At approx. 2:48 am on Monday, Aug 6, 2007, an earthquake was reported at the mine located near Huntington, UT. At the time of the seismic event, it is believed that six (6) employees of UtahAmerican were working at or near x-c 139 in the west mains.

Employee was lifting belt structure and hurt his low back.

Sweeping floor, during movement, struck a metal file cabinet, which caused laceration to right index finger.

Struck by rolling or sliding object

While mining a lift off of a pillar, the rib sloughed off some chunks of coal and one struck individual in right ankle and foot. This caused a fracture to right foot and a severe sprain to the ankle.

Struck against stationary object

While moving a water pump into water hole, stepped on rock and twisted left knee, did not see a doctor until 3/16/06 for treatment. Treatment will be for strained or torn ligaments with physical therapy and knee brace for support and comfort.

Over-exertion in lifting objects

While installing a timber in the breaker row, individual felt pain in lower back. Individual went to Chiropractor and found to have a back strain. He is receiving treatment for this and has not been released to return to work.

Struck by falling object

While changing bits on the longwall shear, a small piece of rock fell from the face striking individual on the right hand causing a laceration to ring finger that needed 6 stitches to close.

Individual was cutting 3"" water discharge hose with razor knife, when hand and knife slipped and cut the inside of left forearm. The laceration required 6 stitches to close wound.

WHILE CLEANING COAL SPILL, LOADER CAUGHT BUCKET ON ROADWAY LIP. THIS FORCED EMPLOYEE FORWARD IN HIS SEAT & HE CONTACTED THE DASH WITH HIS RT. KNEE. THIS CAUSED A CONTUSION/POSSIBLE FRACTURE OF THE RT. KNEE CAP DIAGNOSED ON 5/25/05. EE WAS SUSPENDED PENDING REQUIRED COMPANY POLICY TEST, RESULTING IN TERMINATION PRIOR TO ANY LOST WORK OR RESTRICTIONS.

During roof bolting of the longwall extraction face, individual was holding up a roof matt when a piece of rock at the top of the coal seam face rolled from the face and struck individual in the left lower leg. This caused a fracture to his ankle.

Individual was letting slack out of a hand cranked boat winch used to support the roof mesh on the extraction longwall face, when he lost his grip on the handle and the handle spun around and struck him in the left wrist causing a bruise. Advise by Doctor not to use hand for 4 - 5 days.

While installing a roof bolt, the individual was rotating the chuck wrench aligning the end of the roof bolt and the chuck wrench. While rotating the chuck wrench, caught his left ring finger between the end of the bolt and chuck wrench. This tore fingernail loose. After examination it was determined that the fingernail needed to be removed.

Accident type, without injuries

A bearing failed on the #4 belt tail piece, causing dense smoke and high CO preventing access to the immediate area. After the belt was stopped, the bearing heat burned the belt into and started the inpact bed burning. The area was accessed under apparatus and the fire on the belt ends and inpact bed was extingushed within 10 minutes.

While loading timbers into the bucket of a scoop felt pain and pulling in lower back. Determined to be an acute lumbar strain. Unable to work for 10 days.

Individual was reaching to remove a chain used to transport a longwall shield located within a shield haul vehicle, when the chain lift arm was activated to lower the shield pinching individual's right upper arm (humorous). The pinching of the lift arm fractured the individual's right upper arm.

Struck against stationary object

While adjusting the surface loadout sensing magnet on the gates, individual pulled his hand away from magnet and struck a piece of stationary metal causing a cut to right index finger, which needed stitches.

Inhalation of radiations, caustics, toxic and noxious substances

After spraying some stoppings with Versa Foam polyurethane sealant, individual experienced shortness of breath and nausa. Taken to hospital and Doctor felt was having a reaction to the chemicals used in the foam. Doctor requested follow-up examination in two days for release to return to work.

While walking across a cement pad to a surface belt transfer pad, individual stepped on uneven area of the cement and twisted right ankle. Seen by Dr and treated for sprain and was not allowed to return to work for three days.

While helping install roof bolts, the injured individual was holding the wire mesh screen. The other operator raised the ATRS against the mine roof and the individual who was holding the wire mesh had his right hand pinched between the ATRS frame and the mine roof. This caused lacerations to all four fingers of his right hand and fractured the 3rd and 4th fingers.

WHILE CUTTING SOME TAPE OFF THE END OF A SHUTTLE CAR CABLE FOR MAKING A SPLICE. THE RAZOR KNIFE SLIPPED & CUT LEFT HAND. THIS CAUSED A 1/2 INCH LACERATION THAT NEEDED 4 STITCHES TO CLOSE.

WHILE DRILLING HOLES IN ROOF TO BLAST FOR AN OVERCAST, INDIVIDUAL WAS USING A DOUBLE-BOOM LEE NORSE ROOF BOLTER. INDIVIDUAL MOVED BOOM TO REPOSITION AFTER LOWERING BOOM'S STAB JACK. THE CANOPY AND BOOM ROSE, PINCHING HAND AGAINST ROOF CAUSING A SMALL LACERATION BETWEEN 1ST & 2ND FINGERS THAT NEEDED STITCHES ON RIGHT HAND.

WHILE SERVICING THE CONT MINER, INDIVIDUAL LIFTED A NEW BUCKET OF MINER BITS ONTO THE TOP OF THE MINER AND FELT SOMETHING PULL IN HIS LEFT CHEST. DIAGNOSED WITH POSSIBLE DISLOCATED RIBS FROM S TERNUM. TOLD TO STAY OFF WORK FOR 2 WEEKS.

WHILE ROOF BOLTING WITH A STOPER ROOF DRILL, INDIVIDUAL HAD JUST COMPLETED INSTALLING THE ROOF BOLT. WHEN HE WENT TO LOWER HIS ARM IT WOULD NOT COME DOWN. INDIVIDUAL PUSHED AND FORCED ARM DOWN , FELT POP & PAIN. FOLLOWING AN MRI & DR'S APPOINTMENT ON 6/16/03 FOUND HAD SMALL TEAR TO ROTATOR CUFF IN RT SHOULDER. WILL TREAT W/THERAPY. HAS NOT MISSED ANY WORK, REPORT DUE TO POSITIVE DI

INDIVIDUAL INITIALLY HURT KNEE BY TWISTING IT WHILE PULLING ON LONGWALL FACE. ON 5-22-03 WHILE WALKING, INDIVIDUAL TURNED AND KNEE GAVE OUT AND LOCKED UP. SAW DOCTOR AND DOCTOR FELT SOME SCAR TISSUE WAS IN THE JOINT OF KNEE AND WOULD REQUIRE SURGERY TO REPAIR.

WHILE WALKING IN THE BLEEDER ENTRY, SLIPPED AND FELL ON TO RIGHT KNEE. THIS CAUSED A SMALL CHIP FRACTURE TO HIS RIGHT KNEE CAP.

Absorption of radiations, caustics, toxic and noxious substances

WHILE LOOKING FOR A LEAK IN A HYDRAULIC HOSE ON A ROOF BOLTER, INDIVIDUAL WAS HOLDING HOSE BY DAMAGED AREA. THE HOSE WAS PRESSURIZED FOR IDENTIFICATION AND A SMALL AMOUNT OF HYDRAULIC OIL WAS INJECTED INTO TIP OF RIGHT INDEX FINGER REQUIRING THE FINGER TIP TO BE LANCED TO ALLOW THE OIL TO DRAIN.

WHILE REMOVING A WHEEL BLOCK FROM BEHIND THE WHEEL OF A PIECE OF EQUIPMENT. THE INDIVIDUAL SQUATTED DOWN & GRABBED THE BLOCK AND SLIPPED ON SOME ICE FALLING ON LEFT SIDE. THE EE LANDED ON LT. HIP AND LT. SHOULDER, CAUSING BRUISE AND STRAIN TO LEFT SHOULDER AND QUESTION OF TORN ROTATOR CUFF.

INDIVIDUAL SAT ON A RELAY BAR THAT WAS BEING SUSPENDED FROMT HE MONORAIL BY CHAINS. AS HE SAT ONT HE BAR THE CHAIN BROKE AND THE RELAY BAR FELL ON HIS LEFT LEG FRACTURING THE LOWER PART OF HIS LEG. THIS BAR WAS ON THE BELT LINE OUTBY THE TAILPIECE.

WHILE OPERATING THE TAILGATE END OF THE SHEAR CUTTING COAL TOWRDS THE TAILGATE OF THE LONGWALL FACE, A BOUNCE IN THE FLOOR OCCURRED. THIS KNOCKED THE INDIVIDUAL DOWN WHERE HE STRUCK HIS HEAD A ND UPPER LEFT BACK ON BAS LIFT CYLINDER BRACKET. THIS CAUSED A BUMP ON HIS HEAD AND XRAYS SHOWED A SMALL PIECE OF BONE CHIPPED OFF THE END OF THE PROCESS OF HIS T-1 VERTEBRA.

WHILE WALKING DOWN THE LONGWALL FACE, STRUCK HEAD AGAINST SHIELD JAMMING NECK. INDIVIDUAL SAW DR. AUG.7,2002 SCHEDULED FOR MRI. REVISIT TO DR.ON DEC 9,2002 TO GET MRI RESULTS. NOW NEEDS SURGERY ON NECK FOR DISC HERNIATION. SURGERGY SCHEDULED FOR 12-17-02.

WHILE HELPING REPLACE THE HAMMER TIPS IN SURFACE CRUSHER, THE TIP OF THE INDIVIDUAL'S FIFTH LEFT FINGER WAS PINCH BETWEEN CRUSHER ROLLER AND HAMMER TIP HOLD DOWN ROD WHILE IT WAS BEING INSERTE D. THIS CAUSED A LACERATION AND SOME TISSUE LOSS TO THE TIP OF THE FINGER REQUIRING STITCHES AND DEBREDMENT ON END OF FINGER.

AS THE CHUCK WRENC H CONTACTED THE OF THE ROOF BOLT, THE JOINT MOVED AND PINNED INDIVIDUALS THUMB BETWEEN CHUCK WRENCH AND DRILL GUIDE BRACKET, CAUSING A 1/4"" CUT ON RIGHT THUMB NEEDING 2 STITCHES TO CLOSE.

WHILE TRAVELING TO THE TAILGATE OF THE LONGWALL TO OBSERVE CONDITIONS, A SMALL BOUNCE OCCURRED, WHICH CAUSED A LUMP OF COAL TO COME OVER THE BACK OF THE PANLINE, STRIKING INDIVIDUAL IN RIGHT L OWER LEG CAUSING A FRACTURE TO RIGHT ANKLE.

COMPANY WAS NOTIFIED OF SCHE DULED SURGERY ON 5/14/02 FOLLOWING DOCTORS APPOINTMENT AND MRI. THIS IS NOW A LOST TIME AND REPORTABLE INJURY. INDIVIDUAL RECEIVED NO TREATMENT UNTIL THIS TIME. SURGERY 5/20/02.

THIS CAUSED A LACERATION TO THE SIDE OF RIGHT LOWER LEG REQUIRING STITCHES.

ON 12/22/00 SAW DOCTOR FOR X-RAYS DUE TO CONTINUED PAIN, DIAGNOSIS SPRAIN. ON 4/11/01 INDIVIDUAL SAW DOCTOR AGAIN NOW HAS SUSPECTED LIGAMENT TEAR. SCHEDULED FOR SURGERY ON 4/23/01.

AS THE PROPMAN WAS TRAVELING TOWARDS THE HEADGATE OF THE LONGWALL, THE FACE YIELDED AND A LARGE LUMP OF COAL CAME OVER THE PANLINE STRIKING THE INDIVIDUAL ON HIS LEFT SIDE KNOCKING HIM BACK IN TO THE SHIELDS. THIS OCCURRED AT SHIELD 115-116 WHERE THERE ARE 147 SHIELD ON THE FACE. THIS CAUSED A FRACTURE OF THE RIGHT FEMUR.

WHILE EXITING A SKID STEER LOADER, CAUGHT FOOT ON BUCKET EDGE AND TRIPPED AND FELL CATCHING THEMSELF ON RIGHT HAND. HE SAW DOCTOR ON 9-7-00 AND WAS DIAGNOSED WITH PAIN IN HIS RIGHT WRIST, AND A POSSIBLE FRACTURE.

WHILE REMOVING THE NAIL THAT WAS HOLDING THE DRILL BIT ON THE DRILL STEEL, HE SLIPPED WITH HIS HAND TOOL (DIKES) AND STRUCK HIMSELF IN THE MOUTH. THIS CAUSED A LACERATION TO HIS LOWER RIGHT L IP REQUIRING 5 SUTURES.

WHILE MOVING THE LONGWALL TAILGATE DRIVE WITH A WAGNER 25X THE INDIVIDUAL WALKED TO THE SIDE OF THE SCOOP AS IT WAS BEING TURNED AND THE ENGINE END OF THE SCOOP STRUCK HIM AND CAUSED A FRACTUR E TO HIS LEFT FEMUR.

WHILE PREPARING TO CONNECT HOSE TO ROCK DUST TANK OUTLET, THE 4"" COVER CAP THAT WAS RESTRAINED BY A CHAIN WAS REMOVED. THE TANK STILL HAD PRESSURE IN IT AND THE CAP BLEW OFF WHEN THE CLAMPS WE RE REMOVED. THE RESTRAINED CAP FLEW AROUND AND HIT A GLANCING BLOW TO NOSE, CHEEK LT, AND UPPER LIP CAUSING SWELLING BRUISES AND ABRASIONS. THE CONTENTS OF THE TANK (ROCK DUST) BLEW INTO EYES

AFTER DOING SOME SHOVELING ON SOME SPILLED COAL ALONG THE BELT LINE, EE COMPLAINED OF A SORE BACK WITH SPASMS. THIS CONDITION HAS OCCURRED 4 TIMES BEFORE WITHOUT HAVING CONDITION REPAIRED. THI S IS A CRONIC PROBLEM. SEE DR.'S NOTE ATTACHED.
