The victim was unloading a rail car using a transloading conveyor and was entangled in the feed end of the conveyor.

Struck against stationary object

The employee was attempting to sharpen a garden hoe when employee's hand slipped resulting in a laceration to right thumb. The resulting wound required 4 sutures to close.

Fall onto or against objects

EE was cutting screen when EE stepped back to evaluate work and their right heel got caught in screen causing them to fall and strike the bucket on the skidsteer bucket that was parked in the vicinity.

Struck by falling object

EE was putting a bearing away on a mid-height shelving area in the conex when the bearing slipped from hand and struck the metatarsal area of left foot.

Miner fell off railcar and struck right hand on side of car. Fall protection devices (harness and retractable lanyard) arrested miners fall.

Over-exertion in lifting objects

Injured miner reported on 4.28.15 that on 4.27.15 he ""tweaked"" his back while manually sliding a piece of conveyor belt onto skid steer forks for lifting. Later, same day (4.27.15), he hurt his back again lifting scrap conveyor out of skid steer bucket. Diagnosed with lumbar strain on 4.28.15.
