While repairing a damaged trunion tire, the employee needed to trim a piece of rubber in order to get the puller installed. The box knife being used slipped lacerating the employee's left ring finger.

Caught in, under or between a moving and a stationary object

Employee was attaching a trailer mounted pump to a track hoe bucket in order to move the pump into position. The track hoe bucket unexpectantly lowered, catching the employee's hand between the bucket and pump frame.

Absorption of radiations, caustics, toxic and noxious substances

While treating mine water at Outfall 2, hydrated lime in the hopper escaped through a gap at the flow adjustment plate and hit the employee in the eyes. Employee immediately began flushing eyes with bottled water. Due to severe irritation, the employee was taken in for medical evaluation.

Struck against stationary object

While repairing shredder tire, a sliver of metal stabbed employee in the left thumb. Employee removed the metal but a small piece remained embedded in the thumb causing infection and swelling.

While attempting to remove a machine guard to tighten drive belts employee experienced ""pop"" and tingling sensation in left shoulder.

Employee strained lower back while attempting to lift a four-inch rubber discharge hose that was partially filled with water.

EE was traveling from the shredded clay storage shed to the BC control room along a travel and fell onto concrete floor. Fall caused a laceration to EE right eyebrow, fracture of his cheek bone, and a contusion to his right shoulder. He also scratched his right hand while attempting to arrest the fall. After Med evaluation 11/15/13 EE needs surgery

EE was attempting to reinstall a bucket tooth pin on an excavator. He was using a shop hammer to reinstall the pin and inadvertantly struck the tip of his left index finger, causing crushing injuries to the finger tip.

While using a sledge hammer to dislodge material during railcar unloading, the employee felt a pain in his lower abdomen/groin. Employee reported the injury on 10/14/08 at approximately 7:00am. The employee was taken to the doctor & was diagnosed with a hernia.

Employee was replacing the bolts on the cutting edge of the dozer blade. The round, flush mount bolt rotated as it was engaged by the pneumatic impact wrench causing a laceration to the employees left index finger.

EE WAS ABOUT TO SCRAPE AN OLD REGISTRATION STICKER OFF THE WINDSHIELD OF THE WATER TRUCK WHEN HE DROPPED THE RAZOR-TIPPED SCRAPER WHICH CUT HIS WRIST.

A 35"" X 35"" X 56"" SUPERSACK FILLED WITH 1 TON OF POWDERED CLAY BEGAN TO FALL OVER. EE ATTEMPTED TO CATCH IT AND WAS PUSHED AND TRIPPED, FELL, AND SHATTERED HIS RIGHT ELBOW WHEN HE STRUCK THE C ONCRETE FLOOR.
