Repairs to the boulder breaker were being done by the employee. The breaker had been removed to weld cracks on the boom eye. When the injury occurred, they were in the process of putting it back into place. The employee was using a bar to align the pin and bushing, when the cylinder slipped and crushed his right middle finger while it was in the pinch point.

A 314 EP Butt Fusion System was being used to fuse pipe together. The injured employee was pushing up against the system to align the pipes together. When he pushed hard enough on the machine, part of it moved and pinched his finger between two parts of the machine.
