Employee was dismounting from loader when boot caught on bolt causing employee to loose balance and fall.

Employee amputated tips of two fingers on left hand with a mag drill.

Rock came off of conveyor and hit employee on head while ee was shoveling near conveyor.

Employee was operating the haul truck unit. After having it loaded with dirt he was driving around a turn at the north end of the pit on the haul road. As he rounded the turn he stated he felt the shift in the rear end toward the inside of the turn and as he tried to compensate the truck rolled over.

Employee was pulling a come-a-long to get a wedged rock out of the jaw of the crusher when he felt a pain in both his shoulders

REMOVING/REPLACING DAMAGED MANTLE FROM TELSMITH SILVER BULLET CONE CRUSHER. TAPERED HEAD & SHAFT REMOVED FROM CRUSHER, POSITIONED ON BLOCKING (ATOP OF SAW HORSES). DISTANCE FROM GROUND 3-4 FT. EE TOLD TO CUT BURNING RING & WAIT FOR HELP. EE W/O HELP, LOOSENED NONSTABLIZED SHAFT NUT W/SLEDGE HAMMER. DIDN'T REALIZE HOW LOOSE NUT WAS, SHAFT NUT & WRENCH FELL OFF SHAFT HEAD (100-150 LB

EMPLOYEE THOUGHT HE HAD ARTHRITIS, THEN PAIN AND NUMBNESS WORSENED OVER TIME.

AFTER EE FINISHED CLEANING UP AROUND CRUSHER, WENT HOME THAT EVENING 5/1, HE NOTICED THAT HIS RIGHT EYE WAS IRRITATED. EE DID NOT REPORT THIS UNTIL 5/4 AND HAD MEDICAL (FIRST AID) TREATMENT ON 5/4/00.
