A FATALITY OCCURRED AT THE WINDFIELD SOUTH MINE ON AUGUST 21, 2001 AT APPROX. 11:30 A.M. MECHANICS HAD BEEN ASSIGNED TO TURN THE DRAG ROPES AND PUT ON ONE DUMP ROPE ON THE D-22 DRAGLINE. THE D RAGLINE BUCKET WAS CONFIGURED SUCH THAT THE ARCH TO EAT. A CLOD OF DIRT FELL FROM THE BUCKET. APPARENTLY STRIKING THE VICTIM ON THE BASE OF THE NECK. EE ON THE SCENE IMMEDIATELY ATTEMPTED TO

Accident type, without injuries

F-150 got stuck in the mud. Ignition was shut off and materials were gathered to put under the tires for traction when crew noticed smoke under the truck. They used the fire extinguisher and made phone calls to notify Luminant and 911. At this time, fire started on the truck between the bed and the cab of the truck. Fire Department arrived to extinguish the fire.

Accident type, without injuries

Fire was started by lightning strike at H area at Winfield South mine. Fire department and forestry service responded and extinguished the fire and cut fire road around fire. Approx. 9 acres affected. No injuries or property damage. Fire area was monitored to ensure it did not reignite. Fire was extinguished in approximately 1 hour and 20 minutes.

Caught in, under or between a moving and a stationary object

Individual was operating torch to cut sections of plate. While checking how much remained to be cut, the piece came loose of the structure, pinching the upper ring finger against plate below. This caused a laceration through the welding glove as EE pulled the hand free. The laceration required three stitches. X-rays were forwarded to radiology and resulted in negative findings.

The employee was pulling on a steam cleaner hose and tripped over dirt clod and fell back and strained employee's left hamstring.

Accident type, without injuries

A contractor was cutting a hole in the roof of the 2570 BE dragline to make ""A"" leg repairs. During this process sparks fell around the drag gear case and ignited grease. Efforts to extinguish fire with portable fire extinguishers was not successful and the local fire department was called out to extinguish the fire. No injuries to the miners and fire fighters.

Accident type, without injuries

Sparks from locomotive started extremely dry grass on fire. The fire started off the mine at approximately 11:20 and spread on to the property at approximately 12:20 p.m.. At that time a Luminant supervisor called the MSHA 800 number to report it had burned 30 minutes on mine property. The fire was located in a non-active area of the mine.

Accident type, without injuries

Lightning struck an arrestor on a power line and caused sparks to fall down on an extremely dry field. There was no miners fighting the fire but fire departments from Winfield and Mt. Pleasant came out to control the fire. The location of the fire was in a remote area of the mine, no mining activities were close to this event. The fire burned approximately two hours.

Accident type, without injuries

Fire was started beside railroad track in a remote area of the mine. The fire was extinguished by the Mt.Pleasant fire department at approximately 7:35 p.m. No damage was done to any equipment nor injuries to personnel fighting the fire. No miners were involved in fire fighting efforts. Cause of fire was not determined.

Employee was helping another employee to remove a becket pin from an intermediate cable and the pin hung up and then the pin came loose and dropped on cable and smashed little finger of left hand.

Employee was moving trail cable and got his pinky finger of left hand piched resulting in several stitches.

Employee was in process of sawing through a PVC pipe that was located in an area that was causing tension on the pipe. When the employee cut completely through the pipe, the tension caused the pipe to snap up and trap his finger between it and a metal support. This caused a laceration on the top side of the right index finger of the employee.

Injured was putting trail cable shoe on the back of the truck and dropped it on his middle finger of left hand.

While stripping dirt from spoil side with 2570 dragline the spoil became unstable. There was no equipment damage or injuries due to this event. The reason for reporting is that it interrupted coal haulage for over one hour.

Was climbing back on his D10 dozer when his foot slipped & he caught himself with his left foot, hper-extending it upward, pulling tendons in his left lower leg.

Employee was installing wear plate on dragline bucket. As he turned to get another piece of metal he tripped and landed on his side resulting in two cracked ribs.

Employees were hooking up to a truck to pull a kink out of a 6"" collapsed pump discharge line that was laying embedded in the grass. When the truck began to pull the discharge line and remove the kink the hose became very taunt, then instantly the hose snapped out of the embedded grass and hit the employee on the side of his lower left leg.

Employee was driving a t-post with a t-post driver, as he raised the driver up above the t-post the driver had come off the top of t-post, on the way down, the employee's finger was caught between the t-post and the handle of the t-post driver. This caused the end of his finger to be severely cut between the two pinch points.

Employee was removing upper penant line pins at the boom point of a dragline. He was using a Link-Belt to hold socket up while driving the pin out. As the pin was being struck with 5 ft. pry bar it came out causing the pry bar to swing back and strike the employee in the left side of the head.

EE WAS SQUATTED DOWN ON LEFT SIDE OF THE ENGINE COMPARTMENT TO LOOSEN A SMALL BOLT. WHEN HE STARTED TO STAND UP HE FELT A CATCH IN HIS BACK.

This accident occurred on 5/20/2005 but was not reported till 5/25/2005. At D-24 the spoil was sliding towards the power cable. Employee got the dozer operator to help him move cable with the dozer. The rest of the cable he had to move by hand. The dragline was held up while moving the cable into shoe print where the shoe would not drag the cable when it turned.

EE WAS DISMOUNTING LEFT SIDE OF LOCOMOTIVE TO REPAIR FLASHING MARKER LIGHT. STEPS WERE WET AND EE SLIPPED OFF FROM SECOND STEP FROM TOP. EE HIT HIS RIGHT ELBOW ON STEP AS HE WAS SLIDING HANDS DOWN THE HANDRAIL.

EE WAS ASSISTING WITH THE INSTALLATION OF THE PLANETARY OF A SCRAPER. WHILE MANEUVERING THE PLANETARY, AN EYE BOLT THAT WAS BEING USED TO ATTACHTHE RIGGING BROKE. THIS CAUSED THE PLANETARY TO FALL AND STRIKE THE EE. EE WAS STRUCK ON THE RIGHT SIDE OF BODY IN THE BACK, HIP, KNEE, AND CHEEK.

EE WAS CLEANING THE TOP OF THE MACHINE HOUSE ON THE CROSS PIT SPREADER. IT HAD BEEN RAINING & THE AREA WAS SLICK. HIS FOOT SLIPPED OUT FROM UNDER HIM & HE FELL. HE FELL FORWARD & ATTEMPTED TO BREAK THE FALL WITH HIS HAND. THIS RESULTED IN A DISLOCATION OF HIS LEFT THUMB. THE THUMB WAS PUT BACK INTO PLACE AT THE E.R. & SPLINTED.

EE WAS ASSISTING WITH A DRAG ROPE CHANGE. WHILE IN THE PROCESS OF ROUTING THE ROPE THROUGH A SOCKET, THE ROPE BECAME STUCK. EE STRUCK THE ROPE WITH A FIVE FT PRY BAR, THE ROPE DISLODGED AND STRUCK HIM ON HIS LEFT ANKLE. THIS RESULTED IN AFRACTURE.

NOW REPORTABLE

THIS RESULTED IN A 1"" LACERATION TO THE LEFT INDEX FINGER.

EMPLOYEE WAS CHANGING THE FUEL PUMP ON A COAL HAULER. HE STEPPED UP ON A LADDER WHICH WAS UNDER THE HAULER IN ORDER TO REACH THE FUEL PUMP. AS HE STEPPED UP HE STRUCK HIS HEAD ON THE AIR FILTE R BRACKET.

EMPLOYEE REPORTED WEAKNESS AND PAIN IN BOTH WRISTS. EMPLOYEE WAS SEEN BY A PHYSICIAN WHO PERFORMED NERVE FUNCTION TESTS. BASED ON THE RESULTS OF THOSE TESTS, THE PHYSICIAN WHO DIAGNOSED THE PAIN AS CARPAL TUNNEL SUNDROME. EMPLOYEE WAS STILL ABLE TO PERFORM ALL ASPECTS OF HIS JOB UNTIL 2-14-02 WHEN HE UNDERWENT SURGERY ON HIS RIGHT WRIST.

EMPLOYEE WAS REMOVING A HOIST BRAKE AIR CYLINDER FROM A DRAGLINE. AS THE CYLINDER WAS BEING LOWERED, THE CYLINDER HOUSING FELL, STRIKING EE ON THE LEFT HAND. HE WAS TAKEN TO SEE HIS PERSONAL P HYSICIAN ON DECEMBER 31, 2001. X-RAYS WERE TAKEN AND THEY WERE NEGATIVE. ON JANUARY 28, 2002 AN MRI WAS TAKEN DUE TO CONTINUED SWELLING IN THE HAND. THE RESULTS REVEALED A CRACKED BONE.

EE IS STILL ABLE TO PERFORM ALL ASPECTS OF HIS JOBAT THIS TIME AND IS TAKING PRESCRIPTION MEDICATION FOR PAIN.

A SLIVER OF MET AL (1/4""X 1/16"") AND SEVERAL SMALL FRAGMENTS HAD TO BE EXTRACTED BY A DR. ONE STITCH WAS RQUIRED DUE TO THE EXTRACTION INCISION.
