Employee was stepping off a ladder when they stepped onto uneven ground, causing them to roll their ankle.

Caught in, under or between a moving and a stationary object

While lowering the anvil into the Remco, EE was holding the anvil and a co-worker was pulling with a strap to move it into place. It wasn't lined up, so they were trying to maneuver into place. At some point the cable of the crane got some slack and then released smashing EE's left pinky finger between the anvil and the rotor.

Struck by... (Not Elsewhere Classified)

A belt ripped on an 8x20 screen causing mud to fall through and build up on the tail Polly stopping the conveyor. LOTO was performed. EE started chipping mud off the Polly using a 30-inch pry bar to knock the mud away. EE used EE's right hand to brace self against the frame of the belt and used EE's left to chip. As EE was chipping, EE missed the Polly and hit EE's right thumb.

Employee was adjusting the jaw wedge plates and couldn't get the nut loose with cordless impact. Employee decided to use an open ended wrench and got the nut to come loose, as it broke free elbow struck the corner of the jaw and cut elbow open. Employee received stitches and a prescription for an antibiotic.

Climbing into loader by pulling himself with his arms. Injury date 4/24/08, seen by Dr. 5/7/08, went recordable 6/5/08 with an injection of cortizone. No time off from work or work restrictions.

CREATING STOCKPILES WITH THE DOZER. UPON PUTTING THE DOZER IN REVERSE, IT STALLED CAUSING IT TO ROLL FORWARD AND OVER THE 6-8 FOOT EDGE THAT WAS THERE. EMPLOYEE JERKED HARD BY THE SEATBELT HE WAS WEARING.

USING A CUTTING TORCH IN A 9 TO 10 INCH OPENING UNDER JAW BELLY BELT TO CUT A BOLT, EMPLOYEE MADE THE CUT & WAS MOVING THE TORCH TO SHUT IT OFF. TORCH HEAD CAUGHT ON THE FRAME & SPUN AROUND TH E FLAME, CROSSING HIS ARM.
