EE was walking on even ground near the oversized conveyor and slipped on a slick spot causing them to twist their left ankle and fall to ground (same level).

Struck by falling object

EE placed troughing idler on (R) shoulder with(R) hand over the center can & (L) hand under the angle iron frame to carry the 64lb idler. EE walked approx 64ft when right hand slipped off the center can. EE lost control of the idler & the roller fell end-over-end striking top (L) foot 1.75' behind the steel toe. EE suffered an open fracture & released to restricted duty.

Accident type, without injuries

EE discovered a fire on south end of property river bank. Cause of fire unknown - dried vegetation burning on the river bank. Dozer cut a fire break on top of the old spoil piles to prevent the fire from coming onto mining area. Volunteer fire dept. attempted to extinguish the fire. Dozer to push sand over the fire, the fire was extinguished at 10:15pm.

On May 21, 2014 at approximately 6:30PM, employee was pulling a swing block out of the conex building just East of the bucket wheel. He reached in and pulled it out with his left arm. He felt a pain and burn in his left bicep area. Employee immediately notified another employee. He was taken to Texoma Medical Center ER for evaluation.

On February 6, 2014 at approximately 10:00AM, EE was working at the bucket wheel on the West transfer conveyor with 2 co-EE's. He slipped on ice and fell on his right side. He immediately felt pain in his right elbow and his lower left back. He reported this to company immediately. EE was driven to One Medical for evaluation.

EE was putting power cord back on pontoons. he was trying to step from the work boat to one of the pontoons he slipped and hurt his knee

Employee was working on the dredge power cord to get it pulled loose from the bank near the dredge pit. As he was pulling, he felt a pain in his left arm between his elbow and shoulder.

On Monday, August 5, 2013 at approximately 3:50pm, IE was assisting in reassembly of the booster pump. He was helping another employee pick up and carry a steel ""foot"" support for the pump. As the two were placing the foot onto the deck, EE appeared to have dropped his end first causing the weight of the foot to pull on IE arm.

Employee was using a Freightliner water truck to wash out under the field conveyor. The nozzle came off. The employee then crimped the hose turned to shut the water off at the valve. The hose came un-crimped and employees hand slipped the hose clamp cut employees hand

On May 6,2013 EE was greasing tripper feed conveyor. As he was descending the steps he twisted his left ankle.

Accident type, without injuries

One of the trucks being used for the overburden stripping activities caught on fire, probably due to an unknown damaged hydraulic line. The operator heard a pop, then saw that the fire had started. The operator of the truck was unable to extinguish the fire, so the engine compartment and cab completely burned.

EE was pushing roll of old belting. Hurt his back. Placed on light duty.

Employee was operating the john boat to retrieve a broken dredge stern cable from the bottom of the pit. When he was pushing the cable off the front of the boat he felt a pain in his right forearm.

Injured EE and another EE were repairing Dredge pipeline. The other EE was hammering bolts into the flanges of the pipeline. Injured EE reached down to push one of the bolts in, thinking the other EE was finished hammering, but his left middle finger was struck by the hammer. Injured EE was taken to the Doctor for evaluation.

Struck by falling object

EE was helping replace the top shaker screens at the Bucket Wheel. EE was at the end of the 1/2 punch plate screen pushing uphill using the palm of his hands. The screen slid and his left middle finger dropped down into the hole of the screen. This caused amputation to the tip of his left middle finger. EE was immediately taken to hospital.

On Jan. 4, 2010 employee was checking out the plant for start up. He was underneath the plant making sure the valves on the slurry tank and screws were closed. He went to step over the plant's 18"" wide drain ditch and slipped on ice. This event caused his left leg to over extend forward. He felt pain in his upper leg.

On 11/24/2009, EE and an employee went out to the John Boat to the fresh water pumps. They were unhooking the Grindex pump to bring to the bank to be removed from the dredge. EE was having difficulty unhooking the water line. He lifted as hard as he could and noticed his back started hurting. He reported incident to Supervisor.

Injury was grinding on a piece of angle iron near the bucket wheel. He felt a piece of slag hit him in the chest and then it bounced off entering the face shield then stricking him in the right eye. He stated to me that he was wearing his safety glasses and safety shield. He immediately reported this to the plant Mgr.

On Thursday, July 15, 2009 at appr. 1230pm, employee was using a scraper to remove paint from a workboat. As he began using the scraper, paint pealed up and stuck in between his right thumb and index finger. He received 3 stitches.

On Wednesday, July 8, 2009, employee was stepping from one small boat onto another small boat. When he stepped off of one boat onto the other he slipped and fell. He dislocated his left shoulder.

Injury and another were working on the dredge pipeline. other and injury were trying to break the pipe apart. He loosened the bolts and the pipe float flipped back and caught his arm between it & the boat.

Employee put his hand on a pump box to move it. There was a small piece of metal that was sticking out from the box. He cut the palm of his left hand.

On Tuesday, July 8,2008 employee was help working on repairing a power cord. He bent over to pick the power cord up and strained his lower back.

On Wednesday, June 11, 2008 EE was standing behind the MCC room having a discussion with another EE. The other EE had a wash down hose and sprayed it towards injured EE. In an attempt to get away from the water, injured EE turned around quickly to run, hitting a walkway support beam. He ran into the beam and knocked his tooth out & broke others.

Employee was riding in the boat with co-worker operating it. Operator came upon a sandbar unexpectedly and it knocked employee against the cab. He hurt his right arm.

Employee was trying to straighten a piece of plate metal with a sledge hammer on the floor of shop at TXI Bells. The piece of metal flew up & hit him on the head above his left eye. He was cleaned up at plant then taken to hospital for treatment. He received 9 stitches in forehead.

Over-exertion in lifting objects

Employee was delivering bearings to repair the concrete stacker. Once job was completed, employee was returning an unused bearing back the shop. The employee reached over the side of his pick up truck to pick up the bearing and felt his back strain.

Employee was trimming the concrete stacker belt. The belt was wet, causing his hand to slip and cut his index finger on a utility knife he was using. He received 5 stickes.

Employee was changing paddles in the logwasher. He climbed out of the logwasher to shut off the air compressor. He then went to climb back into the logwasher when he felt a sharp pain in the middle of his back. It is suspected that the back strain happened prior to him climbing out of the logwasher when he was handling the log paddles which weigh 40-50 pds.

Employee was installing bolts onto a pump case. The bolts were being tightened with an impact. One of the bolts spun while he was attempting to hold the bolt thereby smashing the tip of his left index finger.

EE was performing maintenance on the workboat. He had the lid to the engine compartment open. wind blew the lid closed. As the lid fell it struck him on the right ankle and leg.

EE was near the parking area at the Texas plant, when he was stung by a bee on his shoulder. He was taken to the doctor as a precaution. There was no lost time and no restricted duty of any kind.

EE was performing routine maintenance to the crusher. He was being helped by another EE was lifting out the shoe ring when he lost his hold on the ring. EE attempted to catch the ring which resulted in his left index finger getting smashed between the ring and the top of the crusher. The injury required 3 stitches and about 7 days of restricted duty.

EMPLOYEE WAS WORKING ON A TAIL PULLEY BEARING OF A STACKER. IN THE PROCESS OF CUTTING OFF AN OLD BEARING A PIECE OF METAL SLICED THROUGH HIS LEATHER GLOVE AND CUT HIS HAND.

EE was making repairs to the cutterhead drive when he slipped and fell into the dredge pond causing him to ingest a small amount of hydraulic fluid and get some into his eyes. He was wearing all requird PPE at the time of the incident.

Struck by flying object

EE ON 5/23/06 WHILE WORKING ON THE DREDGE GOT SOME SAND IN HIS EYES. THIS WAS ATTRIBUTED TO THE WIND BLOWING. HE THEN PROCEEDED TO THE 1ST AID KIT, WASHED OUT HIS EYES WITH EYE WASH. SEVERAL DAYS LATER, WAS TAKEN TO DR AND DIAGNOSED WITH AN EYE IRRITATION. HE LOST NO TIME AND RET'D TO FULL CAPACITY. THE DR GAVE HIM EYE OINTMENT FOR IRRITATION.

Employee was welding on the dredge discharge pipe near the pump. He was standing on the middle rung of the handrail when he slipped off & fell onto the deck of the dredge. Fall was approx 2' down from the point he was standing to the deck. He felt as though he might have pulled something but did not feel it was serious. Dr diagnosed a pulled stomach muscle & put him on light duty.

Miner was washing under the plant with a water hose. He stepped backwards onto a pile of spilled material. He experienced pain in his left knee and was taken to the emergency room and x-rayed and given a cat scan. He had strained ligaments. He was given restricted duty. He returned to Dr. on 3/14 and was given extended restricted duty. Will return to Dr. on 3/21.

Struck by rolling or sliding object

Employee was helping unload a roll of belt from a trailer by rolling it off. He got in front of it to pull on it and when it freed he slipped on the ramp. The roll of belt knocked him down and rolled onto his right leg, twisting it. He went to the doctor on 2/7 who diagnosed it as a knee and ankle strain and took him off work until 2/14/05 and he returned with no restrictions.

Employee was looking at the damaged trunnion sockets while standing on the discharge pipe on the dredge. To get a better view, he stepped onto a 3/8"" safety cable. The cable broke and he lost his balance. He grabbed a hydraulic hose to stop himself from falling but fell into waist to chest deep water between the pontoons. His shoulder started hurting him several hours later.

EE WAS WORKING ON THE GOLF COURSE HEAD PULLEY WIPER. HE WAS WORKING 2 FT OFF THE STEP LADDER. HE HAD ONE END OF THE SHAFT TIED OFF WITH ROPE. HIS FOOT SLIPPED. HE HAD TO LET GO TO CATCH HIMSELF. HE FELL AND PINCHED HIS THUMB AGAINST THE CONVEYER. HE RECEIVED 5 STITCHES AND RETURNED TO WORK WITH NO RESTRICTIONS.

EMPLOYEE WAS TAKING THE FIRST STEP UP A FLIGHT OF STAIRS AND FELT A SHARP PAIN IN HIS LOWER BACK. IT GOT WORSE AND HE REPORTED IT. HE WAS TAKEN TO THE DOCTOR AND DIAGNOSED WITH A PROBABLE PINC H NERVE. HE WAS PLACED ON LIGHT DUTY WITH DAILY PHYSICAL THERAPY FOR 6 DAYS.

EE WAS IN A CLASSIFIER TUB DOING MAINTENANCE, PULLING ON A VALVE ROD WHICH WAS STUCK IN THE SAND TO TRY TO DISLODGE IT. HE FELT A PAIN IN THE LEFT SIDE OF HIS BACK. IT HURT FOR A LITTLE WHILE THEN LATER THAT DAY HIS BACK POPPED. IT DID NOT HURT MUCH AFTER THAT BUT PAIN INCREASED AS THE WEEK PROGRESSED. HE BEGAN WEARING & BACK BRACE & RELATED THE EVENTS OF THE INJURY TO TEAM COOR.

EE WAS CHANGING SPUD PIN ON THE DREDGE. HE WAS TRYING TO PULL A PIN OUT AND LOST HIS BALANCE. AS HE FELL HE HIT HIS LEFT HAND ON THE TOP OF THE HANDRAIL, WHILE STILL HOLDING PIN.

EMPLOYEES WERE PULLING ON THE WIRE RETAINING CABLE WHICH HOLDS DREDGE SUBMERSIBLE SVC PUMP, IN ORDER TO REMOVE PUMP, INJURED WAS ASSISTING BY PULLING PUMP. SUDDENLY PUMP CAME IN A RUSH AND HIT END OF MANIFOLD PIPE HOSE NIPPLE, CATCHING HIS INDEX FINGERTIP BETWEEN PUMP AND HOSE NIPPLE.

CHERRY PICKER CRANE WAS BEING OPERATED WITHOUT OUTRIGGERS EXTENDED, TIPPED OVER AND FELL ON THE MINER AND CRUSHED HIM UNDER THE BOOM AGAINST THE GROUND. SIGNIFICANT BRUISING, COLLAPSED LUNG.

WHILE OPERATING A CRANE IN DREDGE AREA, THE OPERATOR WAS MAKING A PICK WITH THE BOOM FULLY EXTENDED, WHEN HE STARTED THE SWING, HE CAUSED CRANE TO LAY OVER ON ITS SIDE.

HE WAS MAKING HIS ROUNDS AROUND THE PLANT, FOUNDA BROKEN SHOVEL, PICKED IT UP TO DISCARD IT, ANDSLIPPED & FELL TO THE BELT; THE SHOVEL HIT THE BELT & STRUCK HIS RIGHT HAND, CAUSING AN INSTANT SWELLING AND A MINOR CUT.
