Employee was operating an angle grinder when EE felt a discomfort in EE's right eye. The next day EE was taken to a medical clinic and was prescribed an antibiotic eye drop.
Employee was ascending the steps of 10 wheeler when slipped and tried to catch self. This caused a strain to EE's left shoulder.
Injured was climbing on a piece of equipment and his right foot struck the bottom of the step causing pain. he did not seek medical treatment until 04/09/2014 when his foot became swelling and he could not get his boot on. diagnosis tenosynovitis to foot and ankle build up of fluid.
