Fall onto or against objects

Employee was performing maintenance of sump pump/slurry tank of Scavenger Cyclone. Employee stepped onto a pipe, lost footing, and slipped, causing them to strike their shin on the pipe. This caused an abrasion and contusion to the left shin.

EE was stepping down onto catwalk after working on feeder hopper and as they stepped down they stepped on a rock. This action caused left ankle and knee to twist. EE notified manager but did not seek medical attention until 7/30/20. EE was given restrictions and referred to a specialist.

Employee struck right index finger with a hammer while repairing a return roller in the maintenance shop. The injury resulted in 2 stitches and 10 days light duty.

Miner was installing a metal ring on a water pump. He was using a 5 lb sledge hammer to drive the ring in. When he swung, the hammer missed the ring and smashed his R index finger between the ring and the hammer handle. He was wearing gloves. He cut his finger and cracked the bone on the end. Three stitches were required and a finger guard was placed on the finger.

Miner was releasing a 1 1/2 ton cable come-a-long to lower a take-up bearing on #201 conveyor belt, when the rubber grip slipped off of the handle. The handle struck the miner on the right forearm causing a 1"" cut. It took 5 stitches to close the wound. The miner returned to full duty with no restrictions on the same day.

Heard noise in motor area, went to investigate. Turned to leave engine room, radiator hose blew a 2"" hole in it, spraying water on lower legs & down into boots.

While installing transmission and tightening bolts on drive line, hands slipped off wrench while using considerable downward force. Fell forward, striking mouth & teeth on truck frame. Chipped front tooth, caused two more to become loose and pushed backward.

Struck by falling object

Welding on cyclone, had pipe suspended in air with come-a-long. Welding inside pipe. Come-a-long cable broke. Pipe came down on right arm, bolt went into interior rt. forearm and fractured rt. radius.

EE WAS TAPPING ON A TROUGHING ROLLER TO DISLODGE BUILT-UP SAND. HE WAS USING A SMALLER HAMMER THAN USUAL. THE HAMMER MISSED AND TRAVELLED INTO THE ROLLER PINCH POINT, TAKING EE'S RIGHT HAND WITH IT.

WAS ATTEMPTING TO ENTER THE 1979 JOHN DEERE MOTOR GRADER. PLACED RIGHT FOOT ON BOTTOM STEP, GRABBED BOTH HAND RAILS AND RIGHT KNEE POPPED. INTENSE PAIN AND IMMEDIATE SWELLING.

WAS INSTALLING A RACE IN SLEEVE ON TRACK HOE BUCKET. WAS HOLDING THE HANDLE OF THE DRIVING TOOL. ANOTHER EMPLOYEE WAS USING A SLEDGE HAMMER TO DRIVE IT IN. THE HAMMER GLANCED OFF OF THE SUPPOR T ON OPPOSITE SIDE, CAUSING IT TO CHANGE DIRECTIONS. HAMMER STRUCK RING FINGER ON LEFT HAND.

SEVERAL EMPLOYEES WERE REBUILDING A GEAR BOX. WHILE REPLACING THE GEAR & SHAFT, THE GEAR SLIPPED & CUT AN EMPLOYEE'S LEFT INDEX FINGER.

MINER WAS CLEARING HAUL ROAD WITH A 1996 CAT LOADER. THE BUCKET DUG INTO ROAD CAUSING THE LOADER TO SUDDENLY STOP, THROWING THE MINER INTOTHE WINDSHIELD. HE WAS NOT WEARING THE SEAT BELT . HE HAD BEEN INSTRUCTED AND TRAINED TO WEAR THESEAT BELT AND OPERATE LOADER SAFELY.

WHILE CUTTING BOLTS FROM DRAGLINE GEAR LOCATED BETWEEN FRAME AND TRACK A SMALL SLEDGE HAMMER WAS USED TO KNOCK BOLTS OUT. WHEN WORKING ON 3RD BOLT, FEET SLIPPED IN MUD CAUSING EE TO STRIKE SMA LL FINGER ON RIGHT HAND.
