Struck by... (Not Elsewhere Classified)

Miner was holding a timber while setting a breaker row. Another miner swung the axe to hit the cap piece and the axe glanced off and struck the victim on top of the hard hat.

2025 · 11 incidents

Over-exertion in lifting objects

EE strained groin area while loading heavy items into a truck on 10/26/2025. EE reported injury but refused to see doctor and continued working until 11/9/2025. Examined by doctor on 11/10/2025. Diagnosed with a Hernia on the right side

Contact with hot objects or substances

EE burned the palm of EE's left hand when EE unknowingly placed it on the surface of a hot metal work bench. Another EE had been working metal, with torches, on the bench a few minutes before the injured EE walked up to talk to EE. EE was taken to Clinic and examined & treated for 2nd degree burns then released to return to work.

Struck by... (Not Elsewhere Classified)

EE was using a battery powered hand grinder to cut metal pins. The cutting wheel stuck on the metal, causing the grinder to kick-back, coming in contact with the EE's left thumb. EE sustained a laceration that required 6 stitches. EE received a work release, giving time for the laceration to heal.

Fall to the walkway or working surface

10/2/25 EE working on surface when a Front-End Loader ran over a wooden pole. The pole swung knocking EE to the ground. EE hit EE's head & R-shoulder on ground. EE reported incident but continued to work EE's normal shifts until 10/6/25, then requested to have right shoulder checked. Doctor excused EE from work until cleared by MRI.

Fall down stairs

9/30/25 while walking up steps EE tripped and reached out to catch self, but missed and fell, striking EE's face and left shoulder on the handrail and steps. EE continued to work EE's normal shifts until 10/7/25, when EE requested to see a doctor to have EE's left shoulder checked. Doctor excused EE from work to do Physical Therapy before returning to work.

Struck against stationary object

While unclogging drill steel wrench by hitting it on the roof bolter frame, the EE swung the wrench, and the back of EE's right hand made contact with a roof bolt plate located on the supply tray. EE sustained a laceration to the back of the right hand. EE was taken to the Clinic and received stitches to close the laceration. EE was released to return to work.

Over-exertion in pulling or pushing objects

Roof Bolting, EE was bending bolt to get it in roof. While pulling back on the bolt, they felt a pop in their right wrist. EE did not want to see a doctor and continued to work. Six days later - went to doctor. Diagnosed with Wrist Tenosynovitis and was released to return to work. After working 2 more days, EE went back to doctor (5/8/2025). EE is off work waiting for an MRI test.

EE was in the surface fabrication shop. EE was holding a ¾"" pipe as a punch, to push a keeper pin into place. Another EE used a pipe, as a hammer, to hit the pipe EE was holding. When the pipes made contact, the pipe being swung ricocheted and struck the small finger of the EE's left hand. This caused a laceration that required stitches, and a fracture to the finger.

Caught in, under or between a moving and a stationary object

EE was in the surface shop pulling a rolling tool cart behind EE. The cart caught the heel of EE's left foot, causing the cart to run up the back of EE's heel and put weight and pressure on EE's foot. X-Rays showed a fracture to the big toe's metatarsal bone of EE's left foot.

Fall from ladders

EE on ladder to foam top of Vent Line Curtain. Ladder moved & EE fell to mine floor. EE landed on feet, but left leg folded under EE upon impact. Doctor referred EE for MRI to diagnose if any damage to ACL. MRI was not available till 2/27/2025 and results not received till 3/6/2025. MRI was normal but was diagnosed with knee sprain and referred for PT.

EE was using a battery hand grinder. The tool slipped and when grabbing, EE hit the trigger. This caused the grinder to flip around. The spinning grinding wheel cut the back of EE's left hand, causing a laceration that required 9 stitches. It also chipped a finger bone & ligament. EE was referred to an Orthopedic Doctor.

Over-exertion (Not Elsewhere Classified)

After the EE finished the shift, EE was walking from the bathhouse to EE's car. EE stepped wrong and twisted their right ankle. EE thought was fine and went home. When EE arrived at home, EE's foot was hurting. EE was taken to the doctor and examined. An X-Ray showed a fracture to the 5th Metatarsal (small toe).

Accident type, without injuries

A Roof Fall occurred immediately to the left of the Lift that the Continuous Miner was in, and just outby the Mobile Roof Support units. In the #3 Entry of the 4th West Pillar Section (just inby XC #19). It was not above the anchorage zone, and only about 4 feet high, 10 feet long and 14 feet wide. No one was hurt or entrapped and no ventilation was damaged.

Over-exertion in lifting objects

On 10/24/2024 an EE was hand loading Roof Mats, by self, and felt a pop and pain in EE's right biceps muscle. EE was examined at the Clinic and an MRI was ordered for diagnosis. An MRI was not available until 10/30/2024. EE was diagnosed, after the MRI, as having torn tendons/muscles in EE's right biceps.

Struck by falling object

EE used 16-inch screw bar to hang Ventilation curtain. The screw bar slipped out of EE's hand and fell, striking EE above the left eye, causing a small laceration. No treatment needed. Finished shift and went home. Next day EE did not feel good. Went to the ER and was Diagnosed with a Concussion. Doctor prescribed days off.

Accident type, without injuries

A Rock Slab from the Surface Highwall slid down and covered the #1 and #2 Portal Openings. The size of the Rock Slab that slid down was approximately 3 to 5 feet thick, 180 feet long and 30 feet high. All miners were evacuated via the #3 - Right Intake Portal. All miners were accounted for and no injuries were reported.

Accident type, without injuries

Unplanned Roof Fall - Outby Intake entry of South Mains (Entry 5 between XC #6.75 and XC #7). It occurred sometime between 2 & 2:10 PM. It is about 3 feet high, 20 feet long and rib to rib, in the Primary Escapeway. No one was hurt or entrapped and no ventilation was damaged.

Caught in, under or between a moving and a stationary object

While helping change out a pump motor, EE was moving the motor into place and got EE's left ring finger caught between the motor and the frame. EE sustained a laceration to EE's left ring finger that required 8 stitches. EE was treated and released to return to work.

Caught in, under or between a moving and a stationary object

EE was unhooking a trailer from EE's machine. EE was throwing a wood crib block under the front of the trailer to prop the trailer up. When EE threw the crib EE smashed right index finger between the crib and the trailer. Sustained a laceration the required 5 stitches and a fracture to the distal joint.

Struck by falling object

EE injured while servicing, changing oil, on Co #308 / DST 07 LHD Loader. EE was reinstalling the belly pan when it slipped and fell, smashing EE's left ring finger between the belly pan and the ground. EE sustained a laceration and Distal fracture to the finger. EE received stitches, finger splint and antibiotic.

Struck by falling object

An EE was injured when EE stepped off the Roof Bolter Operator Platform, with the platform raised about 2 feet off the mine floor. EE reached up to the bolt tray to retrieve the Drill-Steel and it slipped off the tray and struck EE in the face, causing a laceration to the left side of lip/cheek. It required 2-stiches to close the laceration.

Accident type, without injuries

Roof Fall - 3rd West Retreat Mining Section (Entry 5 inby the Pillar Line) between XC #7 & #8. About 15' wide by 15'long and 3' high. NOT at or above the anchorage zone, fell on top of a battery scoop. One miner was entrapment in the cab of the scoop. The miner escaped with no injuries. Preliminary diagnoses; Strata failure due to weight over-ride from retreat mining in Entry 4.

Absorption of radiations, caustics, toxic and noxious substances

EE injured while removing a hose from a Fletcher MRS leg cylinder ram. EE inadvertently hit a fitting on the check valve, next to the hose EE was removing, with a wrench. The fitting failed and released high pressure oil that hit the EE's right hand, causing a laceration and injection of oil. A surgical procedure was required to clean the oil out.

Caught in, under or between a moving and a stationary object

EE injured operating continuous miner. EE split the tracks to turn the CM to the left and was struck on the left hip by the CM Tail Boom and then caught between it and the mine Rib. EE sustained contusions to both hips, no evidence of broken bones. Later, during EE's stay in hospital, EE noticed pain in right hand. X-Rays show a possible small bone fragment at a metacarpal base.

Fall from machine

Climbing into the back of a lube truck to get a 5-gallon can of oil. Placed foot on ball of the hitch and pulled myself up. My foot slipped off the ball and I fell backwards onto the ground, landing on the forks of the telehandler. Right arm went behind me as I fell and I landed on it.

Struck by falling object

EE was injured when handling a belt roller. The roller fell and struck the back of EE's hand. EE sustained a contusion and laceration to the back of the left hand.

Accident type, without injuries

An unplanned Roof Fall occurred at an unknown time. Right Return Air Course of Main West, Entry #6 XC 21. Approximately 5 ft. wide by 6 ft. long and 4.5 ft high. It is NOT at or above the anchorage, but has impeded travel. No injuries, entrapment and no ventilation interruption. Preliminary diagnoses; Strata failure due to deterioration of the seam transition area.

Over-exertion in lifting objects

EE was injured when lifting a bottom belt roller up onto their shoulder. They felt a pop and pain in their chest and left shoulder. EE has a pre-existing injury that is non-work related. EE was diagnosed to have strained their Sternum and Left Shoulder. EE was given a Modified Work Release.

Over-exertion (Not Elsewhere Classified)

EE was injured after exiting a Skid Steer. They jumped out of the cab instead of using 3 pints of contact. When they landed on the mine floor, they twisted their right ankle. X-rays were negative. Diagnosis is sprain/strain to the Right Ankle. EE's ankle was wrapped and they were put on crutches and given modified duty until follow-up appointment.

Fall onto or against objects

An EE was injured when they stepped back on the Roof Bolter Operator Platform, and missed footing. They fell between the platform and the TRS Scissor-lift frame. They struck the left side of their face on the frame, causing a laceration to the left side of their face. It required 3-stiches to close the laceration.

Over-exertion (Not Elsewhere Classified)

EE was injured when descending a ladder. They misplaced their right foot and only caught the ladder rung with the tip of their boot. This caused their ankle to hyperextend. They felt pain in their right foot and knee. X-rays were negative. Diagnosed with sprain and given 3-days off work. After the 3-days EE still having pain in knee and has an appointment to see doctor.

Inhalation of radiations, caustics, toxic and noxious substances

EE became ill, nauseas and light headed, while operating a diesel powered scoop in the face. EE finished the shift. The next day EE returned and worked EE's shift, stating EE still felt nauseas. 2-days after incident, before coming to work, EE went to doctor and was diagnosed with CO poisoning. The EE self-terminated EE's employment the same day.

An employee was injured while shoveling accumulations in the belt entry. EE used the shovel to knock accumulations off the structure. EE's shovel caught a roller and flipped the shovel back at him. The shovel struck EE on the chin, causing a laceration. It required stitches to close the laceration.

Struck against stationary object

EE was injured while tending Shuttle Car Cable. EE bent over to grab the cable, at the same time the operator started the machine. The Hydraulic power to the cable reel was on, causing the cable to pull tight. This caused the cable to come up off the mine floor and the cable struck the EE on the chin. The EE sustained a laceration under the lower lip that required stitches.

Accident type, without injuries

An unplanned Roof Fall was found at 6:50 AM in the common Intake of 3rd West. In Entry #3 at XC #35.5. It is 35 ft long by 20 ft wide and 5 ft high. It is at the anchorage zone and impedes travel. No one was injured or entrapped. Ventilation was not interrupted. Preliminary diagnoses; strata failure due to roof support not having good anchorage in competent top.

Struck by... (Not Elsewhere Classified)

An EE was injured while cutting through plastic with a razor knife. The knife slipped out of control and EE cut left leg, just above the knee. It required 10 stitches to close the laceration.

Fall to the walkway or working surface

An EE was injured when EE slipped and fell while walking down an overcast ramp. EE twisted left ankle. EE was diagnosed to have sustained soft tissue damage to the left ankle, requiring modified duty and the use of crutches.

Caught in, under or between a moving and a stationary object

An EE was injured while removing a keeper pin out of a track pad on a continuous miner. While hitting the keeper with a hammer the hammer slipped and smashed EE's left thumb, between the hammer and the track pad. It required stitches to close the laceration. X-rays also show that the thumb has a small fracture, requiring modified duty.

An unplanned Roof Fall was found at 11:00 AM in the Return of Main West, Intersection of Entry #1 & XC #22. It is 20 ft by 10 ft and 4 ft high. It is below the anchorage zone, but impedes travel. No one was injured or entrapped. Ventilation was not interrupted. Preliminary diagnoses; strata failure due to seam transition area.

Accident type, without injuries

An unplanned Roof Fall was found in the Intake Air Course of 3rd North, Intersection of Entry #9 & XC #7. It is approximately 20' wide by 20' long and 7' high. No one was injured or entrapped. Ventilation was interrupted. Preliminary diagnoses; Strata failure due to water saturating the upper mudstone band, causing it to swell and weakening the Roof Control.

Fall onto or against objects

An EE was injured while moving a water pump. They slipped on the muddy mine floor and struck their jaw on top of the pump. Diagnosis was a fractured tooth that required dental work.

Accident type, without injuries

A water inundation occurred due to a Flash Flood from a rainstorm. The Flash Flood washed out a dam in the drainage system, on the surface, which allowed water to enter the mine. The water created holes in four stoppings, one Overcast and one Box Check. No major air changes occurred and no one was injured or entrapped.

An unplanned Roof Fall occurred at 3:45 AM in the Intake Air Course of Main West, Entry #2 XC 21. The cave is approximately 18 ft. wide by 10 ft. long and 6 ft high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; temperature change caused air-slack and Strata failure around supports.

An unplanned Roof Fall occurred at an unknown time in a Return Air Course of 2nd West, Entry #6 XC 19. The cave is approximately 20 ft. wide by 25 ft. long and 10 ft high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; Strata failure due to water saturation of seam transition area.

Over-exertion in lifting objects

An EE was injured while lifting a 5 gal can of oil onto the bed of a pick-up. EE felt a stretch/rip and a bump next to EE's belly-button. The Hospital ER made an appointment for the EE to see a general surgeon. Nine days later the general surgeon diagnosed the EE with a Hernia. Surgery was scheduled 2 weeks after the appointment with the surgeon.

Struck against a moving object

An EE was injured while operating a battery scoop. The service brake did not work properly. EE panicked and hit the E-stop, killing all the power and controls. The scoop rolled forward and hit the rib. EE hit head on the cab, sustaining a neck strain.

Over-exertion (Not Elsewhere Classified)

An EE was injured when attempting to hang continuous miner cable to the mine roof. The hook EE was using to attach the cable to the roof screen slipped and the cable fell down. The EE was holding onto a drill steel EE was using to hang the cable when it fell. The jerking motion injured the EE's right shoulder. Diagnosis was a Sprain/Strain.

Struck by falling object

An EE was injured installing a rear axle in a Mantrip. With a floor jack holding one side & a ratchet strap holding the other side, about 2 feet in the air. EE attempted to lower it with the ratchet strap and it released and fell, the axle struck EE on the right lower leg. Diagnosis was a contusion.

Fall onto or against objects

An employee was injured while installing/working on the communication system. The employee tripped and fell face first into the mine rib. Employee's hard hat hit the rib and slid down striking employee's glasses and nose. Diagnosis was a laceration and fracture to the nose.

Struck by falling object

An employee was injured while attempting to replace a heavy mechanical part on a continuous miner. When attempting to move it into place, the part sild and fell, striking right ankle. Diagnosis was a right ankle sprain / contusion.

Accident type, without injuries

An unplanned Roof Fall was found by MSHA, in 2nd West Return XC #17.25 to #18.5 of Entry 7. Approximately 20' wide and above the anchorage point. No one was injured or entrapped. It is not in a regularly traveled entry and it is common with another Entry. Preliminary diagnoses, strata failure due to water saturation.

Struck by powered moving object

A miner was injured when they walked into the Red Zone of a High Voltage Skid that was being moved. Their foot was caught between the High Voltage Skid and the mine floor. They sustained a contusion to the right foot.

Accident type, without injuries

An unplanned Roof Fall occurred in Main West Intake XC #19.5 (#2 Entry - Primary Escapeway). The cave is approximately 10 by 10 and 4 high. No one was injured or entrapped. Preliminary diagnoses; 2 supplemental supports in the form of Steel Sets and Beams were unknowingly hit by a trailer, causing steel sets to fall over. This caused the roof support to fail.

Accident type, without injuries

An unplanned Roof Fall occurred - 1st West Pillar Section between XC 17 & 18 - #9 Entry, inby the loading point. Approximately 17 by 20 and 4.5 high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; Cable Bolts were being installed to replace Floor to Roof Supports. When 2 timbers were knocked out with the ATRS the fall occurred.

Accident type, without injuries

An unplanned Roof Fall occurred in the 1st West Pillar Section, in the #5 Entry - Intersection of crosscut #19 (Pillar Line). The fall is 20'x20'x 8' high. No miners were injured or entrapped. No ventilation was interrupted. 4-MRS units are inby the fall. Preliminary diagnoses; The Intersection was developed too wide which caused the strata to fail.

Struck by falling object

A miner was injured when they were handling a belt structure Top Roller. Miner tripped & fell to the mine floor. The roller they were carrying struck them on the left shoulder as they fell. Miner sustained a deep bruise to the left shoulder.

Struck by rolling or sliding object

A miner was injured when a chunk of coal, approximately 3' tall by 10' long & 6' thick, slid vertically down the rib and tipped forward striking miner's left leg and knocking miner down. Miner sustained a fracture to left leg.

Caught in, under or between running or meshing objects

A roof bolt operator was injured when placing EE's drill steel into the drill box chuck. EE inadvertently hit the rotation control lever while holding the drill steel. The glove on EE's left-hand stuck to the drill steel and EE's hand/fingers were twisted around with the steel. One finger sustained a fracture, laceration and dislocation, & the other finger was dislocated

Over-exertion in lifting objects

Three miners were team lifting a steel I-Beam. When lifting, one felt a pain in EE's lower back. EE has a pre-existing back injury. EE was diagnosed with a back strain/sprain.

Fall onto or against objects

While unloading material from off of a trailer, a miner was injured when the miner slipped on the muddy mine floor. Miner struck right elbow on a sharp piece of stopping material that was on the trailer, causing a laceration .75 inches long. The laceration required 3 stitches.

Accident type, without injuries

An unplanned Roof Fall occurred at an unknown time in the West Mains Section between XC 17 & 18 in the #6 Return Entry. The cave is approximately 18.5 ft. wide by 30 ft. long and 8 ft. high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; Strata failure due to seam transition area.

Accident type, without injuries

An unplanned Roof Fall occurred in the 3rd North Section, in the intersection of #6 Entry and crosscut #9, outby area. The area that caved is approximately 20 sq. ft. and 9 ft. high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; Primary Roof Support did not have the resin the length of the bolts as required by the Roof Control Plan.

Struck by falling object

A miner was injured when they accidently bumped against the mine rib causing a chunk of coal, approximately 5"" wide by 4"" tall & 8"" thick, to slide vertically down the rib. The piece of coal struck right ankle, spraining it, and right foot was caught between the chunk of coal and the mine floor.

A miner was injured when miner was bent down looking for a lost drill bit that had fallen under the boom of the bolter. When miner began to stand up miner's foot was stuck in the mud, making miner's leg twist as miner stood. This caused left knee to pop out of joint. When miner was able to stand up straight the knee popped back into place.

Caught in, under or between a moving and a stationary object

Employee was laying on the mine floor, repairing the transmission of a pick-up truck. The transmission was accidently shifted into gear and the rear tire ran over the employee's stomach & the rear axel struck the employee's face causing a Chest Wall Contusion with Internal Bleeding, Pneumothorax (collapsed lung), back fractures (T5 - T10) & small lacerations & bruising to the face.

Over-exertion in lifting objects

While installing belt structure, a miner lifted a 48"" bottom roller into place and felt a pop in the back of EE's right hand. Soon after EE's middle fingers went numb. An X-Ray on day of injury was negative. Doctor requested an MRI. Workers Comp took 7 days to approve MRI. MRI done on 03/22/2021 and results received 03/25/2021 showing 2 torn ligaments. Possibly requiring surgery.

Accident type, without injuries

A Roof Fall occurred in 2nd West in the intersection of XC #33 in Entry 5, and extends inby & into the XC towards Entry 4. No one was entrapped and no ventilation was interrupted. It affected approximately 44 by 40 and is 8 high. Preliminary diagnoses, area was submerged under water during an inundation, saturating the strata, causing it to fail. Area Dangered-off.

Fall to the walkway or working surface

A miner was injured, due to a fall, while EE was carrying supplies. EE fell when EE stepped on a piece of conveyor belt fabric that was wet and lying flat on the mine floor. EE's left leg sustained a fracture to the Fibular bone.

Accident type, without injuries

A water inundation originated outby XC36 - #9 entry of 1st West Pillar Section. The water has inundated the bottom of 2nd West starting at XC38 and continuing inby through XC40. The water prohibits travel and ventilation of the Left-side Return Air Course of 5th North and 2nd West Sections. No miners were injuries or entrapped.

Accident type, without injuries

Roof Fall occurred 2nd West, #2Entry @ Intersection of XC #35 (Outby area). Appears 20' x 20' & 7' high. No one was injured or entrapped and no Ventilation was interrupted. The diagnoses are that the Intersection was wide, (67' sum of the diagonals), and water dripping from a test hole saturated the upper Mud Stone strata, allowing it to fail. New Support has been installed.

Accident type, without injuries

A Roof Fall occurred in 2nd West @ the Face-Intersection of XC #40 in Entry #4. It appears to be about 7' height, 19' wide and 35' long. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses is that vertical cracks in the strata, parallel with the crosscuts, allowed the strata to fail. New Primary & Supplemental Support has been installed.

Over-exertion in lifting objects

While installing belt structure, the miner lifted a 48 inch Top-Roller cluster and felt a ""pop"" in the left shoulder. Soon after EE felt left hand and fingers go slightly numb.

Accident type, without injuries

Roof Fall in 1st West at the Intersection of #6 Entry and XC #23. It is about 38' L x 19' W & domed up to 19.5' H. Water is pouring out of 2 places & dripping in others. It appears the excessive water pressure caused the strata to fail. No injuries or entrapment. Three ventilation stoppings were damaged. New Primary & Supplemental Support was installed.

Flash burns (electric)

High wind caused a suspended ground monitor wire to come unattached from a pole. The winds caused the wire to swing close enough to conduct power from 7200-volt lines above it. This allowed power to back-feed to an electrical box being worked on inside of the sub-station. An electrical arc flash occurred that injured a miner causing Superficial burns to EE's face & eyes.

Accident type, without injuries

A Fall was found in 2nd West in the #6 Return Air Course, between crosscuts #10 and #11. The area is approximately 36' L, 18' W and domed up to 6 ½' H. No miners were injured or entrapped. No Ventilation was interrupted. The area has been 'Dangered-Off' and Supplemental Roof Support has been installed. The area appears to have become saturated with water, causing the failure.

Struck by... (Not Elsewhere Classified)

When working on a Continuous Miner, an employee had the top Rub-Rail next to the Scrubber swung out. Employee was raising the Scrubber cover with the hydraulic controls when the Scrubber Lid hit the Rub-Rail, causing it to swing. The Rub-Rail struck employee across the face. The struck-by injury resulted in a Laceration just below the left side of Nose that required 4-stitches.

Caught in, under or between a moving and a stationary object

Employee was recovering belt structure from an old belt line. While removing a bottom roller, they pulled on the roller handle and as the roller fell downward it struck a 4-inch water pipe. The employees middle & ring fingers on left hand were smashed between the handle and the pipe causing lacerations to both fingers and a bone fracture to the middle finger.

A Fall occurred in 1st Right - Crosscut 10 between entries #5 & #6. It is approximately 18' wide, 60' long with 2' to 7' of top falling. No one was injured or entrapped. No Ventilation was interrupted. The Fall was Dangered-Off and timbers were installed to prevent access. The Roof Bolts anchored below a 1' thick rock band that appears to have failed due to water saturation.

Accident type, without injuries

A Roof Fall occurred in the Main West #1 (Return) Entry, just inby crosscut #18. The fall is 16 ft. wide by around 60 feet long, and around 6 ft. high in the highest spot. No one was injured or entrapped and no Ventilation was interrupted or blocked. Access was physically blocked. A 103(k) Order was issued. When approval was given, Timbers were installed on both sides.

A Roof Fall occurred in the Main West #2 Entry, in the intersection of Crosscut #18. The fall is 20' Sq. with 1' to 5' of top falling. No-one was injured or entrapped. No Ventilation was interrupted or blocked. The Fall was Dangered-Off. Timbers &/or Roof Jacks were set to prevent access till the area can have new Permanent Roof Supports installed.

2 miners were building a block stopping. 1 miner was on a ladder placing the blocks on the stopping while the other was on the ground shaping the block to fit. The miner on the ground bent over to pick up a tool when the miner on the ladder lost grip dropping a block that struck the other miner on the back of the head, causing a laceration that required 4 stitches.

Over-exertion in lifting objects

While lifting a 5 gallon can of fuel a miner felt a pain in the groin area. EE was examined at a Medical Clinic and told EE may have a small hernia. They set up an appointment to see a specialist and released EE on Modified Work Release. The full Diagnosis has not been received at this time.

A Roof Fall in the GOB area of the 2nd East Retreat (Partial-Pillar) section occurred that damaged, or knocked-out, as many as 37 Ventilation Controls, (Stoppings, Regulators & Overcasts). This caused the Air to reverse direction and/or short-circuit. It also caused Rock Dust to be suspended in the mine's air courses for several minutes.

Caught in, under or between a moving and a stationary object

2 miners were stacking ventilation tubes in a cross cut. 1 of the tubes became caught. 1 miner pushed one end while the other lifted the other end. The tube moved smashing the lifting end miner's fingers between the tube and a block stopping. The injured miner sustained a laceration to right-middle finger that requires stitches.

Employees were installing outrigger pads for placement, as they were taking the pads out of the racks on the crane the employee placed their hands under the pad instead of using the designated handles. As they pulled the pads out of the rack the employee's left index finger was pinched. This resulted in a laceration and broken left index finger.

A fireboss conducting a weekly examination found a roof fall in the 5th west section between #46 and #47 x-cut # 4 entry. The roof fall measured 30'L x 16'W x 7'H. while investigating the fall a second roof fall was found # 5 entry # 48 x-cut 20'L X 18"" W X 7' H it was determined later that the second fall may connect with a old fall at # 47 x-cut in the # 5 entry.

Injured stopped to fill the make-up tank on a Wagner LST-5 scoop, while removing the cap the air pressure that was built up inside the tank caused the fill cap to blow off striking the exhaust filter housing then striking the employee in the mouth causing broken teeth and lacerations to the lips.

A roof fall was reported on 12/01/2011. After further investigation of dangered off areas 5 falls were reported. All five falls had been dangered off with a physical barrier and were assumed to be inactive areas.

While traveling in the mine a roof fall was found in the croc cut # 20 between #3 and # 4 entries in the 2nd Right section. This fall is a continuation of an area that had been cribbed and dangered off and later fell. The new area of the fall is 20'W x 20' L x 6' H and has fallen over a return/intake stopping and a single row of timbers.

Employee was walking belts doing his weekly exams. He slipped causing his right leg to slide forward. His left leg folded under him putting his full body weight on it. He felt pain is his left knee and heard a pop.

Injured was installing roof bolts when his drill steel became lodged and bent as he freed the steel and lowered it from the roof he reached in to grab the still and inadvertently bumped the bolters control lever rotating the steel sticking him on the inside of the right arm and hand causing a fracture to his thumb.

Injured was kicking with his foot a 8""x8""x3' crib block under a fifth wheel trailer so he could un hook it from a prime mover truck. He missed the crib block with one of his kicks and felt a sharp pain in his left knee falling to the ground and laying there until he received help to get up. Employee started missing work on December 14th 2010

Struck by falling object

Two bolter operators were re-bolting an area that was breaking up. They had bolted to a simplex jack. They decided to manually remove the jack. While one bolter started to let the jack down the other bolter pulled back on the mesh. At this time a rock fell out of the roof between the roof bolts striking the bolter pulling the mesh back on the legs causing a fracture.

Employee was lifting a cylinder out of the truck without sufficient help. The cylinder was greasy causing it to slip from the employees grip. When the cylinder slipped, it lacerated and smashed the employees right pinky finger.

During a preshift examination by a fireboss he found a roof fall in the 1st North section at #3 entry x-cut #9. The roof fall was approximately 15' x 15' x 6' high and the area had been bolted with 6' resin bolts. The fall was not in an escapeway and did not impede ventilation. Travel was rerouted around the cave area.

Employee was cutting the plastic bands on a pallet of resin when he slipped and cut his left forarm with the belt knife that he was using. The laceration required 7 sutures.

Firebosses were pre-shifting the mine for the midnite shift on Sunday night in 5th West entry #2 at x-cut 47-48 when they found a roof fall. It was dangered off and reported to shift foreman and MSHA. The fall was 20' wide by 75' long and approx. 8' high. A double row of timber was set at all accesses to danger the area off.

On 07/08/2009 the firebosses found an unintentional roof fall in the 5th West section at x-cut 46-47 in the #5 entry (return). The roof fall measured 30-40 feet long by 12 feet wide and was 5-6 feet high and 5 foot resin bolts had been installed in this area. Not an escapeway-no ventilation impeded-no injuries.

Employee was rock dusting the return at a breaker row when he stepped over and thru the check curtain and tripped falling off of a four foot lip that had been left from mining floor coal in the entry. He received a laceration to his left wrist requiring 5 sutures.

Employee was carrying a crib block on his left shoulder when his feet slipped out in front of him on the muddy wet mine floor. When he landed on the mine floor he put his hands back behind to catch himself and the crib landed on his left ring finger causing a laceration and a fracture to last digit (5 sutures).

Employee was installing a roof bolt when he leaned the bolt against the drill head and was inserting tube of resin in hole when he pushed up on the mesh and the bolt tipped over striking him in the mouth chipping his right front tooth.

At approximately 11:35 am the belt went off in 12th West. A roof fall was found at 21 x-cut in the belt entry that measured 10'L x 20'W x 4'-4.5'H and had cut the remote line shutting the belt off. The area was dangered off and federal inspector Donnie Durrant was notified on the property.

A roof fall in the 6th West bleeder 100' inby MPL #1 at x-cut 3-4 was found by a weekly examiner and reported to the Safety Supervisor. Shift foreman went to determine if it was reportable and it's size. Reported to be 20'W x 60'L x 6'H. Five foot resin grouted bolts were used in this area. No injuries, not impeding travel, or blocking escapeway.

Weekly examiner found a roof fall at 8th South right return #6 entry at #6 x-cut. There had been a previous roof fall in the x-cut 6-7 and the intersection fell in approx. 20' x 15' x 7-8 feet high. The area was dangered off and timber set at both accesses. Does not impede ventilation and is not an escapeway.

At approximately 1:35 am the belt crew was moving belt when they heard a roof fall. Supervisor immediately went to the last open x-cut in #8 entry at #23 x-cut of 12th West section and found a roof fall 20' x 20' x 4.5' high. The area was dangered off and called to the shift foreman. Shift foreman notified superintendent and safety supervisor. Not above 5' bolts being used.

Employee was pulling on miner cable to get slack to hang to the mine roof and felt a burn in his lower left abdomen.

At 11:25 am Mine Foreman called out to me that he had found a roof fall in the 6th West bleeder at x-cut 22 in the intersection of the #2 entry that measured 30' x 25' x 6'-7' high. Five foot resin bolts had been installed in this location. MSHA was notified at 11:30 am and a K-Order was issued verbally. Also called into 800 number at 11:33 am.

Employee was operating Joy shuttle car and pulled behind the Joy Miner when the boom contacted the mine roof and shot a piece of coal into the shuttle car cab striking the employee in the head and neck causing a laceration on his right cheek that required 5 sutures.

A roof fall was found at 1:52 am by the midnite shift firebosses in 6th West at x-cut 14 in the #3 entry. The fall is approx. 20' wide x 30' long x 8' high. The fall has been dangered off on all sides and additional support set. It is not an escapeway and does not impede ventilation (intake). No injuries or equipment involved.

A roof fall was found at x-cut 62, #4 entry, in 4th East. This is an outby area of the mine and is not an escapeway or travelway. The roof fall is not impeding ventilation. It has been dangered off and additional support set on both sides of the fall. The fall is approx. 15' wide x 40' long and 6' high. The area was supported with 5' fully grouted resin bolts.

Employee was grinding slag off of a piece of metal that he had cut when the 9"" grinder he was using caught on the slag. The grinding wheel cut his right forearm that required stitches. Employee was wearing leather welding gloves that the wheel hit before contacting his forearm. He was grinding on the bottom of the sheet metal with the grinder being held upside down.

Employee was setting a timber (post) when another employee used an axe to tighten the wedge and the axe slipped out of his hand striking employee on the mouth causing a laceration that required sutures (9). The employees gloves were wet and oily.

A roof fall occurred in the 6th West #2 entry at #8 x-cut in an adjacent intake entry that is not a manway, travelway or escapeway. The fall was measured at 25' by 15' by 8.5' high. There were no injuries or equipment damage.

In 14th West Pillar section in the #5 entry a roof fall occurred at approximately 12:45 pm. It fell 18' wide x 31' long x 6' high. 5' resin bolts were installed during development. The cave buried two Mobile Roof Supports. There were no injuries. Two MSHA inspectors had been in the section observing mining prior to the fall.

A roof fall ocurred in the 5th West section in the #2 entry to the face in the x-cut toward #1 in the panel 1 North neckoff entries to the right of 5th West. The roof fell in 5'-5 1/2' and took out 3 rows of 5' resin bolts at or above the anchorage point. There was no equipment damage or persons injured.

Employee was trimming a splice on the conveyor belt that had some loose buckles and slipped with his razor knife and cut the back of his left hand. The cut required 5 sutures to close the cut. Employee was not wearing gloves at the time of the accident.

EE was closing the door to the scrubber at the left discharge on the rear of the miner. He caught the end of his middle finger on his left hand between the handle on the door and the frame. It caused a laceration to the end of this finger that required sutures. There was a fracture to the tip of this finger also. He returned to work with no lost time.

At approximately 11:20 pm in the 1st South Pillar section, MMU 004-0 the roof fell on top of the continuous miner. There was a combination of top coal and rock that fell on the miner causing it to be trapped. MSHA was notified and investigated. The continuous miner was recovered with minimal damage by 7:00am on 12/30/2005. There were no injuries.

At x-cut 62-63 in 4th East in the #5 entry we had a roof fall that was 8' high x 80' long. It isn't an escapeway or travel way. It is in the intake. All access has been dangered off and supported. It does not impede travel or impair ventilation. MSHA was notified on 07/05/05 at approx. 8:00 pm. The area had 5' resin bolts installed.

Over-exertion in lifting objects

EE was unloading some tires out of a pickup and heard a pop and felt pain in his right shoulder. He saw a doctor at the Medical Center and was released back to work. He continued to have pain and recently (on 7/18/05) went off for shoulder surgery. He worked from the accident date to the date of surgery.

Over-exertion in pulling or pushing objects

EE was pulling miner cable when he felt a pain in his groin. He told the miner operator and worked his full shift. He worked the next day and was in a little more pain at the end of his shift. He told his supervisor on this day. The next morning he was in a lot more pain and decided to get it checked. He was diagnosed with a hernia and referred to a surgeon.

Rick was walking down the belt entry and felt an achy pain in his right foot. He was rockdusting and patrolling the belts.

EE was dragging water hose when they stepped on the miner cable, that had some oil and water on it, slipped and fell face down. Landed on both elbows, his hip, and knees causing them to hyper-extend in the abdomen area. They saw a Physicians Assistant who referred them to a surgeon that diagnosed them with a hernia. They hasn't lost any time (12-23-04) for hernia.

Employee was loading B.O. belt rollers into the bucket of a scoop (LST-5S). He picked up a bottom roller & slipped on some wet rock dust which caused a sudden movement & strained his lower back. He stayed on the job until the end of his shift & then was taken to the E.R. at Castleview Hospital. He was diagnosed with a muscle sprain/strain and taken off of work for 3 days.

Employee was loading a trailer with belt structure when he lifted a bottom roller and felt a pop in his left shoulder which resulted in a ruptured bicep muscle. This occurred on the surface.

Employee was lifting a roll of electrical cable thru a man door and strained his groin area. He stayed on the job and saw a doctor on 09-14-2004. He saw a surgeon on 09-16-2004 and was diagnosed with a hernia. His first missed day was 09-27-2004. HERNIA SURGICALLY REPAIRED ON 10/6/04.

HE HIT HIS HEAD ON A ROOF BOLT, INJUR ING HIS NECK AND BACK. AFTER HITTING HIS HEAD HE FELL TO THE MINE FLOOR ON HIS REAR CAUSING PAIN TO HIS HIPS AND LOWER BACK.
