Employee was changing the motor on feed conveyor, using grinder to cut off an old bolt when grinder shifted and made contact with left index finger

EE was removing a bearing from a tail pully, and while striking the bearing race with a hammer, a tiny piece of fragmented steel came off, striking the EE just above the safety glasses in the eye brow. The fragmented piece embedded in the bone and had to be removed.

EE was tasked to clean out a screen box adjacent to the #2 screen, that got overloaded with materials. As they finished, they climbed down the ladder after cleaning off screen component and felt light discomfort in the right knee.

He was installing a new screen. He was pulling and twisting and hurt his lower back.

Removing sampler bracket from product conveyor and strained lower back.

Shoveling OT Belt. It was windy and dusty. Eyes got full of dust and sand. Eye kept hurting through the next day.

Heavy lifting. Tearing out crusher plant, pinched nerve, strain. Sharp pain shooting down leg.

Carpal tunnel in left wrist, ongoing for quite some time. Caused by steering loader - using joy stick with left hand.

Rock chipped and piece of something went into left eye. Went to doctor and he drilled out imbedded peice of metal.

Loading Metal chutes into the Bobcat when he smashed and cut his ring finger on the right hand

climbing into loader, hand slipped off hand rail, right foot wedged between step and bumper of loader

CUTTING BELT, SLIPPED & CUT UPPER LEFT THIGH.

EE WAS SETTING UP CRUSHER & PUTTING IN PROTECTIVE RAILING WHEN IT SHIFTED, HE THEN FELL BREAKING HIS HEEL.

EE WAS ON CONVEYOR BELT TRYING TO LOOSEN A TURNBUCKLE, WHEN IT GAVE WAY HE LOST HIS BALANCE & FELL. HE LANDED ON HIS ANKLE & FRACTURED HIS LEFT HEEL.
