Struck by falling object

Miner was on the plant clearing the roll box while the plant was running when a rock that was estimated 6 inches fell roughly 4-6 feet from the top deck and struck the miner. Miner was wearing a hard hat at the time and there was a witness that saw the incident occur. There was no open injuries or bruising.

EE was removing screw from Skidsteer bucket namely Edge Plate. Head bolt lacerated left middle finger requiring stitches to close the wound.

IE was fueling up haul truck with DEF fluid, as IE went to place the nozzle back IE's foot got wrapped with the hose. Causing IE to stumble when IE stepped forward causing pain in right knee. Took IE to clinic, was fully released. IE was still in pain after a week of first visit so we took IE back to the clinic on 1/18/2021, that's when IE received restrictions.

Struck against stationary object

Employee was picking up an air filter that was to be thrown away and it slipped through his hands causing a laceration to the left index finger. He was not wearing his gloves at the time. Required stitches but no restrictions given and he was released to go back to work full duty.

Struck against a moving object

A contractor's pit truck had come down a ramp into the pit area. At or near the bottom of the ramp the driver said his brakes went out and he steered the truck toward some material berms. When the truck went up onto the side of the berm the truck to turn on its side. Damage to the truck is unknown. The driver received some significant contusions to his shoulder.

It was beginning to get dark and the employee, as part of his duties, was preparing to power up one of the portable light towers for night work. He was walking around the tower and said he slipped as it was muddy from a recent rain and fell against an out rigger leg at the back of the tower injuring one of his left ribs.

Employee had been running the 988H Loader and had parked the unit at the end of his shift. Just prior to the incident he had exited the cab carrying his lunch box and a thermos. As he was coming down the first set of stairs he missed a step and fell hitting his left hip on the landing below the stairs.
