Fall from machine

Day of incident and on, the injury was treated as a first Aid. EE's injury became a modified work duty on 10-29-2025. Equipment Operator was descending the ladder and missed the last step causing EE to fall and injury EE's right shoulder.

Dozer operator exited the dozer cab and pinched left pinky finger in the door as EE closed the cab door.

Haul truck driver stated: drove over a pothole in the road then began experiencing lower back pain and numbness in left leg.

Admin asst entered shop to clock in for the day. As EE entered the building EE slipped on silt and mud that was on the ground. EE injured right hip. Injury initially treated with first aid measures. Employee received medical treatment on 08/09/21.

Equipment operator exited D9 dozer and fell down to the track. Operator suffered laceration to right knee.

Employee was attempting to place a shackle over the frame of a log washer and smashed right pinky finger resulting in stitches.

Struck against stationary object

A plant operator was changing the bearings on a field conveyor. EE was using a wrench to loosen a bolt. The wrench slipped off the bolt and the employee hit right elbow on the feeding trough. Injury was initially first aid. Management found out on 02/17/20 that employee was prescribed pain medication by a DR from this incident. Employee never got the prescription filled.

Over-exertion in wielding or throwing objects

Plant manager was assisting employees with driving in a bushing on an excavator bucket. The plant manager was using a 20 lb sledge hammer to drive in the bushing. The next day the plant manager felt pain in the back and hands. Their hands also started to swell.

Fall to the walkway or working surface

The plant operator went to shut off the water valve that was left open overnight due to freezing weather. As EE approached the valve EE slipped on some ice that was covering the plate and fell hitting EE's knee on the corner of the plate that was embedded into the concrete slab causing a puncture below EE's right knee cap.

While exiting the load the employee stepped in the mud and his foot got stuck. The employee strained his left knee trying to get his foot out of the mud.

While performing maintenance on the conveyor EE crawled over conveyor belt from one catwalk to another to check bearing placement while crawling back to original catwalk he placed his foot on a piece of wood on the catwalk rolled his left ankle resulting in sprain/strain/contusion to the left foot and ankle.

Employee was manually rolling the conveyor due to a power outage when his finger got caught between the conveyor and the pulley resulting in a laceration to his right index finger.

Employee was using pressure washer to clean equipment when he decided to use on his boot. The wand slipped and went in his boot resulting in a burn to left foot

EE WAS BENT DOWN CLEANING OIL SPILL ON DRAGLINE WHEN SLIPPED AND FELL BACKWARD HIT FINGER AGAINST METAL GRATING ON FLOOR OF DRAGLINE. THEN YANKED FINGER OUT, RESULTING IN OPEN FRACTURE OF LEFT LITTLE FINGER.
