EE opened doors to the unit reached inside to clean a press head when the machine activated and moved pinching their arm between a fix and moving part, this cause an abrasion to the forearm requiring first aid. 5/2 EE asked to be seen by Dr resulting in restricted duty from Dr.

Employee was exposed to caustic fluid during clearing of a plug and cleaning of the area to their feet when their boots and socks became saturated with the fluid causing chemical burns.

3rd party driver was delivering copper slag to the plant. They had dumped a load of copper slag and was parked. Driver was standing near the rear of the truck and flagged down another driver for help complaining of chest pains. EMS was called, and driver was transported by emergency services to a local hospital where they passed away.

Employee was poking through a poke hole clearing limestone plug, a hinged door was open above poke holes. another employee was hitting on side of chute at same time. The hinged door fell down pinching the employees right pinky between door and bar. this cause a laceration and the nail to be partially removed requiring stitches to be repaired.

The worker was removing V-belts from pulley in order to remove a cover to repair a seal on the quarry water pump. When moving a v-belt to a lower groove, it trapped the tip of the glove on the finger area, and when it moved again, pinched the workers finger inside the glove, between the v-belt and the pulley.

Accident type, without injuries

At 9:58pm the control room operator received an alarm from the thermal cameras on the shredded tire storage pile showing a potential hotspot/fire. The control room operator dispatched crew members to evaluate the situation, and based on their evaluation, the fire department was called. The fire lasted approximately 31 min, no damage occurred except to the tire pile.

Heavy Equipment Operator was walking around equipment performing pre-operation inspection while EE slipped on an ice patch, due to winter storm. The miner landed on EE's right side, flaring up an old shoulder condition. The injury was first aid until 02/14 when physical therapy was prescribed.

The miner was working on the relocation of a steel hopper on a conveyor belt. The skids of the hopper got stuck on the conveyor belt frame, so the miner approached to inspect when the hopper had a sudden move pinching the tip of EE's left Index finger in between one of the hopper columns and the conveyor structure, causing a cut on the skin of EE's Distal Phalanx.

Maint. employee struck shin on concrete ledge on 1/27/22. Employee had pain off/on for next few months until outage treated with OTC meds/ice. Outage employee walked/climbed stairs a lot, pain still there. MRI on 4/1/22 showed tear in patellar tendon, PT prescribed.

Employee hanging barricades for preheater plug when plug flushed through line, and out cracks in cooler doors. Dust got into EE eyes, causing cornea abrasions in right. Meds prescribed.

Accident type, without injuries

L2 pre heater tower tertiary cyclone flushed with hot clinker material into the clinker pit. This caused the grease on the deep bucket elevator chain to catch fire. The fire traveled up the belt conveyor outside of the pit, where flames were seen, and emergency services were called. The fire was extinguished by the local fire department within 34 minutes. No injuries occurred.

Process engineer was looking up at process fan and dust got into left eye causing a corneal abrasion requiring prescription medication.

Employee was pushing on a silicone patch on the bottom of a screw conveyor housing. Silicone was covering a hole. Employee's right index finger went through the hole and made contact with the screw, partially amputating the underside of the index finger under the nail.

The incident that led to the smashed finger occured while the subject was attempting to remove several bolts and leaned a hand against the fan blade and in doing so the fan blade moved because weight was applied to it. Began to free spin and that movement is what caught the subjects finger between the case of the fan and the blade.

EE was leaving work for the day and as they pushed the building door closed, felt pain in their right shoulder. Tried otc and self-care until 12/18/2019 when was seen by dr and given prescription meds.

Employee was shoveling beside and under a ramp. EE pushed the shovel under the ramp and mashed EE's right thumb between the side of the ramp and the shovel. This ""strike"" broke the employees right thumb. As a result of the break EE cannot use EE's right thumb for a short period.

Employees raising an aerial lift to clean & did not see a conduit bank. Lighting insufficient but were using hard hat lights. Employee struck hard hat on conduit bank causing employee to strike chin on the left side causing a laceration. Employee was transported to hospital & received stitches for the laceration. Employee released to go back to work same day with no restrictions.

Employee was lifting hinged bearing cap on the belt winder. The cap sprang back quickly, smashing the employee's right middle finger. The right middle finger sustained a small laceration and fractured bone.

EE was removing a feed belt. Belt became bound during the removal. EE reached up and grabbed the rigging with both hands to release the tension. The belt clamp ripped out of the belt at this time. The rigging slid through the employee's left hand, pulled gloves off, lacerating EE's left palm, requiring stitches.

Employee was tapping a finish mill liner over with a ball pin hammer when a shard of metal broke off striking ee on right knee, causing a very small cut. EE thought ee was fine but swelling would not go down. Employee was taken for Xray on 12/7 that showed imbedded metal in right knee. Antibiotic given. Ortho referral to remove the metal. Placed on restrictions.

Employee was walking on catwalk underneath the kiln when something blew into EE's eye. Diagnosed with scratched left cornea. Given meds/drops.

Employee's finger was smashed while attempting to re-install a large guard. Right ring-finger tip was amputated from middle of fingernail upwards.

Service line Truck driver lined truck up improperly underneath spout.EE accessed top of truck using safe access platform. When forcing open hatch against the spout, ee slipped. EE struck nose/face against the handrails, causing broken nose/lacerations. Truck company refuses to gain contractor ID number and file injury themselves.

At approximately 3:25pm, a minor explosion was heard/detected in the Line 2 AFR hopper/belt/elevator system. Crew responded and tried to extinguish fire however, they could not safely extinguish the fire. Fire department called by the control room operator, and two fire departments responded (Midlothian//Ovilla). The fire was extinguished approximately 54 minutes after it started.

Employee was installing kiln trunnion bearing, while ee working on the bearing pedestal platform. As ee stepped down from the platform, right knee made contact with a piece of angle iron attached to the platform, causing bruise. Treated with OTC meds until Xrays/MRI taken. Ortho prescribed anti inflammatory and phys therapy on 9/5/17.

Center alternative fuel pile caught fire inside AFR shed for reasons unknown. Subsequent piles inside shed also caught fire. Fire depts called, MSHA hotline called. No injuries.

EE was inspecting dust transport screw. The system went positive as the employee was turning to close the screw inspection port cover back on, covering the employee in bypass dust. Employee suffered chemical burns to both eyes.

On 6/17/16 we were told the employee worked their normal shift sometime in late May and the following morning their right wrist was sore. Their wrist was not struck by or against any object. Employee has continued to work normal schedule. On 6/22/16 doctor diagnosed the condition as tendinitis and applied a cast.

Employee was installing a 6"" water pipe. As the pipe was set into place, both index fingers became caught between the pipe and the stand, causing fractures to both fingers.

Steel erection. Drilling/enlarging flange hole. Drill bit caught in/on the flanges hole material, binding/locking the drill. The rapid twisting motion rotated the drill handle left and shocked the operator's hand consequently forcing loss of hand control. Resulted in wrist sprain and Fx of metacarpal

Employee was installing doors on an airslide in a finish mill building. A gust of wind blew through the building doors, causing dust to get into the employees left eye. Employee suffered a scratched cornea and was given antibiotic and numbing drops.

The employee was standing on outside of Line 1 Spray Tower & using jackhammer with 12' bit through 3'X3' access door to clean inside the tower. Inside the structure a chunk of material fell on the jackhammer bit causing the bit to force the jackhammer out and down. The jackhammer struck the employee's right ankle causing a fracture.

Employee was using a jack hammer to clear a large chunk of material. The chunk broke free striking the jack hammer causing it to twist his wrist. Employee thought his wrist was sprained was given first aid and returned to work. He called our office on 08/25/2015 and told us his wrist was not better. We took him to the doctor for an examination and learned that his wrist was broken.

Employee was returning from getting water in Maintenance Office. Upon re-entering the loader cab he put his right hand where door closes and utilized his left hand to shut the door. He thought he could move his right hand before the door shut. The door shut on his right middle finger. He reopened the door and removed his finger.

Employee was climbing fixed ladder on burner floor to bring down cardox equipment. EE was 4-5 rungs up (4-5ft), when hard hat came off. At the same time, the right foot missed the next rung and the employee fell. The employee tried to stop the fall and fell forward. EE's right elbow and hand made contact with the floor.

Employee was getting off of the Maintenance truck. As he was coming off the truck there was a oil spilled on the bumper causing him to slip and fall to the floor. Fell to the floor on right side of body.

Employee was attempting to loosen liner plate in finish mill using a 8# sledge hammer. Employee struck liner plate and a small fragment of metal spalled from the liner plate and struck employee in the right upper leg. Fragment was removed by emergency room personnel and wound was closed using a single staple. Employee released to full duty without restrictions.

Employee was cleaning slots in division screen of #3 F/M using jackhammer. Small piece of metal went up under safety glasses sticking him on the right side of nose and a piece of metal ended up in left corner of right eye.

While using a jackhammer in cooler his left hand was on the handrail (scaffolding) and he was pulling back on the jackhammer with his right hand. When the jackhammer came loose the handle pinned his left hand between the handle and the handrail of the scaffolding.

Employee was loading cement into a rail car and fell off the side when his fall protection harness take-up reel locked up unexpectedly, causing him to lose balance. He sustained a laceration to his right thumb requiring sutures and a small cut/contusion to his right cheek.

EMPLOYEE WAS DRILLING HOLES IN A PLATE OF 1/4"" STEEL WITH A HAND DRILL MOTOR. THE DRILL BIT BOUND UP IN THE STEEL, CAUSING THE DRILL MOTOR TO KICK BACK, FORCING THE EMPLOYEE'S RING AND PINKY FINGERS BACKWARDS. HE WAS DIAGNOSED WITH A FRACTURE TO THE RIGHT RING FINGER.

Employee was working in raw mill with dusty work environment, did not notice any problem until he woke up the next day with irritated right eye. Employee was examined by clinic physician and a dust particle was removed from the surface of the eye.

Employee was operating a jackhammer on the bypass at L2 preheater when he felt a sharp pain in his lower back. He immediately put the jackhammer down and started working with an air lance. The pain continued and employee was taken to doctor.

Employee was walking out of Finish Mill 3. He had oil/grease on his boots. He stepped into a puddle of water and slipped, twisting his ankle. Employee initially refused any medical attention, but decided to go to a doctor when his ankle began to swell on 11/3/05. His doctor ordered that he not wear his safety footwear until 11/7/05, which restricted his normal duty assignment.

Employee was attempting to slide a metal panel when the corner slipped and pinched his fingertip. He reacted by pulling his finger away which caused a laceration on the fingertip. The laceration required two sutures to close.

Employee was installing pump on water truck when pump slipped and struck his left thumb, causing fracture near nail bed.

Employee claims he was using an oxyacetylene torch to cut duct work from top of finish mill. Pieces of hot slag allegedly entered employee's shirt through a gap in the sleeve and caused burns to forearm area. Employee failed to report injury and burns became infected, causing employee to seek medical treatment a week after injury.

AT APPROX. 9:20 AM ON MAY 20, 2005, AN EXPLOSION WAS HEARD COMING FROM LINE NUMBER ONE. THE DEFLAGRATION WAS OBSERVED TO BEING AT THE ELEVATOR BOOT & CONTINUED TO THE TOP OF THE ELEVATOR. THIS EXPLOSION WAS CAUSED BY THE IGNITION OF A DUST CLOUD INSIDE THE ELEVATOR.

Employee was shoveling material out from under 311-BC1 belt conveyor when he claims he struck a secured bolt with his shovel, causing his right wrist to hyperextend. Employee did not report injury until he felt initial pain on 05/04/2005.

EE was at bottom of bypass dust silo performing inspection on 2 bypass dust extraction systems. EE thought isolation gate was closed. EE claims he opened port downstream of gate on airslide to verify gate was closed. When EE opened port on airslide, material immediately began flushing from silo. EE inhaled bypass dust during incident.

Employee was working inside the raw mill roll removing outer bearing race using a wedge and hammer. Employee claimes he was driving the wedge between the bearing race and cage when a piece of race came out and struck him in right thigh.

Injured employee & co-worker used strap attached to forklift to lower Pfeiffer grinding segments into open box. While lowering segment, strap stuck on front of one fork. Injured employee claims that when he used his right hand to pull strap off fork, segment weight shifted in box applying a load on strap causing strap to pinch employee's finger between strap & fork.

Employee claims he was getting set up to cut wood on the welding table in the maintenance shop. When he set circular saw on welding table, an 8' x 2', 1/2"" thick piece of steel that was sitting on the welding table fell on the top of his left foot. The piece of steel was hanging off welding table and the weight of the saw was enough to drop steel off table.

EE WAS CLEANING BYPASS AREA. JACKHAMMER GOT STUCK IN BUILDUP. WHEN EE PULLED ON THE JACKHAMMER, THE LOCKING LATCH THAT HOLDS THE BIT OPENED AND THE BIT FELL OUT OF THE JACKHAMMER AND STRUCK THE TOP OF EE'S RIGHT HAND.

Employee claims he was avoiding a mud puddle as he was walking through the RMS building and ran into a fixed pipe protruding from the south side wall. The pipe hit his upper left front tooth causing 1/3 to break off.
