Struck against stationary object

Four associates were heading into the shop. One associate was opening the rollup door and while it still was not all of the way up, EE began pulling down on the door in order to get leverage to push it up. At this point EE walked through and hit EE's head on the door. EE was wearing a hard hat at the time.

While swinging a sledgehammer onto a crusher head, a metal fragment broke off of the crusher & flew into the associate's neck.

Fall from machine

EE was dismounting EE's rock breaker when EE slipped off the track and landed primarily on head and neck.

While using a steel shaft to drive pins from a rock breaker, employee was holding the shaft down on to a jack stand that supported the shaft. Another associate was using a sledge hammer to drive out the pins. The associate missed the shaft & struck employee's left hand which fractured a bone.

Struck by falling object

WHILE REPLACING OLD SCALE DECK, EMPLOYEE REACHED DOWN TO SUPPORT THE EDGE OF A SECTION OF THE DECKING. WHEN THE SECTION OF THE DECK CUT LOOSE, THE PLATE DROPPED DOWN RESULTING IN CRUSHING HIS LEFT MIDDLE FINGER.

Over-exertion in lifting objects

While loading screens onto truck for transportation to the plant, bent & lifted screen with back instead of legs. This caused the back to tighten.

Reporting to work, associate' s foot slipped on frost covered slope shifting his weight to the left thigh resulting in an injury.

Struck against a moving object

Associate noticed that his back became sore sometime during the afternoon as he operated a haul truck. He does not recall any particular event causing the pain. He left the location to go to his doctor thinking it was from a pre-existing medical condition. The doctor determined that he had a bruised rib that could have been caused by operating the haul truck.

Over-exertion in lifting objects

EE was changing out hammer on drill, removing hammer stabilizer. Associate was leaned over to raise the stabilizer and as he lifted, his back was injured.

EE GRINDED OFF PIECE OF METAL FROM GATE BIN, TURNED GRINDER SIDEWAYS, WHERE DISC WAS VERTICAL , PIECE OF METAL CAME OFF AND WENT UNDER SAFETY GLASSES.
