Employee statement-Checking Dust collector at the re-crush and got a piece of lime into left eye. Attempted to flush myself and was unsuccessful. Got help to flush eye again. Found after going to Urgent Care that I still had a piece of lime in my eye as well as a scratch on my eye.

While closing a manual slide gate beneath a hopper, EE smashed right-hand pinky finger between the slide gate and the hopper, resulting in a distal fracture.

Employee said EE was going up the steps at the Lime Loadout to collect samples when EE felt a ""pop"" in right knee. EE declined medical attention and went home. The next day EE came to work and indicated continued discomfort. EE went to the doctor later that day 8/19/22 and was issued restrictions.
An operator was conducting clean-up of some lime spillage when EE stepped backward into a pile of shoveled-up lime. The lime entered EE's boot. The operator felt a burn around the ankle and went for external care.

Employee was impacting bolts. Socket separated from impact and hit employee in the upper lip, causing a small laceration. Employee received 2 stitches in the upper lip. Returned to work w/no restrictions or lost time.
Employee was using a pry bar, when it got loose from EE's hands between the bar & structure bruising it. No further injury nor prescriptions but doctor did issue restriction to not use due to swelling.
Employee was unplugging chute when employee fell injuring back and ribs.

Employee was coming down ladder on a 980 loader, EE's left foot got caught in 2nd step and EE overexerted EE's right side catching self. Employee did not fall.
Front end loader operator was attempting to clean front corner windshield when the operator lost grip and fell. Upon falling the operator fractured right wrist and was placed in a cast and assigned restricted duty.
A Lab Tech was picking up a pallet to place in the bed of a pickup truck when they heard a pop in left shoulder and felt some discomfort. The Lab Tech went to Urgent care where they were diagnosed with a sprain and provided restricted duty returning to work the same day.
The electrical panel box was not secured before the legs on the bottom were cut off.
The miner exited the cab of a drill and loss of consciousness, falling and slightly injuring shoulder. The miner was seen by a Fire Company medic who checked EE's sugar and vitals, finding nothing wrong. EE was released. The miner was also seen a doctor which treated the shoulder with Ibuprofen and gave EE a full release the day of the mishap.

Employee on Night shift was cleaning up and felt saw some dust falling, when ee looked up to see lime fell into eye. First Aid treatment was administered. The following day ee did not improve and was instructed to go to the doctor. The doctor prescribed antibiotics as a precaution.

Employee complained of discomfort to back and visited personal physician. Strain/sprain to back diagnosed. No specific instance of injury occurred, employee stated increased discomfort throughout week prior to visit.
Employee was taking samples when material was blowing onto back of neck. Employee looked up and material entered eyes.
Employee was cleaning and material entered eyes. Employee washed eye out and felt ""okay"" making no report until 27th of November when ee came to work indicating continued discomfort. Employee was instructed to see a doctor and given prescription medication.
Temp. worker was shoveling and complained of sore back resulting in a strain on second day of work. Was awarded restricted duty but told temp agency they quit the following day.
Employee was attempting to break a piece of one inch sealtite conduit using EE's hands. When the conduit broke, it jumped and hit the employee's mouth, requiring five stitches.
Employee did not report incident until 3-1-16. Employee working with two other employees to install a control box. When they lifted the 30 inch x 36 inch x 8 inch box, employee said felt a pull in lower abdomen. Thinks may have strained a muscle. ***Per e-mail on 3-17-16 was diagnosed with a hernia. Surgery scheduled for 3-24-16.***
Injured worker and another employee were installing a piece of conduit into a panel tub. As the employees were pushing the tub back to slide the pipe into the hole the tub slid and made a cut in the left palm of one of the workers. The injured worker was not wearing cut resistant gloves.
Employee was pulling wire outside, wind was blowing hard and blew lime into employee's eye.
An employee was looking up holding a tagline for a load when EE stepped into a hole, causing EE to bump EE's knee.

An employee painting a piece of 4"" pipe that was laying on a pipe stand when the far end of the pipe was bumped and came off the stand, landing on his upper foot above the steel toe.
Employee was cutting a piece of scrap metal to be disposed of using a cutting torch and it rolled over and struck his right thumb.
Delivery driver had suspected heart attack. CPR and AED were used until EMTs arrived. Driver is a Canadian Citizen from a Canadian trucking company that does not have a SSN, length of time on the job unknown.
On 5/4/15, employee hopped off ladder he had been working on and stepped with his heel of his foot onto a sharp rock or gravel, causing a stone bruise. He continued his normal work until 5/11/15 when it was still sore and he wanted to have it checked out.

Shooting ring at rotary kiln with Remington 8 gauge kiln gun. Bullet ricocheted up over current guarding. The bullet struck ee's left lens of safety glasses causing laceration above left eyebrow. Reportable no lost time accident because of 15 stitches.
Employee working with another mechanic to replace a screen motor at lime processing plant, employee placed his left index finger in a bolt hole to check the alignment. The other employee was prying on base plate, the base plate shifted cutting off end of left index finger.

Employee looking for a water leak in drainage ditch slipped on muddy ground and fell. This resulted in reportable lost time accident.
Employee working in small storage area with 2.5"" aisles when a truck came in needing to be unloaded - employee turned to leave catching his foot on open box falling forward and landing on his right side fracturing two ribs. This resulted in a lost time accident.
EE manually opening a bin gate. Gate was hard to open and employee was pulling and pushing in rocking motion. On the pull stroke the gate broke free causing a muscle strain to right shoulder. EE was taken to Emergency Room at Winchester Medical Center and was given 48 hours off resulting in reportable lost time accident.
Windy day and dust got into left eye under safety glasses. This is a reportable accident because of eye medication.
EE was standing in control room and stretched his arm over head, striking left index finger against sharp edge of light fixture, resulting in laceration requiring stiches. This was a reportable no lost time incident.
EE was assisting with clearing a blockage at Gryatory crusher and feed chute when a slab of rock slid down the chute striking right elbow. This resulted in an MSHA reportable accident because of one day restricted duty.

EE climbed up to the top step of dump truck, approx. 2.5' from ground, to look into the bed to check his load. EE slipped from the step and controlled the fall by grabbing truck handrail with left hand supporting his weight. This is MSHA reportable lost time accident.
EE was unclogging dust pipes at the screen room, lime dust bypassed his safety glasses and entered the right eye causing a chemical burn to right eye.
EE was removing dust collector bags from top access panel, some bags were swelled with material making it difficult to remove, bag assembly was at foot level and EE pulled/jerked assembly upward, EE felt minor pain in his neck but did not think it was bad enough to seek medical attention, the pain continued and EE went for medical attention 6/12/06.
EE opened a manhole to a baghouse hopper and was accidently contacted by dust from the baghouse. Accident still under investigation.

EE was assisting with the removal for the fork attachment on skid steer. EE was using a 5' section of drill steel to break free/striking at the bucket release lever. EE missed the release with the bar overstressing his R shoulder/upper arm, resulting in a MSHA Reportable Restricted Duty Injury.
EMPLOYEE WAS USING A CASE STEER (BOBCAT). THE EE EXITED THE CAB OF THE CASE WHEN THE LANYARD FROM THE HARNESS HE WAS WEARING WRAPPED AROUND THE LT. CONTROL HANDLE CAUSING THE CASE TO ENGAGE THE LT. WHEELS, SPINNING THE EQUIPMENT AROUND CLOCKWISE. THE EE'S RT. ANKLE WAS DRAGGED UNDER THEN TRAPPED UNDER THE LT. SPINNING FRONT WHEEL. A CO-WORKER FREED THE LANYARD.
EE was checking trucks on the ready line. He exited haul truck T332 by use of the ladder. He rolled his left ankle on a rock located on the ground at the bottom of the ladder. He sustained a fracture to the mid-foot and sprained ankle.
Employee was standing waiting for water truck to fill. He stated that he turned to walk back to get in the truck, rotating on his left leg when he heard the knee pop. He reported the injury to his foreman shortly after and continued to operate the truck until taken to receive medical care. Incident resulted in an MSHA reportable injury.
EE STRUCK HIS HEAD AGAINST THE BOTTOM OF THE LOADER FENDER WELL RESULTING IN A 4"" LACERATION REQUIRING 7 STITCHES. EE WAS CLEANING UP AFTER WELDING A PLATE TO THE BOTTOM OF THE LOADER FRAME. EE BUMPED HIS HEAD ON THE FRAME CAUSING HIS HARDHAT TO FALL UNDER THE EXPOSED FENDER WELL. EE RETRIEVED HIS HARDHAT IN HIS HAND & AS HE STRAIGHTENED UP, STRUCK HIS HEAD AGAINST FENDER.

Maintenance EE was assigned to install steps to a conveyor catwalk. The EE cut a piece of angle iron with an acetylene torch. The EE stated that he cut down/off the oxygen supply then hung the lit torch on the mid-rail above his head. The EE proceeded to bolt the angle to the catwalk when the lit torch fell from its position, resulting in 2nd degreee burns to face and neck.
EE FELL FACE FIRST WHEN HE SLIPPED/TRIPPED DOWN A BANK TO A WATER VALVE. EE WAS OBSERVING THE LOADING OF THE WATER WHEN HE NOTICED THE 3"" WATER HOSE CONNECTOR WAS COMING LOOSE FROM THE STAND PIPE ON THE WATER TRUCK. HE HURRIED DOWN A BANK TO SHUT OFF THE VALVE WHEN HE SLIPPED & FELL ONTO THE GROUND. HE SUSTAINED A CONCUSSION/LACERATION TO THE FOREHEAD, NECK SPRAIN, RESTRICTED DUTY.
Ee was assisting quarry foreman to clear blockage from 4"" dust collector approx. 5' off the ground. ee was using a 3/4"" id rubber air hose with metal air hose coupler on the end. coupler wedged inside pipe.He standing to side of dust pipe twisting & pulling the hose while under pressure. hose shot back out of the pipe in wiping action, coupler struck ee
EE was assisting a maintenance crew replace a 48"" by 12"" barrel pulley (120 lbs). EE and maint man were threading and lifting pulley through belt to attach to conveyer frane. EE held end of shaft with left hand and reached for a bolt. The weight shifted to his side, the pulley dropped crushing middle finger and cutting ring finger.

EE WAS TRYING TO MOVE A RAILCAR WITHOUT ASKING FOR SOME HELP. HE WAS IN A HURRY TO MOVE THE CAR AND SLIPPED, FELL, AND TWISTED HIS RIGHT ANKLE.
EE WAS WORKING TO REMOVE BLOCKAGE FROM A #1 SCREEN CHUTE AT THE MAIN CRUSHING. HE WAS USING A 12' CLEANING ROD THROUGH AN ACCESS PANEL. WHEN THEBLOCKED MATERIAL WAS DISLODGED, IT GAVE WAY AND PUSHED THE ROD THROUGH HIS HANDS AND INTO HIS STERNUM.
EE STATED HE GOT IN THE TAIL END OF HIS PICKUP TRUCK TO CHANGE OUT THE OXYGEN TANK. AS HE WAS EXITING THE BACK END, HE SLIPPED AND FELL, HITTING HIS RIGHT ELBOW ON THE TAILGATE OF THE TRUCK. E E STATED THAT THERE WAS SOME WETNESS IN THE BOTTOM OF THE TRUCK.

EE CROSSED SEVERAL LARGE STONES, APPROX. 2'X2' TO BE INA POSITION TO PULL THE PUMP FOOT VALVE OUT OF THE MUD USING A ROPE. WHEN CROSSING BACK, HE PUT THE ROPE ONTO THE BANK AND SLIPPED ON AN A DJACENT STONE, LACERATING HIS MID RIGHT FOREARM.
EE WAS PUTTING WELDING RODS AWAY AND CUT HIS LT.HAND ON A PIECE OF METAL THAT WAS HANGING DOWN BETWEEN SLOTTED WOOD SHELF FROM ABOVE.
