Caught in, under or between a moving and a stationary object

Employee lowering manifold flange part into place. Finger was between the catwalk and the flange. Result was broken finger - left hand ring finger.

Caught in, under or between a moving and a stationary object

Two employees were installing a new bearing on a shaft. Employee #2 was holding shaft up with a pry bar while Employee #1 was installing the bearing. Employee #2's pry bar slipped forward, pinching Employee #1's left ring finger between the pry bar and the bearing.

Fall from machine

The miner was dismounting a 400D John Deere Haul truck, when he caught his left foot on the top step. He did have ahold of the hand rail and was going down backward. He lost his footing and fell 6 feet to the ground landing on his back. He hit his right arm on a hydraulic cylinder, breaking his arm just above the wrist. There were no other injuries.

On manlift to change roller on #2 belt - was raising basket when struck conveyor belt injuring forehead, bridge of nose & eyelid - nose & eyelid required stitches.

Employee was cleaning out material that was stuck in the hopper grizzley. The hopper oveflowed, dumping material into the grizzley. The material knocked off employee's hard hat-additional material caused a head laceration.

Struck against a moving object

Truck driver neede 1000 # sand. He was instructed to exit scales and wait for loader to bring the sand. EE drove to the sand load out area instead. He then went to the sand rile, did a U-turn and pulled behind the loader as it was backing up. The loader hit the right side and damage was done to the truck, loader not damaged. No mining imparment as result of accident.

CHECKED OUT PLANT, WALKED OVER TO SCREEN ROOM, CALLED OFFICE TO SAY NOT SCREENS, CLIMBED IN LOADER, SAT DOWN, THEN NOTICED PAIN IN LOWER LEFT HAND SIDE.
