Over-exertion in lifting objects

EE was unloading boxes of bolts from parts truck. In doing so, she felt something pull/twist in right arm.

Fall onto or against objects

Employee was in the process of removing the blower motor from a A30 Volvo haul unit. He was sitting on the fender of the haul unit when the ratchet he was using to remove the blower motor slipped and he slid off the fender into the back of his pickup truck. His right ring finger dislocated when it came in contact with the pickup truck.

Employee was dismounting from haul unit after pre-check and twisted right ankle when foot came in contact with ground.

Employee was cutting some 1/2"" holes into some skirtboard rubber with a boxcutter when it slipped cutting his left hand at thumb between thumb knuckle & wrist. Required 9 stitches.

Struck by falling object

Employee was removing guard to replace v-belts @ headpulley of conveyor. Lifted about 4"" and wouldn't budge. Reached down down grab guard & pickup on, when guard dropped back into place, severing tip of middle finger on left hand.

Employee was installing dresser coupling on cyclone pump, when it slipped and cut open the pads of his right ring & middle fingers, requiring stitches.

EE stopped haul unit at lab to speak w/plant operator. As he started walking to operator, after exiting haul unit, he tripped and fell. He tried to stop his fall w/his left forearm... in the process hurting his left shoulder.

Employee stated he that when he was lifting trashcan to toolbox of plant truck he reinjured his shoulder.

EE STRAIGHTENING YIELD SIGN, HAD BOTH ARMS AROUND SIGN WHILE APPLYING PRESSURE. AS EE WAS APPLYING PRESSURE, SHOULDER POPPED. RELEASED PRESSURE FROM AROUND SIGN AND MOVED ARM AROUND TO SEE IF IT WAS HURT OR A ""NORMAL POP"", WAS JUST A LITTLE SORE. COMPLETED TASK & TOOK SOME IBUPROPHEN. REPORTED INCIDENT NEXT MORNING

EE WAS TAKING A LOAD OF MATERIAL TO OUR HOPPER IN HIS HAUL TRUCK. HE SNEEZED & JERKED HIS WHEEL, IN DOING SO HE RAN OVER ONE OF OUR CONCRETE BLOCKS THAT WE HAVE LINED ALONG OUR HAUL ROAD. WH EN HE RAN OVER IT, HE SLAMMED HIS HEAD IN THE INSIDE OF THE CAB, OF THE HAUL UNITS WINDOW LATCH CAUSING A CONCUSION.

EE WAS STANDING ON RT FRONT FENDER OF HAUL UNIT CLEANING MIRROR; WHILE STEPPING DOWN TO GET OFF, HE LOST FOOTING & FELL 4 FT TO GROUND. FALL CAUSED STRAIN TO LT KNEE & LT HIP.
