Employee was washing slab under hopper with water hose when the clamp failed and the nipple came off the hose cutting the left hand between thumb and index finger. 4 stitches and tetanas shot.

Employees were cleaning up around the outside of shop lifted a weart plate to clean off slipped out of the hand of the employee and hit the top of employee's right foot

Driller was adding addition to auger. When the auger stop rotation the helper reached for the pin as the driller reversed rotation and the head of the bolt pinched his thumb against the outer portion of the yoke. Helper had a fracture to right thumb.

EE was adding 5 gallon bucket of antifreeze to the dozer and felt pain in back pain went away continued working woke up next day Sunday sore Treatment Monday muscle spasm and low back pain treated with prescription medication.

Employee was working on tracks. He did not realize that he was so close to the open part of the truck bed and when he turned to step away he fell into the open section of the trailer landing hurting his left thigh and right shoulder.

EE was cutting rubber with a razor knife when the blade came out of the knife handle cutting his left hand

EE was getting ready to work on machine, pulled the air hose from the hose reel on the service truck. EE felt a pop in the lower back.

EE was moving a toggle plate that weight about 200 lbs with proper leverage or help

Employee was losening bolts with a drive ratchet when he felt a burning sensation in his right arm near the bicep/elbow.

Employee was attempting to remove a plastic cable tie from inside a haul unit when his box cutter slipped resulting in a laceration to his right thigh. Employee received 7 stitches as a result of this laceration.

On 2/06/09 employee was removing bucket teeth with a hammer when he noticed a laceration on his left forearm. Employee used first aid to care for the cut. On 2/16/09 employee realized that laceration had become infected and sought medical treatment, at which point the injury became reportable to MSHA.

Employee was working on a Loader lifting heavy parts. Over the course of the day he noticed that his back was in pain and uncomfortable. Next day employee wished to seek medical treatment.

Employee was attempting to enter cab of haul truck when his foot slipped off the step resulting in strain of lower back.

Employee was assisting in the maintenance to shaker when the crane malfunctioned when lowering shaker, pinning employee's left hand between the spring and mounting plate.

Injured was assisting another employee with replacement of bucket teeth on trackhoe. The teeth were loosened by cuts made by torch. As other employee struck a tooth, a piece of the tooth shot away and embedded into injured's right leg.

Employee was exiting A-40 Volvo when he slipped on the step, resulting in strain of left knee.

Employee was moving batteries from service truck to the loader when he felt pain in lower back.

Employee was attempting to remove rock that was lodged under tailgate on trailer. When the rock fell out, the tailgate slammed onto fingers of left hand, resulting in amputation of tips of left middle and ring fingers.

AFTER PROPER LO/TO PROCEDURE INJURED AND CO-WORKER WERE CARRYING MOTOR ACROSS CATWALK OF SECONDARY SHAKER WHEN A SUPPORT BAR UNDERNEATH CATWALK BROKE CAUSING EE TO FALL APPROX. 15' BEFORE LANDING ON SCREW/SAND AUGER BELOW.

EE WAS REMOVING PIN FROM TRACK HOE BY HITTING IT WITH BAR SECURED BY CHAIN HOIST. WHEN THE PIN WAS RELEASED EMPLOYEES FINGER WAS HIT BY THE BAR, RESULTING IN FRACTURE OF LEFT LITTLE FINGER

EE was attempting to lift oxygen bottle when he felt pain in lower back.

WHILE ASSISTING IN WORKING ON A40 VOLVO, EMPLOYEE HIT HIS HAND AGAINST METAL FRAME RESULTING IN A FRACTURE OF RIGHT RING FINGER.

EMPLOYEE WAS DRIVING A WEDGE INTO SLOTTED PIN ON PRIMARY SHAKER WHEN THE PIN POPPED BACK OUT IN A TWISTING MOTION RESULTING IN A LACERATION OF THE LEFT HAND.

EE PULLED ON WATER HOSE RESULTING IN STRAIN TO THE GROIN AREA.

EMPLOYEE WAS SHOVELING MATERIAL UNDER A CONVEYERAND STRAINED HIS RIGHT LOWER BACK IN THE PROCESSEMPLOYEE MISSED ONE DAY OF WORK TO REST HIS BACK MUSCLES AND ALLOW THE STRAIN TO HEAL.
