Fall from machine

While pre-shifting the middle deck screen media in secondary station, the associate rolled the chute forward to access the middle deck. EE proceeded to climb while grabbing the side of the screen stepping on metal plate attached to the side of the screen. While doing so EE lost EE's grip/footing falling backwards and landing on angle where the roll away chute rides on rails.

Struck by falling object

We believe that when EE was tightening the side rails on the screen with the impact gun that the metal had fallen into EE's eyes behind EE's safety glasses. The next day, EE woke up with eye pain and went to urgent care to get checked out and they found metal in EE's eyes which they were able to remove.

Struck by flying object

EE was burning the explosives boxes on the bench immediately after the shot. The wind picked up & blew something under EE's safety glasses while back was turned. EE's eye became irritated. EE flushed eye multiple times with no luck. No object or injury to the eye was found during examination, but an antibiotic cream was prescribed.

Contact with electrical current

Employee was working on an electrical panel with push buttons on the front of the door to the panel. Employee checked the wires inside the panel for current and found no current. Employee began working in said panel and didn't realize that the push buttons on the front of the panel were energized on a separate circuit. The door closed on the employee and shocked him, under 120v.

Employee was using a torch to trim a pipe. He was working in some awkward positions from a manlift. Later he noticed a pain in the mid section of his back. He thought it would go away but didn't.

Fall to the walkway or working surface

1 injury; Employee was working at ground level installing an electrical ""can"" when he slipped on a rock, falling back. He tried to prevent impact of the fall by placing his wrist behind him. He landed on his wrist and broke it.

Rigging a beam, forklift was in a tight area. Needed to get forks from under beam in order to lift with hook. Forklift operator completely retracted boom and still couldn't get fork from under beam. He backed up at that time ee leaned forward to get chocker sling; forks came from under beam and hit ee in the forehead.

While picking up an electric hydraulic ram pump, associate twisted back.

Struck by falling object

We had a stack of cribbing timbers to be used for the project. The timbers were 10""x10""x8"". They were stacked 5 high and 4 rows wide with one band around the center. When employee cut the band on the timbers, the first stack toppled over. The timber that was on the top landed on his left foot. The incident resulted in a broken bone on the top of his foot.

Absorption of radiations, caustics, toxic and noxious substances

The individual was checking elevations of a cut section when he fainted and stopped breathing from an allergic reaction to physician prescribed medication. CPR was performed until he awoke and started breathing again. An ambulance arrived and took him to Price William hospital for treatment and observation.

Driller was using a hand held hammer and striking the top head of a down the hole hammer to loosen & remove. a mm size piece of steel went into his left wrist.

Tightening the head nut on the 1560 Omni Cone crusher (P01-0273) at the Bull Run Plant. This procedure requires the use of a sledge hammer. He was swinging the sledge hammer against the tightening tool and when he made contact, the sledge hammer bounced back, hurting his wrist.

Struck by falling object

Employee was hammering on a rock (upper) jaw when dirt fell out of the jaw hitting him in the face. Employee was wearing safety glasses but dirt still got into the employee's eye. The crew was removing the spacer plate from the jaw.
