Struck by falling object

Removing a cutting edge on a Cat 980G Front End Loader bucket. When the last bolt was removed the metal came loose from the bucket and struck the victim on top of the left foot causing a fracture to a metatarsal bone.

Employee was changing the oil on the John Deere 400 haul truck. When he was closing the belly pan he strained his back. This was not originally an MSHA reportable, but became one later when the employee went to the doctor and was put on restricted duty.

Employee was welding on a guard that was lying on the ground. He knelt down on a piece of hot metal and burned his left knee.

Fall from machine

Frature to Coccyx & futher back injury (type to be determined.)** Employee was under the hood of loader. The mechanism holding up the hood failed knocking Employee 4 ft to the ground. Narrative paraphrased from work activity.**

Absorption of radiations, caustics, toxic and noxious substances

Employee was assisting the explosive contractor with loading the drilled holes when the he experienced a burning and stinging sensation in his left buttock. He was standing & bending over at time - did not see or hear anything that bit or stung him; area was red with somes swelling -went to doctor and received a prescription.
