Contact with hot objects or substances

Employee was using a cutting torch without wearing gloves, when the oxygen lead came off the torch, causing a 2nd degree flash burn to the employees left hand.

8/27/09 While stepping down from the 375 excavator, EE got his corrective boot hung in the step twisted his right knee. 8/28/09 he reported his right knee was swollen & needed go to the doctor. Taken to the doctor, told to ice the knee & he could return to regular duty. Back to the doctor 9/8/09, swelling not completely down. Dr gave prescription for anti inflammatory medication

Absorption of radiations, caustics, toxic and noxious substances

Employee has known allergy to poison ivy, his help was needed to help clean the ""green's"" pump. Due to the known allergy he was only to run the backhoe to avoid any contact with the plant. It was unknown that he would have a reaction to the pollen in the air from cleaning the area.

Unloading a pallet with a chain hoist the load shifted, the person helping unload the pallet let go of the pallet & employee's left hand was caught in the chain causing severe bruising and a break in the hand. Due to severe bruising of the hand the first xray did not show a break. Second set of xrays taken on 5/22 found a break which was visible due to reduction of swelling.

Employee was walking down the stairs from the 112 crusher and slipped and fell against the hand rail. When he fell, he injured his right shoulder. Doctor restricted his duties to no lifting over shoulder height with right arm.

Employee was driving a haul truck to the pit to get loaded and hit a bump; he stated that he felt a sharp pain in his back. The employee went to the first aid box for aspirin and then went back to the pit and hit the same bump and felt a ""pop"" in his back and then reported the incident to his coordinator.

Struck by falling object

EE was inserting a metal wedge into travel cylinder pin under dragline. As he reached down to grab a hammer the wedge slipped out and hit him above the left eye brow. This caused a laceration that required 10 stitches.

EE was preparing to install a new conveyor belt. When he went to turn the roll of belt it pinched his left index finger between roll and idler causing a fracture of the tip of left index finger. EE is on restricted duty as of 9/25/07.

EE was preparing to re align rollaway chute with shaker screen. He was using a come a long to roll chute into place. The chute bound up with shaker and pinched his left hand between chute and shaker. This caused a laceration and broken pinky finger on left hand.

EE was working on quarry pickup truck replacing the brakes shoes and front end parts. He was stooping and bending when he felt a pain in his lower back and could not stand straight up.

EE was climbing onto 740 haul unit. As he was grabbing the handrail he felt something pop in his left shoulder. Arm went partially numb.

Employee was using a water hose to clean at the sand plant when he was stung by some sort of insect on the top of his right hand.

EE was operating fuel truck in pit. The front windshields were in the open position. As he was driving back to plant a foreign object flew into his right eye under safety glasses.

EE WAS CUTTING OLD CROWN BAR RUBBER OUT OF SHAKER WHEN KNIFE CAUGHT PEBBLE IN RUBBER. THIS CAUSED THE KNIFE TO JUMP TO EE'S HAND AND CUT THROUGH GLOVE AND FLESH. EE REQUIRED 4 STITCHES TO LEFT THUMB.

Employee walking back from bone yard to get into truck. As he was walking he stepped in a shallow hole and bent left knee back, reinjuring previous knee injury.

Employee was tightening head bolts on a diesel engine (haul truck) when the socket slipped off the bolt head, causing right hand to ram against truck frame, cutting index finger. Required 4 stitches.

While returning to the pit area employee fell asleep. Equipment crossed over a windrow, causing the operator to be knocked around withing the cab, causing injury to the employee's right side.

EE was working outside in a hay field. The wind was blowing about 25 mph. Debris was picked up and hit him in the eye.

Employee was driving down the haul road behind the Package Plant. His unit hit a dip in the road causing his seat back to fall back. While pulling himself up using the steering wheel, he strained his lower back.

Employee was walking on wet surface and slipped. His failure to use hand rails properly contributed to his fall and causing his elbow pain.

Employee was working on the 121 pump. While lifting a section of pipe with a comealong, the cover for the 121 pump's leg gave way, causing cover to fall.

Employee stepped onto hood of haul unit to cross over to other side of unit. In attempt to cross, a bolt caught his shirt causing him to fall and land on the hot exhaust pipe.

EE allegedly hurt his right shoulder pulling the handle that lifts the bed on the haul truck.

While cleaning shop floor with steam cleaner, stepped back onto a jack lying on the floor. While trying to regain balance steam cleaner shot hot water onto right foot penetrating the boot with the hot water. The result was 2nd degree burn to the right foot.

EE WAS PULLING A PULLEY OFF A SHAFT (TAIL PULLEY). THE PULLEY ENCOUNTERED RESISTANCE ON THE SHAFT AND BECAME HARD TO MOVE. EE PULLED HARDER AND PULLEY SUDDENLY CAME ALL THE WAY OFF THE SHAFT, HITTING THE EE ON THE KNEE.

WHILE INSTALLING A BEARING ON A HOUSING, THE BEARING CASE CRACKED INSIDE THE HOUSING. AFTER FLIPPING THE HOUSING ONTO THE OTHER SIDE, THEY FELL TO THE FLOOR, PINNING EE RIGHT HAND MIDDLE FINGE R TO THE FLOOR AND CRUSHING THE TIP OF THE FINGER.

EE WAS WORKING ON THE ENGINE COMPARTMENT OF A DRAGLINE AND A PIECE OF DIRT GOT IN HIS EYE DUE TOAN INWARD SUCKING VACUUM. HIS EYE BECAME IRRITATED THE DAY AFTER AND HE WAS TAKEN TO A CLINIC, G IVEN PRESCRIPTION EYE DROPS AND RELEASED TO FULLDUTY.

EE WAS WASHING HIS HAUL UNIT WHEN HE SLIPPED ANDFELL ON HIS RIGHT SHOULDER AGAINST THE FRONT TIRE OF THE HAUL UNIT. HE REPORTED THE INCIDENT IMMEDIATELY BUT SAID AT THE TIME HE WAS OK.

EMPLOYEE FELL OFF EUCLID B30. WHEN HE GOT CLOSER TO SCENE HE GOT UP OFF THE FLOOR. HE WAS PUMPING GREASE INTO FITTING ON THE OILER HITCH, AS HE WAS GETTING OFF FENDER HE SLIPPED OFF ENDER AND HE LANDED ON THE GROUND. HE HIT HIS HEAD AND RIGHT KNEE CAP AND HE HAD A SMALL SCRATCH ON RIGHT ARM.

WHEN EE JUMPED OFF THE TRACKS & ONTO TO THE HOOD OF THE TRK, & WAS WA LKED OVER TO THE PASSENGER SIDE & TRIED TO CRAWLTHRU THE DOOR WHEN HE LOST HIS GRIP & FELL BACKWARDS ONTO THE GROUND. HE GOT UP & INTO THE TRUCKAT THE TIME HE DID NOT COMPLAIN ABOUT ANY HURTI
