therapy, but rtn'd him to his regular full duty job.

The injured employee experienced 2nd degree burns to his right foot. This occurred when hot slag contacted his foot through a hole in his protective footwear.

He was changing screens in the wash screen and reached up and rubbed his arm across the frame and felt soreness in his wrist. He looked and a big whelp was on his arm, just above his wrist with a red streak going up his arm past his elbow. It appeared to be an insect bite.

Struck by falling object

Employee was pulling drawer full of rock out of the Gilson shaker screen and the right hand corner of the drawer stayed in the slot and the left hand corner came out. The drawer tipped left and started to fall and the employee tried to catch it and it hit him in the wrist causing swelling and pain to left wrist.

An employee was inside a screen deck changing screens. Material was frozen to the side of the screen, so another employee was heating the outside of the screen with a propane torch. Debris poped off the the screen and a hot piece landed on the right side of his face, just under his safety glasses. He wiped it off with his gloved hand and a piece of metal got in his right eye.

While pulling drill steel from a rack on the drill he alleges he slipped and it caused his back to start hurting. He did not fall down.

While loosening bolt on tail pulley bearing, a wrench slipped, mashing the EE's finger between the wrench and frame. The injury resulted in the laceration of the EE's finger, requiring stitches.

Fall from machine

EE was stepping from operator platform to bed of boom truck at which time he stated he lost his footing and landed on hit back. He was working in same area as other ee. Other ee turned saw him on ground EE prof. to help accident yelling for help ee evaluated situation called for ambulance transported him to hospital diagois 3 broken ribs and punctured lung.

Employee was trying to remove a bearing ring located inside the end of a six inch diameter drill hammer. He was using a slide hammer extractor for this task. To accomplish this he had to slide the it back and fourth on the rod. The third time he did this he caught his little finger between the back plate and the slide hammer.

Using screw driver to replace an old hose clamp on compressor hose with a new one. Screw driver slipped and struck employee's left hand scratching it just above his left thumb.

EMPLOYEE WAS TIGHTENING NUT ON BOLT WITH BOX END WRENCH. THE WRENCH SLIPPED OFF THE NUT CAUSING HIS ARM TO JOLT BACKWARDS CAUSING PAIN IN HIS RIGHT SHOULDER.

Struck by rolling or sliding object

EE WAS REMOVING A BUNDLE OF SKIRT BOARDS WITH FORK TRUCK. SKIRT BOARDS WERE BUNDLED TOGETHER WITH A NYLON CHOKER. WHEN THE CHOKER WAS REMOVED, 1 OF THE SKIRT BOARDS (13'6"" LONG X 11"" WIDE) SLID OVER ONTO EE'S FOOT, CAUSING A SEVERE BRUISE. EE WAS WEARING METATARSAL BOOTS, BUT IT CAUGHT JUST ABOVE THE METATARSAL. RELEASED LIGHT DUTY, BUT CAN'T PUT ON STEEL TOE BOOT.
