Accident type, without injuries

A contractor notified the Operations supervisor of a gas well leak in the D3 Area. The gas well valve was shut in and the gas flow terminated. No injury or equipment damage

Contractor was cutting hay in D3 area when the hay cutter contacted a rock causing sparks igniting the grass. The fire burned approximately 20 acres before being extinguished by water truck and Texas forestry department. MSHA hotline was called at 1:25pm. The fire burned for approximately 3 hours and 35 minutes.

Fall from ladders

Employee was standing with one foot on a shelf and one foot on the top rung of a folded ladder apprx. 4' off the ground reaching up to remove an extension ladder being stored on top of a break room. When EE grabbed the ext. ladder, one section slid out causing the employee to lose balance and fall from the shelf and ladder upon which EE was standing on.

Struck against a moving object

Employee was operating a Case 620 scraper tractor with pans loaded with red dirt headed to the dump area. The employee drove through a rough spot in the road and jammed EE's left pinky finger in the steering wheel. Employee reported the incident to the Supervisor and was taken to the Emergency Room. X-rays confirmed there was a small fracture in the left pinky finger.

Accident type, without injuries

At 9:45pm a person on highway 782 called 911 to report a grass fire in the Oak Hill Mine DII Area Reclaim. Fire departments responded and extinguished the fire at 10:25pm. Approximately 1 acre of land involved. No injury or equipment damage. Cause: Lightning strike.

Fall onto or against objects

Employee was using a steam cleaning unit to wash oil from the engine of a D9T Dozer. He was standing on the left dozer track when he slipped and fell, landing on the track, resulting in right shoulder pain. He worked full duty until his surgery on 1/28/2016.

Dozer operator was pushing dirt in D4 Area when he stopped for break. After dismounting the dozer he was stretching when he felt a pop and experienced pain in his right knee. He worked full duty until 11/7/2015 when he took pain medication that prevented him from operating mobile equipment.

D-34 Dragline was digging a box pit in the D4 Area when the spoil dirt slid into the left shoe of the dragline damaging the handrails and causing the dragline to stop production.

A mechanic was performing maintenance on a Caterpillar 930G Loader in the shop. When was ascending the ladder to the cab he heard and felt a pop behind his left knee when he was on the last rung. Employee worked full duty until surgery was performed on 02/12/15.

Employee was working on a dragline hoist drum using a 1 1/2"" impact wrench to remove bolts when he felt pain in his right shoulder. On January 12, 2015 he was diagnosed with tendonitis in his right shoulder.

A spoil slough occurred in the pit after coal loading was finished. A dozer and loader were working in the pit cleaning up. All personnel were removed from pit. No injury or equipment damage or entrapment occurred. Contributing factors- Unbalanced Spoil Placement, Near ramp (spoil split to dig ramp), Near gas well, Additional dig-off material, Toe of Spoil compromised.

A fire was noticed on a oil/gas well site location in the D2 mining area. Two tanks (400 barrels capacity each) containing oil and salt water were involved in the fire. The local fire department responded and the fire was extinguished within 75 minutes. MSHA was notified at 12:56 pm. No one was injured. Probable cause - Static electricity inside one of the tanks.

Over-exertion in wielding or throwing objects

Using hammer to loosen bucket tooth, struck tooth adaptor on bucket and a fragment of the tooth ear broke off and went inside EE left leg.

Employee was installing a suction hose on a 6"" water pump in the Wright Branch pit in D4 area when he experienced pain in his lower back on left side. He was taken to ETMC Henderson where he was diagnosed with a back muscle strain. He was instructed by the doctor not to work the next day.

Employee was in his office at the Main office complex when he experienced chest pains, sweating and nausea. He was transported by ambulance to the Good Shepherd Hospital in Longview Texas where he was diagnosed with a clogged artery in his heart. He underwent a procedure at the hospital to install two stints in his heart.

A miner was performing a pre-operation inspection on truck/vehicle. While getting into truck he had to sneeze and when he turned his head to sneeze he hit his right eyebrow on the door of the truck causing a laceration above his right eye, on the eyebrow, which required 3 stitches. Miner had on all PPE when injury occurred.

Employee was operating an End Dump. At the end of the shift he parked the end dump on the ready line north of the shop. As he was dismounting the machine he felt a pop in his right knee when he placed his right foot on the ground. The employee worked regular duty until June 26, 2014 when an MRI indicated a fractured bone at the end of the femur lopes on the upper part of the knee.

Employee spotted smoke coming through the woods. Employee followed the smoke until he came up on the fire near the Mt Vernon Cemetery off of CR 216. Fire Department was called to the scene to put the fire out. The fire is believed to be caused by lightning strike yesterday morning. The Texas Forestry Service was called in to trench around the fire.

Employee was cleaning coal in the pit on a D10R dozer. At approximately 12:30pm he started experiencing pain in his lower back. He did not report his condition to his supervisor as an on the job injury until April 11, 2014. The injury became recordable on April 17, 2014 when he was issued work restriction by a doctor.

Fall to the walkway or working surface

Maintenance employee was washing the lube truck, when he finished washing one side of the truck he started walking to the other side and tripped on a piece of grating. The employee fell, but was able to catch himself with his hands before hitting the ground. He sustained a lumbar strain/sprain to his lower back from the fall.

A mulching machine was clearing a pipeline right of way. The mulching machine contacted a pine tree and the top of the tree broke off and fell onto a set of power lines. An arc from the power line started a fire which spread rapidly. Three fire departments and the forestry service responded. The fire burned approximately 4 acres and was contained within three hours.

When installing axle in final drive housing on left front of 815 cat compactor employee was pushing down with right hand sliding axle into place when right hand became pinched between snap ring and sun gear on axle shaft causing injury to right hand.

Injured employee was walking on uneven terrain with another employee unrolling barb wire and the wire slid on the support bar cutting the employees left forearm. The employee was taken to the ER for a Tetanus shot and the ER Doctor decided to close the wound with glue and two steri-strips for support .

Employee was loading a D9T dozer on a haul truck trailer. After getting the dozer loaded and exiting the machine, he accidentally stepped off the outside edge of the trailer falling approximately 4 feet to the ground landing on his right hip. Employee was taken by ambulance to the hospital were he was diagnosed with a fractured right femur.

Two electricians were moving a section of dragline trail cable when it became snagged on a stump. The stump broke free causing the trail cable to sling forward striking the right ankle of the electrician working on the ground. He was diagnosed with a sprained right ankle at the emergency room.

Fall from machine

Employee was installing rear pack on a 637E model scraper. The pack caught on ladder mounting locations and when came loose it shifted back about 10"" contacting employees head. He lost his balance causing him to fall 4.5 feet to ground. Contact with ground cause injury to employee.

Fall from machine

Employee was climbing down from the cab of the Peterbilt truck-tractor when he lost his footing and fell backwards to the ground. The employee tried to brace himself with his left arm, subsequently suffering a fracture of his left wrist.

Absorption of radiations, caustics, toxic and noxious substances

Two employees were in a pump truck that parked at the top of the D3 #1 ramp. One of the employees got out of the truck. While outside he dropped his snuff can and when he reached down to pick it up a water moccasin bit his left middle finger.

Worker was standing too close while opening the hood to a Kubota tractor. When the hood opened worker thought the hood of tractor was going to hit him in the face so he put his hand up to guard his face causing his hand to hit his mouth and breaking out his bottom tooth.

A grass fire was noticed at 1:25pm at the north end of the D4 Mining area near county road 272. The local fire department and the state forestry department were called. The fire was extinguished at 2:30pm. No equipment or injuries occurred. MSHA was called at 1:35pm. CAUSE - spark or cigarette from a vehicle on the public county road.

Employee was descending a filter fan house ladder on D-34 Dragline (Marion 8750) when his foot slipped off the top rung. His left shin (6 inches below the knee) contacted the angle iron corner of the fan shroud guard causing a laceration requiring 2 stitches to close. No restrictive duty issued.

Over-exertion in lifting objects

Employee was lifting a plastic 6"" water discharge pipe when he felt pain in his lower back. His pain increased and on 1/25/12 he was examined by a doctor and prescribed physical therapy.

Employee was stepping off and marking the cut for the dragline. He was walking in soft sand when he felt a pop in his right ankle and felt pain. He was taken to the emergency room and diagnosed with a sprained ankle. He was placed on restrictive duty.

Employee was removing the discharge line from a water pump by cutting the 6"" flexible hose with a folding razor knife when he cut his right leg just above the knee. Two stitches were required to close the wound.

A gas leak was discovered at a gas well near the ramp road to the pit. The well company was called to investigate. MSHA was also called at 8:33pm. When the gas company arrived they found no leak. The gas company stated the gas release was probably caused by a relief valve that can sometimes open for a short time then re-seal.

Employee was returning to his backhoe in a creek bottom which has tall grass and weeds. Before he mounted his backhoe, a snake (water moccasin) struck him on his lower right leg.

Employee was unhooking a fertilizer buggy from a truck. He was using the jack welded to the frame of the buggy to jack it up. After jacking it up he pulled the pin to unhook the buggy from the truck causing the buggy to shift. The shift of the buggy caused the weld to break on the jack that connected it to the buggy frame causing it to smash employee's foot.

At 5:00pm a grass/tree fire entered the mine premises at the north end of the D4 area. The origin and cause of the fire is unknown. The north end of the pit was evacuated at 11:00pm due to reduced visibility from smoke. Local fire departments and Texas Forestry Department were called. The fire was contained at approximately 6:00am on 9/6/11. No injuries or equipment damage.

A grass/tree fire was discovered in a heavily wooded area in the D4 area. The fire department was called and a contractor water truck was sent to the location to fight the fire. MSHA was notified of the fire at 3:08pm. The fire was controlled at 4:00pm. CAUSE OF FIRE - Unknown possible cigarette butt or lightning.

A fire was discovered at 2:55pm in the D2 North mining area caused by a downed 25 kv overhead power line. The fire department was called and the fire was under control at 4:30pm. MSHA was notified of the fire at 3:36pm.

Mechanic was dismounting haul truck when he twisted his right knee- employee stated he must have stepped on a rock or clod of dirt. Leg was put in splint and he was issued restrictive duty.

An AEP 138 kv air switch in the Oak Hill Mine DII mining area experienced an arcing event that caused hot metal to fall to the ground and ignite the grass and trees near the area. MSHA was called at 3:05pm. The local fire departments were called and the fire was contained at 4:45pm. MSHA Supervisor visited the area on August 5, 2011.

Employee arrived at work and told his employees he was up all night with stomach pain. He continued to experience stomach pain and nausea. During a meeting he left the room and collapsed in the hall. CPR and AED was performed until ambulance arrived. Employee died on the way to the hospital.

At approximately 4:05pm lightning struck water and oil storage tanks at a gas well compressor station site owned by Anadarko Inc located in the DI Mining Area. Two tanks were distroyed and the oil/distillate caught fire. The MSHA hot line was called at 4:42pm.The local fire department was called and the fire was extinguished at 5:40pm.

Employee was getting his lunch box out of the back seat of the shift change pickup when he hit his right elbow on the edge of the pickup door. The elbow swelled and was drained by a doctor on 3/22/11. No restrictive duty was issued.

Struck by flying object

Employee was setting on a metal bench near the operation building when wind blew dust in his right eye. The next day he requested to see a doctor. Doctor stated the sand in his eye caused an infection. Prescription issued.

Over-exertion in pulling or pushing objects

Employee was releasing air pressure from an end dump tire when the vapors caused him to start coughing. He started sneezing approx 1 hour later. During the sneezing he started to spit up blood. He reported this incident two days later. Doctor determined the vapors triggered the coughing and sneezing irritating his sinuses causing the blood.

EE was operating a D10T Caterpillar dozer. After his lunch break he started having chest pain, sweats & nausea. He eventually contacted his supervisor who in turn called an ambulance. An EKG, x-rays & blood work did not indicate a heart attack but it did show elevated white blood cell count. EKG results indicated irregular heart beat.

An older T-post snapped in the frozen ground as the employee attempted to remove it from the ground by hand. As the post snapped, it struck the employee's safety glasses causing a laceration to the nose. The weather, frozen ground, and age of the T-post were a contributing factors in this incident.

Fall from machine

Employee was returning from the restroom to his end dump. He slipped on the second rung of the access ladder to the end dump and lost his grip and fell 3 feet to the ground. His back struck the ground and caused a bruise to his lower back. He was taken to the hospital in Henderson, Texas and given seven days of light duty.

Fall to the walkway or working surface

Employee was checking for grease leaks around the swing shafts in the dragline revolving frame. While he was in #1 swing shaft room he stepped on the sloping floor near the base of the swing shaft. This action caused him to turned his right ankle inward causing him to fall to the floor. When he stood up and transferred weight to his right foot he heard a pop and felt extreme pain.

A mechanic was installing tires on haul truck. As he was tightening the clamp up on the wheel the tire turned a bit and his finger was underneath the clamp. He smashed his finger when he ran the inpact to tighten. The tip of his right index finger was fractured and he also required 3 stitches to close the laceration.

Employee has been experiencing numbness in both hands. On 9/30/10 he was diagnosed with carpal tunnel syndrome in both wrists and cubital tunnel syndrome in his left elbow.

A mining supervisor noticed a fire in the D 1 area behind the pump barn where oil and gas workers had been mowing pipeline right of way. The fire was put out within 20 minutes of being seen. No Miners were working in the area. This did not affect the Oak Hill mining operation. Estimated 5000 sq ft of underbrush was affected.

A mechanic was helping a group of employees install a roller and roller guide assembly onto a Hitachi Backhoe under carriage using a forklift. The assembly fell off the forklift and mashed the mechanic's left hand fracturing 4 of his fingers.

Employee in the DII Mining Area noticed smoke in the DI Mining reclaim area. The supervisor investigated the area and noticed a fire burning in the tree farm near 782 highway. The water truck and local fire department extinguished the fire in one hour. CAUSE - Wind re-ignited embers from the 8/25/10 grass fire in the same area.

Employee was fueling a dozer when he slipped off the dozer track he was standing on. He fell and hit his back on the edge of the lube truck platform ending up on his feet on the ground. He was diagnosed with a cracked rib on the left side of his back.

While baling hay in the D1 reclamation area, a roll of hay caught on fire inside the baler, when it was kicked out, it caused about 7 acres of hay meadow and about 7 to 8 acres of an adjacent pine plantation totaling about 14 to 15 acres to burn. The fire never affected the mining operation and did not leave Luminant property. The fire was contained in about 1 hr and 30 minutes.

Employee was dismounting a front end loader. As he was descending the stairs his ankle turned and he fell down the stairs to the lower platform. During the fall he landed on top of his left leg. This action tore the tendon of his quadriceps muscle near the knee. Employee was placed on restrictive duty.

Lightning struck an oil storage tank at gas well location in the DIV area. The tank was destroyed and the oil ignited inside the containment berm. The fire department was not equipped to extinguish the flame so the well owners decided to allow the fire to burn itself out. The fire stopped at 10:00pm. No injuries or equipment damaged.

A backhoe was incorrectly loaded onto a lowboy trailer whereby the boom was positioned high enough to allow contact with a highway underpass bridge.

Employee was dismounting the shift change bus at the beginning of the shift when his foot slipped on the bottom step. When he reached out to break his fall he hit his right hand on the step well panel wall breaking his right little finger.

Employee was in the process of grinding a piece of metal when he got his thumb against the grinding wheel. This caused the loss of his thumb nail and top of his thumb through the last digit only.

at 5:24 pm the mine manager and operation mananger noticed a grass fire in the DIII mining area. The fire was located in an area behind the mining activity near the intersection of abandoned hiway #850 and county road #421. The local volunteer fire department was called out and extinguished the fire at 6:15 pm

On 7/14/09 a dozer operator contacted Operation Supervisor to inform him of a grass fire on the NW side of the DI Area Auxiliary Coal Stockpile. 2 water trucks were called to the scene & 2 dozers were used to cut a fire break. The fire was not under control within 30 minutes so MSHA was called(3:45pm). The fire Dept. was called & arrived at 3:50pm. Fire was under control by 4:15

Struck by flying object

A mechanic was assisting a group of employees change our a drag chain on a dragline bucket rigging. One employee was utilizing a loader with a hydraulic hammer attachment to remove hitch pin. During the pin removal a chip of metal flew off the hammer head and struck the other mechanic in the left forearm requiring surgery to remove the chip and repair a damaged artery in his arm.

Employee was assisting with installing pins on the right dragline fairlead sheave. While repositioning his body to enter the manlift, his foot slipped causing him to fall back into the fairlead assembly. His left arm struck a chain hook on the catwalk handrail causing a laceration with stitches.

On May 19,2009 at approx. 7:38 am at the Oak Hill Mine a mechanic was found dead in the cab of a fuel truck. It appears that his death was due to natural causes. An autopsy will be performed to determine the cause of death. There are no indications that an accident contributed to his death.

While operating a 740 end dump, the employee slammed on the brakes to avoid an incident. The employee stated that their knees hit the radio and ashtray in the truck.

Employee was in the process of holding a PVC pipe up from being damaged by a backhoe, the end of the PVC pipe pulled loose from the recently glued fitting, striking the employee in the upper lip which caused a laceration that required four stitches.

At 12:15 pm on 3/27/09 Oak Hill Mine experienced a high wall slide in the DII Area at the west end of the pit. #34210 loader was cleaning coal at the locations and was trapped by the wet soil. A backhoe was called to the area and the loader was pulled out of the slide material at 12:47 pm. No one was injured and there was no equipment damage. MSHA hotline was called at 12:48 pm.

Employee was rolling up a 1 1/2"" water hose at the bottom of the clean coal plant, when she lost her balance at the edge of the concrete slab. When she reached out to grab something to keep from falling, she hit her right thumb on a metal pipe. She did not report the incident until 3/6/09. She worked her regular job duties unitl she had surgery on her thumb on 4/28/09.

Caught in, under or between a moving and a stationary object

Employee was working with a group of mechanics installing a drag hitch keeper plate on a dragline bucket. A IT28 loader with a hydraulic hammer attachement was being used to drive in and hold the keeper plate in place. While installing the plate bolts, the IT28 unexpectedly moved mashing the employees left middle finger between the hammer head and the plate requiring 4 stitches.

Employee was replacing a trunion pin on the dragline bucket lower drag chain when the pin slipped and mashed the tip of his left middle finger between the pin head and the top of the trunion causing a hair line fracture on the tip of the finger.

Operator on #34226 Cat loader was loading out 3 haulers in the DII area pit just west of the #4 ramp. As he was loading #32039 hauler, a spoil slide occurred (approx 120' long by 45' wide). The slide pushed the rear of the hauler over approx 3' into loose coal causing the hauler to become stuck. The loader & haul operator along with two others were able to walk to a safe location.

EE could not reach coal with lancer, EE went in the coal bin throught the side hatch door, EE worked in postion for about 30mins standing on the coal pile working the coal down the shute with the lance when suddenly the coal dropped quickly sucking him down into the coal fines up to his neck.

Employee was installing an inline fuse in a pump skid. When he pushed the fuse into the holder it broke. Glass from the fuse cut his right hand between his thumb and first finger. 4 stitches were required to close the wound.(misc mail 12/24/08 #1-surgery on 11/14/08 repair nerve damage return 12/23/08 with 18 days restrictive)

Employee was installing drain pans under a conveyor system and bumped his right elbow on the edge of the pan and cut right arm above elbow and recieved stitches.

Employee was stepping up onto barge pump in the auxiliary pit in DIII Area, when his right knee gave way. He was diagnosed with strained knee ligaments. He was given a knee brace and placed on restrictive duty. (No operating heavy equipment)

Electrical Laborer cut the middle finger at 1st joint below knuckle. Cut occurred when using drill ""chuck key"". Cutr required 4 stutches.

Employee was assigned the job of changing out a blown hydraulic hose located below the hitch on a haul truck. As he was positioning himself below the hitch, his left foot slipped and he twisted his left knee. The employee was diagnosed with a torn medial meniscus of his left knee on 4/2/08. He worked regular duty until his knee surgery on 5/9/08-returned to full duty on 6/16/08-

Accident type, without injuries

A spoil slide occurred at 2:45pm on 2/28/08, in the Oak Hill Mine DII Area pit. The slide was approx 250' wide and 125' across the pit to the toe of the highwall. The slide blocked in a euclid hauler and a caterpillar loader. The operators were able to walk out of the pit. No injuries and no equipment damage. MSHA was notified. Investigator arrived at 3:45pm.

Employee was installing a cover over a hauler engine. As he lifted the cover into position, he felt pain in his middle back. His pain increased until his doctor prescribed physical therapy on 7/1/08.

Over-exertion (Not Elsewhere Classified)

Employee was in process of removing a nut from a gate hinge with a pipe wrench. During process of removing nut, in the motion of pulling on the pipe wrench to release the nut, employee strained his right shoulder. On 11/9/07 a MRI was given to EE, it was determined he would require surgery. Lost time began on 11/8/07.

A spoil slide occurred at 2:35am on Oct.10, 2007 in the Oak Hill DIII Pit Area. Slide was approx. 200' long x 80' wide. The pressure from the spoil material caused the 8' coal seam to fracture & rise approximately 20'. The action damaged a hauler & loader working in the area. No one was injured. MSHA was notified. Investigator arrived at the mine site to investigate @ 10:20am

Accident type, without injuries

A power line contractor came by the maintenance planner's office and told him he noticed a gas pipeline leak near the auxillary stockpile area. MSHA was immediately called and the haul road near the leak was blocked to thru traffic. The pipeline was closed in and the pressure bled down. The haul road was re-opened at approx. 1:30 pm. Repairs completed at 6:00 pm 9/28/07.

EE was changing ripper teeth on a motorgrader. He hit one of the teeth with a hammer and a sliver of metal came off the tooth and hit the EE in the chin. At 10:42 a.m. a TXU employee smelled gas while traveling down the northwest road. He stopped to investigate and found a small gas pipeline leak on the north side of the haul road approx. 30' from the roadway. A pipeline company representative was called to the location. The pipeline was closed and pressure released from the line by 3:30 p.m.

D-36 Dragline was stripping in the DIII west area, when the bucket cut throught a plugged gas/oil well casing. Residual conderisate liquid inside well casing ignited. On 4/30/07, employee was washing hauler tire, when he slipped on the concrete slab and twisted his right knee. He did not wish to see a doctor until 11/19/07. At that time the doctor diagnosed strained knee ligaments and place his knee in a brace. No restrictive duty was issued. EE had surgery on knee 2/15/08. He returned to full duty on 3/31/08.

A grass fire was noticed on the north side of the RR tracks approx 200 yards east of Oak Hill coal silo. The mine water truck & local fire dept., extinguished the fire in approx. 35 mins. No injuries, equipment of facility damage. MSHA hot line was called at 1:42pm. Msha ret'd call at 2:20pm. Probable cause; sparks from coal train.

Operator was attempting to ascend a hill at the location, when the dump truck slid backwards down the hill striking a parked dump truck. The operator sustained neck injuries. There was damages to the right side hood and front fender of the parked dump truck.

Cond.Contr.-Frontend loader dumping on a 4 to 1 slope. Loader was in a articulated position. Refuge pile was loose. As loader went up pile to dump, pile gave away and caused loader to tip over. No injuries were sustained to the operator and their were no damages to the frontend loader. The operator was wearing his seat belt.

Employee was in process of hooking up a 6"" barge pump, he ran into a problem with the discharge hose not attaching. Employee began to over heat during this period. Employee was noticed to be overheated and clammy. EMT transported employee to Emergency Room where he was treated for heat exhaustion and released.

While lifting a power supply transformer (250 lb) from a pallet the employee felt pain in his right knee. This injury became MSHA Recordable when he started physical therapy on 8/14/06.

Employee was helping a group of mechanics change out a dragline bucket. While replacing the bolts on the drag hitch, the pin keeper fell and cut the employee's left fore arm requiring 4 stitches.

Employee was removing a line boring tool setup from a Komatsu loader. During removal all welds were cut loose except one. Employee grabbed the tool setup with both hands and twisted loose the remaining weld. Suddenly, the setup rolled out of his hands and fell, catching his left hand's 5th finger between the tool and the main frame of the Komatsu loader.

After placing the fuel nozzle in the tank, and while waiting for the fueling to complete, he decided to sit on the step and wait. While turning to sit on the step he mistepped and fell from the track, causing a broken rib.

EE WAS DIAGNOSED WITH WORK INDUCED CUBITAL TUNNEL SYNDROME IN HIS RIGHT ELBOW AND CARPAL TULLNEL SYNDROME IN HIS RIGHT WRIST ON NOVEMBER 29, 2001. THE COMPANY WAS NOT INFORMED OF THIS INFORMAT ION UNTIL MAY 23, 2002. HE HAD NOT HAD ANY RESTRICTIVE DUTY OR LOST WORK DAYS ASSOCIATED WITH THIS ILLNESS.

EE WAS USING AN IMPACT WRENCH TO REMOVE TIRE STUDS FROM A HAULER. HE PERFORMED THIS TASK FOR ABOUT 3 HOURS. HIS RIGHT ELBOW STARTED HURTING BUT HE THOUGHT NOTHING OF IT ON THIS DATE (10/6/01). A FEW DAYS LATER, HIS ELBOW STARTED HURTING CONTINOUSLY SO HE REPORTED IT ON 11/3/01 AND DECIDEDTO SEE A DOCTOR. HIS CONDITION WAS DIAGNOSED AS TENDONITIS.

EE WAS LIFTING A PIN INTO THE PICK UP AND STRAINED HIS BACK. NO DR APPT AT THAT TIME. HE CONTINUED WITH BACK PAIN AND SAW HIS FAMILY PHYSICIAN IN JUNE OF THIS YEAR 2001. WHO DIAGNOSED HIM WITH ARTHRITIS AND SAID IT WAS NOT JOB RELATED. AFTER CONTINUING PAIN, THE INJURED WENT TO A SPECIALIST DR WHO RAN SEVERAL TEST AND DETERMINED AT THE DR ON 9-26-01 THAT THE EE'S PAIN IS JOB RELATE

WHILE REMOVING CHAINS FROM A SPREADER BAR, THE EMPLOYEE KNEELED DOWN IN A GRASSY AREA AND PLACEDHIS LEFT KNEE ON A SHARP OBJECT (ROCK). HE THOUGHT HE HAD SUFFERED A STONE BRUISE AND CONTINUED TO WORK. HE STARTED HIS SEVEN DAYS OFF AND HIS KNEE CONTINUED TO SWELL AND HE HAD TWO STREAKS RUNNING DOWN HIS LEG. DR DIAGNOSED HIS CASE AS BURSITIS.

EE WAS CHANGING OUT A FUEL NOZZLE ON A LUBE TRUCK AND THE WRENCH HE WAS USING SLIPPED OFF OF NOZZLE AND CAUSED HIS ELBOW TO TWIST. THIS CAUSED HIM TO HAVE SHARP PAIN IN HIS ELBOW. THE EE MADE AN APPOINTMENT TO SEE THE DR. ON MONDAY, MAY 22,2001. DR EXAMINED HIS & SAID THAT HE HAS TENDONITIS IN IT. HE HAD BEEN TREATED 3 WEEKS PRIOR TO THIS INCIDENT FOR THE SAME PROBLEM/ SAME ELBOW.

ON APRIL 30 TH, HE WAS EXAMINED BY DR. AND SCHEDULED FOR ANMRI ON MAY 4TH. ON MAY 18TH DR PERFORMED SURGERYON HIS LEFT KNEE & IT WENT WELL. EE IS CURRENTLYRESTRICTED TO LIGHT DUTY(DESK) FOR NEXT 6 WEEKS.

EE STATED THAT HE HAD BEEN USING A 20LB SLEDGE HAMMER DURING A DRAGLINE OUTAGE ON 3-24-01 AND ON 3-26-01 AS HE STARTED TO RISE FROM A DREEPER, HIS BACK STARTED TIGHTENING UP HURTING AND CONTIN UED TO GET WORSE THROUGHOUT THE DAY. THIS CAUSEDHIM TO HAVE PROBLEMS WHILE STANDING, SITTING, AND BENDING OVER. THE DR CONCLUDED THAT HE HAS A LUMBAR STRAIN.

EE WAS CALLED TO CRUSHER BECAUSE TWO 6"" PIECES OF METAL CAUGHT ON THE MAGNET. ANOTHER EE USED APRY BAR TO HOLD THE COVER OF THE STAMLER CASE OPEN WHILE HE ATTEMPTED TO PUSH THE COVER ON OVER. AS HE PUSHED UP, THE PRY BAR SLIPPED & THE COVERCAME DOWN & PINCHED HIS PALMS BETWEEN THE COVER & CASE. THE PALM OF HIS RIGHT HAND SUSTAINED TWOLACERATIONS & HIS LEFT HAND WAS BRUISED WITH ONE
