Contact with heat

Plant operator suffered Dehydration/Heat Exhaustion while working outdoors. EE was drinking water all day but was not alternating with electrolyte rich fluids. After sitting in the shade for a while, the EE stood-up and felt light headed. When the EE experienced cramps requested medical attention. EE was transported to the local hospital where EE was administered 3 bags of IV.

Fall to the walkway or working surface

On 10/29/24, an employee was walking in the surge tunnel when EE slipped and fell. The employee sought medical attention and was diagnosed with a left elbow and left shoulder sprain. EE was issued restrictions and OTC medications.

Struck by flying object

In the process of removing a 990 loader bucket tooth with a soft metal hammer, a fragment of the tooth superficially entered the employee's chest. In order to remove the metal fragment, surgery is required.

Fall to the walkway or working surface

While washing material from a plugged crusher, the material flowed into the employee's legs, causing employee to fall to the same level. Diagnosed with concussion without loss of consciousness, strain to neck muscles, and contusion on arm and scalp.

While drilling out a cable fitting the drill bit pierced the employees glove and the outer portion of their left thumb. The employee received stitches.

A bar weighing an estimated 500 pounds fell 2 feet and landed on employee's left foot while doing maintenance on a feeder. Employee fractured EE's toe and received stitches. EE is currently out of work.

While removing a filter housing using a 24"" ratchet and socket, the socket slipped off the bolt and struck the employee in the mouth, cutting his lip and knocking out his top left front tooth.

Struck by flying object

The employee was part of a crew changing the blow bars on the base crusher. While working on placing a bar in the rotor, a small amount of material fell from the crusher bonnet above his work area. The material fell between his forehead and glasses and got into his left eye.

Miner was using chain to hoist equipment. Chain swung and struck miner in upper mouth, breaking a tooth.

Employee was assisting in changing out the blow bars in a Sandvik HSI crusher. He was repositioning the rotor in order to bring the next worn bar into position for removal. The rotor ""free-wheeled"" and in the process his right forearm became caught between the blow bar and the crusher side wall. He suffered a compound fracture to his forearm.

Employee was cutting on crown rubber with a razor-blade-type knife.The knife struck against a metal support. Knife jerked back and cut emplyee above the cuff of his gloves. Needed a doctor to stitch the wound.

Note:Late report by employee+originally denied as Workers' comp claim. He claims to have strained his back while lifting an 80-pound screen while attempting to access smaller screen behind it. He says that he lifted awkwardly.

EE WAS DISMOUNTING THE RENTAL WATER TRUCK. AS HE WAS DESCENDING THE TRUCK DOWN THE STAIRS-LADDER, HE FELT SHARP PAIN IN LOWER BACK. THE INCIDENT WAS NOT REPORTED TO MANAGEMENT UNTIL 2-12-02.
