Struck by falling object

Employee was cleaning beneath the grizzly, at the dump hopper. When a clumped mass of material fell through the gap in the plate steel shielding the top grizzly, dropped approximately 15 feet, striking the employee on the top rear of their hardhat. Employee sustained a concussion without loss of consciousness and a neck sprain.

Caught in, under or between a moving and a stationary object

Employee was performing maintenance on 870 transmission hydraulic pump. While positioning pump on service truck bed, pump began to roll to employee's right. Employee attempted to stop pump from rolling which resulted in employee's right ring finger being pinched between pump and vise mounted to service truck bed. Employee required 15 stitches to right ring finger.

Struck by falling object

While positioning a sheet of 5'x13' 1/4"" plate steel to weld, the injured employee became momentarily trapped underneath it when the steel came off of the hook used to lift it into position and fell on top of EE.

EE came down stairs from small shaker. When he got on the concrete he slipped on an old metal pad hitting his head, back and arm.

EE was washing green algae from concrete. He slipped down hitting right shoulder on concrete and scraped side of his face.

EE climbed portable ladder onto control house roof. He was walking across it and slipped off of the roof and fell approx. 10 feet onto the tounge of welding trailer sitting next to control house. He had compound fracture to lower right leg.

EE WAS WORKING A TRACK OFF OF EX550. HE WAS HAMMERING IN A TRACK PIN, WHEN A SMALL PIECE OF PIN BROKE OFF AND WENT INTO HIS LEFT UPPER ANKLE. THE PIN WENT THROUGH COVERALLS, TIP OF BOOT, APPROX 11/2 INTO UPPER ANKLE. HE WAS DRIVEN THE THE ER AT NAVARO REGIONAL HOSPITAL.

EE was pulling on wash hose under dump hopper. He fell over a large rock and twisted his knee, he attempted to wash sand out from under charge belt hopper.

OPERATOR WAS DOING A WALK AROUND INSPECTION OF DOZER, HE SLIPPED ON A CLUMP OF MUD AND FELL, RESULTING IN THE STRAIN OF BACK. CONDITION WERE MUDDY AND WET.

PLANT MAN STEPPED UP ONTO MIDDLE OF HANDRAIL TO LOOK INTO LOG WASHER. AS HE WAS ATTEMPTING TO STEP BACK DOWN HIS FOOT SLIPPED OFF THE MIDDLE ANGLE OF HANDRAIL AND FELL BACKWARDS ONTO HIS BACK. MIDDLE ANGLE WAS SLIPPERY DUE TO WATER SPRAY IN AREA. FELL ONTO PLATFORM NEXT TO LOG WASHER.
