Distribution employees were moving railcars in the Canyon and noticed a grass fire, fire department was called after it started spreading. No injuries and no damage to any equipment. Fire put out in approximately 50-55 minutes
Employee was trouble shooting an FK pump and got dust in EE's eye causing EE's eye to get scratched. Prescription eye drops were prescribed (no work restrictions)
Employee was walking back to the main office in the main parking lot and stumbled causing them to fall, they landed on their right hand and fractured it ""boxer's fracture"".
Employee was working on K1 tower plug and some material got on EE's neck causing a minor burn.

Exiting loader and slipped on last step of ladder and fell on ground causing a fracture to upper end of humerus
EE was removing cutting edge from belly pan of a 631 scraper. While removing the last bolt the edge popped off the pan, fell & struck the employee in the hard hat & safety glasses. The impact caused a laceration to the left eyebrow & cheek from the top & bottom shields of the safety glasses that required 16 sutures.
Employee was cleaning and got some dust in right eye, washed eye out that day. Went to work next day and said eye was irritated, took employee to clinic and they washed the eye out. No prescriptions were prescribed.
The injured party parked in a tight parking space/caught tip of right index finger in the truck door smashing the finger nail / EE received two numbing shots to the tip of finger to enable finger nail to be cleaned, then put splint and wrapped.
Tire chip chute plugged and caught the conveyor on fire. Approx 10 - 15 ft of the belt burned
EE squatted down to look under burner floor ramp when EE's knee popped. Diagnosed with knee sprain.
Employee was hooking up vibrator to rail car and a finger got pinched between the bolt on the vibrator and the railcar while they were tightening the bolt causing a laceration and a fracture to the tip of the right pointer finger.
Employee was removing final liner from the mill. The hammer dislodged a fragment of the line, the fragment struck the employee's right thigh causing a laceration.
Employee was installing drive belts and the tip of EE's right pinky finger got pinched between the drive belt and sheave causing a laceration and a fracture to the tip of EE's right pinky finger
Employee was unplugging an air slide and got dust in EE's eye. Prescribed eye medication
Employee was working on a temperature probe and got dust in EE's eye. Was prescribed eye drops
Employee was lancing the tower and felt a pain in back. The employee was taken to the doctor on 5/21/20 and diagnosed with a chest wall strain.
EE was inspecting rollers on conveyor on Finish Mill 3 while it was running. There are two tracks running above and below and while inspecting the one below EE accidently placed EE's hand above and the roller caught EE's left hand and broke one finger and cut two others.
Employee was trying to open a damper on a dust collector and hit self in the forehead with a pry bar causing a small laceration. The employee received 6 stitches.
Clinker coming out of K1 cooler was hot and caused the tripper belt to catch fire
Employee was making/installing hot boxes on the tower, EE left at the end of EE's shift and when EE got home EE called EE's supervisor and told supervisor EE thought EE might have gotten overheated. Went to med clinic and was given fluids and diagnosed with heat exhaustion/heat cramps.
Plant hand told our contractor to pull up chain and buckets with a rope. The ropes became untied from the other rope used and smashed the right, middle finger.
EE was working on the clinker breaker. They were trying to remove the clinker breaker hood when D ring that the comalong was attached to broke off and struck them on the left leg. There were supervisors on site and one was right next to them. EE had just turned around when struck on the left leg. EE was taken to the hospital and was determined they had an open fracture.
Employee was making a chute to empty out slag bin and cut right pinky finger on a piece of tin. The employee received five stitches.
Build up in the pulverized coal bin was exposed to oxygen during an outage and it started smoldering.
While holding tension on the cable while it was spooling back to the drum on a Grove GMK 5225 the employee raised hand to high and pinched right thumb between the sheave and the cable retaining pin. Employee got 8 stitches in right thumb. No lost time from work and no narcotics were prescribed.
Employee received chemical burns to left foot after hot material sloughed onto lower leg while cleaning in the Dog House. Material entered into employees left boot. Employee continued to work then felt discomfort after the end of shift. Employee then sought medical treatment.
We brought up coal mill A and the merrick belt caught fire. Belt burned up and a little damage to belt structure. There were no injuries
Employee was reinstalling a 4160 breaker when an arc flash occurred causing burns to the hands, face and arms.
Employee was working in dusty environment. The concrete dust irritated employee's skin on arms and back causing a rash.
Individual cut 2"" X 2"" X .25"" angle utilizing a shop band saw. Transported two 4-ft sections (12.76-lbs) by truck to the work area. Subject individual remained static, in a shaded area, for the next 45-minutes awaiting a craft technician to deenergize the equipment. Expressed pain in the left-arm, shortness of breath and dizziness. Temp:83°F, Wind: 12.7 mph, Humidity: 79%
On 5/11/17 employee cleaning tank with high pressure washer. Water stream from high pressure washer contacted their foot causing a laceration.
Employee was helping change out a dust collector bag on #2 Magaldi. Dust got into right boot. Employee did not think anything of it, so did not report it. Employee went to the doctor on Saturday January 21st. The doctor diagnosed the right foot as having an infection, so gave employee an antibiotic prescription.
EE was repairing lights in the Clinker Barn at Hunter Cement. Clinker dust fell from an overhead beam striking EE's hardhat & going under safety glasses into right eye. EE was given first aid at the plant to wash out eye & then taken to a clinic for examination. EE's right eye was again washed out & a prescription cream was given for the eye.
On 3-18-16 employee was knocking in a thermal coupling and sprained EE's right wrist. It turned into an MSHA reportable on 3-30-16 when it was found EE had a fracture.
EE WAS GRINDING EE WAS WEARING BOTH SAFETY GLASSES AND A FACESHIELD. EE FELT SOMETHING IN EE'S EYE AND RUBBED IT WITH EE'S HAND EE FLUSHED EE'S EYES FOR 15-20 MINUTES, WE DIDN'T FIND ANYTHING IN THERE BUT I TOOK EE TO HOSPITAL FOR WASH OUT. E.R SAID EE SCRATCHED EE'S EYE AND SENT EE BACK TO WORK. I BOUGHT EE SOME GOOD EYEDROPS EE IS FINE AND BACK TO WORK
Employee was trying to get a diverter gate to switch and when it did his finger got pinched between the pry bar and the housing causing a laceration/fracture to the tip of his left ring finger
On 07/01/2015 employee was trying to stand up base plate and it began to fall pinching his left hand in between it and the hand rail causing bruising and swelling. It did not turn into a reportable until 07/13/2015 due to doctor having to remove blood.
Employee was washing off a skid steer and got some water in his eye causing a minor infection
At 11:15 am the employee stepped on a finish mill ball in the finish mill area spraining his ankle. He tried walking it off on his own thinking it would get better but around 3:30 PM he was taken to the clinic and diagnosed with an ankle sprain.
Employee was cleaning a diverter get at the top of K2 buffer bins, when he finished he started to leave the area and ran into a beam with his head
Employee was walking back from break and slipped and strained his right knee.
On 10-3-2014 the employee was cleaning up some material with a shovel and strained his lower back. He did not go to the doctor until 10-4-03.
This was NOT an employee or a contractor. This was a CUSTOMER. He was closing the lid on his tanker and slipped and fell.
EMPLOYEE WAS UNPLUGGING A FEEDER IN THE RECLAIM TUNNEL. WHEN MATERIAL BROKE FREE EMPLOYEE STEPPED ON A CLINKER BALL STRAINING HIS RIGHT KNEE
Miner was cleaning a roller shim for roller placement. He unhooked his lanyard to move to a new location and did not stay 100% tied off. He tripped and fell backwards from the platform to the ground 14'9"" below. The handrail had been removed to allow the roller installation. He broke both wrists and his left rib.
Employee was pushing a roll into the roller mill with port-a-powers. As employee released a port-a-power to insert a spacer the roll came back pinching three fingers on his right hand causing lacerations and fracture.
EE was working on a machine. The cab of the machine came down and caught right hand. The right hand middle finger was fractured and there is a 1.8 cm laceration on hand.
Employee was attempting to light burner pipe. When he put the torch in the flame went positive causing him to get second degree burns to his face.
Employee went to get a sample from the alkali bag house. He got the sample and was walking back when he tripped and fell fracturing his left wrist.
Employee was holding the base of a ladder for an employee when a chunk of debris fell from the ceiling and struck the employee's forearm. The employee reported pain in the foreman where he was struck with debris.
Employee was walking down some stairs and stumbled. After he stumbled he felt a pain in his hip. Employee did not want to go to the doctor at this time. The pain has not gone away and the employee wished to see a doctor on 11/14/2013. Employee has been put on restricted duty as of 11/15/2013.
Employee was taking a tool box out of the lab cart and felt pain in his right shoulder.
On August 13, 2013, employee was climbing to the top of the water truck to check the water level and strained his back and leg. EMPLOYEE DID NOT REPORT THE INCIDENT UNTIL AUGUST 20, 2013.
Employee was loading a cement rail car. When he was finished he bent down to close the lid on the rail car. The rail spout was not raised high enough so when he stood up he hit his head on the rail spout causing a concussion/neck strain.

Employee was lancing finish mill airslide got dust in his right eye.
Employee was unplugging airslide with lance, material broke free and got on employee. Material went up her left pant leg and also on upper left arm and on face causing chemical burns on leg and arm.
Employee was unloading coal from a railcar. While closing the railcar door, he smashed his left pinky finger
Employee was rebuilding a cardox tube. Instead of tightening the head he loosened it causing some co2 to puff into his eyes.
Employee was moving a ramp to an upright position. It slipped and his 5th finger on left hand was caught between the ramp and hand rail. His left 5th finger was fractured and lacerated.
EE was fitting together process duct for the coal mill when he got his finger caught between the fit-up ring and stiffener.
EE had his hand on a handrail directing a suspended kiln hood being lifted over the handrail. The load shifted catching his finger between the kiln hood and handrail.
EE was trying to reposition an I-beam when he got his finger caught between it and another I-beam.
Employee was taking a 5 gallon bottle of water up the stairs to the fifth floor of the tower. He felt a pop in his right knee and was in alot of pain. He was taken to the doctor and was diagnosed with internal derangement of the right knee.
Employee was walking up some stairs when he felt pain on his upper right leg muscle. He went to the doctor the next day and diagnosed with right hamstring muscle strain.
Employee was operating a 973 track loader to move some clay. He developed a pain in his neck. He went to the doctor and was diagnosed with 2 herniated disks and returned to work with restrictions.
Employee was inspecting silo 4 by opening an inspection door on the airslide under the silo. When he opened the door the material puffed out of the door and some cement got in his eye causing eye burn to the left eye. He was taken to the doctor the next day for treatment and was prescribed some eye drops. Employee returned to work on the same day on light duty.
Employee was walking down the clinker outhaul tunner and he stepped on clinker causing him to roll his right ankle. He has a sprained ankle.
Employee was picking up trash in physical test lab and was moving cement cubes from one bag to another. He began having lower back pain and heard a pop.
EE alleges he was moving about on the ground floor of the raw mill wearing a harness. His feet got tangled up in the lanyard that was connected to the harness and fell, injuring his right knee.
Employee was picking up a roll of skirt rubber when he heard a pop in his right shoulder. He went to the doctor and was diagnosed with a shoulder strain. He returned to work that same day on light duty.
Employee was coming down from the sweeper and lost his footing causing him to hit his elbow on the sweeper. He went to the doctor and was diagnosed with an elbow sprain. He returned to work on the same day on light duty and returned to full duty on 11/09/2010.
Employee was walking around heat exchange floor. While he was walking he twisted his ankle. He was sent to the doctor and was diagnosed with a left ankle sprain. He returned to work the same day on light duty.
Employee opened the door under the rotary feeder while attempting to chain the door open. He lost his grip and it slammed on his hand. He was taken to ER with laceration to the left hand requiring stitches. He returned to work on the same day on restricted duty.
Employee was in #1 Magaldi working when a mixture of water and hot clinker got in his boot which caused a chemical burn to the right foot.
Employee was unloading a piece of water pipe from a trailer. He stepped back away from the pipe when the pipe started to fall. He stepped on a rock and lost his balance and fell. He was driven to the doctor and was diagnosed with a tail bone fracture. He returned to work the same day with no restrictions.
Employee was trying to use a jack hammer bit to pry a door shut and it slipped and it pinched a finger on his left hand causing a fracture. He was taken to the doctor and returned to work on light duty the same day.
Employee was looking up at burner pipe and dust flew into his right eye. His eye was irritated and employee went to doctor. He returned to work on the same day with no restrictions. The doctor gave him medication to help with the redness and irritation.
While checking on tower plug, cannon blaster went off spraying hot materical on employee. He had protective equipment on but hot material came in contact with skin. also when he took protective hood off, he hit a pipe and had a laceration on his forehead. Employee had 1st and 2nd degree burns and a cut on forehead that required stitches. Employee is on light duty.
Employee's hand slipped off while closing handle on an equipment container. It crushed his right middle finger between the handle and the container. Employee went to the doctor the next dayand was diagnosed with laceration and fracture of right middle finger. He returned to work that day on light duty.
Employee was opening the main elevator door upward. His right arm was pushed backward by the door causing injury to his right shoulder. He went to the doctor and was diagnosed with a strained shoulder. He returned to work on light duty.
Employee was at the Clay Pit and operating a Trac Loader. While backing up, the loader shifted and the movement caused pain to the employee's neck and back. He was taken for medical treatment and returned to work on light duty.
Was using magnetic drill press, used his hand which was gloved, to sweep away shavings. While the drill was running, his glove got caught in the drill, pulling his finger into the drill. He suffered an open fracture to his left index finger. He required stiches.
While bolting up wrong holed it and pulled bolt out and when he did this, his finger got caught in pinch point and smashed it. His finger was lacerated and the bone was fractured.
EE saw a 4X4 in the way of a duct being lowered down by chain falls and reached in between to grab it. He pinched his little left finger in between 4X4 and duct. Finger nail was smashed.
While bolting up a steel frame on ground , fork lift operator hit the frame with a outrigger while swinging around to unload trailer. The steel frame moved forward hit worker behind both ankles. He was taken to dr Xray: tiny avulsion fracture off the dorsum of left navicular the R ankle showed no evidence of fracture. He was put in a boot and given light duty restriction.
Employee was using Hi-Lift jack to level Mega-Stand that is used to lift aluminum dome. Employee did not lower the handle enough for safety latch to engage stop blocks. Employee took his hand off the jack handle and was raising his head, when the handle came back up to upright position, striking employee in the lower lip. Employee received laceration to lower lip.
Empoyee was moving on platform of scissor lift and tripped on lanyard cutting left side of neck below jaw.
Employee was removing a lug on a welding machine and hand slipped off of the wrench resulting in a lacertion across the top of the right hand. He was taken to the doctor for stitches and returned to work on light duty.
Employee was running to unplug coal mill B when he fell and injured his left ankle. He was taken for medical treatment and was diagnosed as a fractured fibula. Employee returned to work on light duty.
EE was cutting slip form from silos, as it was cut on one side the weight was to heavy for the other side, causing it to fall, which left EE hanging with his fall protection, at this time the other worker lifted him out. He was checked out and put on lite duty. Failure in communication and company policy.
Employee was rolling up a hose to load onto a truck. the hose started to fall and he attempted to keep it from falling. When he did he hurt his shoulder. Employee was taken to the doctor and diagnosed with a right shoulder strain. He returned back to work on light duty the same day.
Employee was using a trolley to move a motor on the 1st floor of tower. Going into the elevator wheel stuck between the elevator and motor floor causing the trolley to tip. The employee's left index finger was caught between motor and elevator injuring his left index finger. He was taken to the doctor. He received stitches and was released to return to work on light duty.
The employee was going down the stairs at #1 Magaldi and twisted his ankle. He went to the doctor and was diagnosed with a sprained ankle. He returned to work on light duty. He returned to the doctor for a follow up evaluation on June 1, 2006 and release to return to work to full duties without restrictions.
Employee was servicing analyzer and dust blew back under safety glasses and into his left eye. He was taken to see the company doctor. The doctor washed his eye out and gave him an eye wash. He returned to work.
Employee stated after unloading some coal he was driving on the road again and a gust of wind blew material from the bobcat bucket and some coal blew in his right eye. He was taken to the doctor and his eye washed out. Nothing embedded in the eye. He returned to work on the same day.
Employee was on the 2nd set of steps going to the alkali screw. He lost his footing causing his foot to slip on one of the steps, causing muscle strain to right leg calf. He was taken to the doctor and diagnosed with a strained muscle. He returned to work on light duty.
Employee was guzzling in the finish mills when his arm started to hurt. He has pain in his left shoulder resulting in shoulder strain. He returned to work on light duty. He was released to full duty after physical therapy on January 12, 2006.
Employee was climbing down some stairs next to the cooler. He twisted his right ankle and fell. He was transported to McKenna Hospital. He had a broken right ankle that required surgery. Surgery was done on Nov. 17, 2005. He is as home recovering. He has not been able to return to work.
Employee was checking cardox port for build up. He turned around and took hood off and bumped into HOA Tower. He suffered a laceration to the head requiring 2 stitches. He returned to work on light duty. He returned to the doctor on 08/19/2005 and was released to full duties.
Employee was lancing material out of the bottom of #8 GCT when a chunk of material fell hitting the lance. The lance came up & struck his jaw on the left side causing pain & swelling. He was transported to hospital and treated for bruising & pain. He was released & returned to work on light duty.
Employee was checking the fringe bin at finish mill #2. CKD came out of bin and splashed in his eyes causing his eyes to get irritated. He went to the doctor and was prescribed with an antiobiotic eye drop solution. He returned to work on the same day without restrictions.
Employee was using ball puller to remove balls from division screen at the #2 Finish Mill. He got his left thumb caught under the weight and smashed his left thumb. He went to the doctor and was diagnosed with a smashed and fractured thumb. He returned to work on light duty.
EE was using a jack hammer trying to dislodge a chunk from #8 clinker feeder when another EE shut the gate and struck the jack hammer causing it to strike the EE on the right side of the face and neck causing pain and swelling. EE was transported to the emergency room for treatment. EE was diagnosed with a facial contusion returned to work on light duty.
Employee was working in the cooler lifting buckets out during clean up duty. When he lifted a bucket he felt a pain in the abdomen. He was diagnosed by comapany doctor as having strained his abdominal muscle. He was released to return to work on light duty for 2 weeks.
Employee was working with the cooler drag chain. While moving a duct it fell over and hit him in the face cutting his bottom lip and the bottom gum inside his mouth. He received 20 stitches and returned to work.
He was walking down stairs at #2 FM. He stepped in a hole in the metal crate and lost his balance and twisted his right ankle. He has a right ankle sprain and did return to work on light duty.
The employee was climbing down off of FM#1 trunion. He stepped on the trunion pusher block and twisted his right ankle. He was diagnosed with a sprained ankle and is wearing an air cast and receiving physical therapy. Employee is currently on light duty.
Another employee was using a 4 lb. hammer to drive rivets through the conveyor belt. Injured tried to move the frame holding the rivets before the other employee was finished hammering. The hammer hit his right hand servering the index finger at the first joint.
Employee was working in the clinker barn. He suffered heat exhaustion. He was transported to hospital by ambulance after first aid was administered. He was treated at hospital and released. He returned to work on light duty.
Employee was walking on the first floor of the tower. The wind blew and stirred up some dust and some got in her left eye under her safety glasses. The dr.'s diagnosis was eye foreign body and corneal abrasion. Returned to work on light duty. She was to avoid dust and particles in the air. Returned to the doctor the next day for a check up and was released to full duty.
Employee was jackhammering material buildup in finish mill feeders. Material dust caused a rash on face, head and arms. Doctor diagnosed the rash as contact dermatitis and avoid dust, fumes and particles in the air and to wear long sleeves and also prescribed medication. Employee is on light duty.
Employee was working with his CB radio in distribution dept. when he hit his right elbow on his desk causing swelling. He went to the dr. and was given meds for inflamation & has taken it all with no improvements. He went on 07/02 for checkup and had fluid drawn from elbow and given antibiotics.
EE WAS CUTTING HOSE CLAMP WITH LINEMAN'S PLIERS. HE FELT SOMETHING POP IN HIS RIGHT HAND. THE HAND BEGAN TO SWELL AND HAD NUMBNESS. HE WAS TREATED BY OUR CO DR AND RETURNED TO WORK ON THE SAME DAY WITH RESTRICTIONS. HE IS NOW ON LIGHT DUTY AND RETURNS FOR A FOLLOW UP APPT WITH THE DR ON MONDAY JAN 26TH.
EMPLOYEE WAS HELPING TO LIFT SUGAR SCOOP AT THE KILN INLET. IT FELL AND HIT EE'S HAND CAUSING A FRACTURED RING FINGER AND AN OPEN FRACTURE TO HIS PINKY FINGER ON HIS RIGHT HAND.
WHILE CROSSING THE SCREW AT PROPER CROSSING, EE SLIPPED ON MUD ON STAIRS CAUSING HIM TO FALL AND TWIST HIS LEG AND HIS ELBOW.
EMPLOYEE WAS PRYING CONNECTION FOR HOSE ON SWEEPER HEAD WITH A CHISEL. THE CHISEL SLIPPED & HIT HIM CAUSING A LACERATION ON HIS LOWER LIP. HE RECEIVE 5 STITCHES & RETURNED TO WORK.
EMPLOYEE WAS TRYING TO BEND LANCE AND IT HIT HIM IN THE HEAD CAUSING CUT TO THE FOREHEAD.
EE WAS TAKING SAMPLE FROM SOUTH MILL CHUTE. CANNON BLASTER DISCHARGED FORCING MATERIAL UNDER EE'S TOWER SUIT HOOD AND INTO HIS EYES.
EMPLOYEE WAS STEPPING OFF FUEL ISLAND AND HIS LEFT FOOT SLIPPED AND HIS RIGHT FOOR CAME DOWN ON A 3 INCH ROCK CAUSING THE RIGHT FOOT TO TWIST.
EMPLOYEE STEPPED ACROSS SCREW AND SLIPPED ON MUD & FELL FORWARD CAUSING HIM TO TWIST HIS ANKLE.
EE WAS WALKING IN FRONT OF MIKE DAVIS'S OFFICE AND HE TURNED TO WALK ON SIDEWALK. HE THEN TURNED HIS ANKLE AND HIS KNEE.
EE WAS WALKING DOWN THE STEPS AND THE STEPS BROKE CAUSING HIM TO TWIST LEG.
PROCESS ASSISTANT WAS LOWERING A JACKHAMMER FROMBOTTOM OF STAGE II CYCLONE. EE WAS TOLD TO GET THE JACKHAMMER BUT THE JACKHAMMER CAME LOOSE FROM THE AIR COUPLING. IN ORDER TO AVOID BEING HIT BY THE JACKHAMMER, THE EE RAN AT WHICH TIME HIS ANKLE GAVE OUT.
THE EE WAS WALKING DOWN THE STAIRS AND LOST HER FOOTING FALLING ON THREE STEPS.
EMPLOYEE WAS AIR ARC GAUGING ON TOP OF #1 FINISH MILL AND CAUGHT HIS PANTS LEG ON FIRE. BEFORE HE COULD PUT IT OUT IT HAD BURNED HIS ANKLE.
WHILE TAKING BAGHOUSE DOOR OFF, SUCTION FROM BAGHOUSE PULLED DOOR BACK DOWN CAUSING LOWER, MIDDLE, AND LEFT SIDE OF BACK TO HURT.
EE WAS WALKING AND FELT HIS FOOT STARTING TO HURT. HE HAD PAIN BETWEEN THE ANKLE AND TOES ON INSIDE OF LEFT FOOT.

EE WAS PULLING GUZZLER HOSE AROUND DRAG CHAIN WHEN THE HOSE SUCKED UP A LOT OF MATERIAL CAUSING THE HOSE TO SHIP & SMASH HIS WRIST AGAINST DRAG CHAIN COVER.
WHILE WELDING A PIECE OF SLAG OR DUST BLEW INTO HIS LEFT EYE. HE TRIED TO WASH IT OUT BUT IT WASSTILL IRRITATED.
EE WAS AT THE FINISH MILLS CATWALK. HE STEPPED OVER THE HEAD PULLEY COVER AND STEPPED ON THE HOSE WHICH CAUSED HIM TO TWIST HIS ANKLE.
VICTIM STATED WHILE WALKING ON BURNER FLOOR TRIPPED OVER LANCE AND FELL AND INJURED LEFT SHOULDER.
EMPLOYEE WAS LANCING #8 GCT AND A CHUNK FELL ON THE LANCE AND HIT RIGHT LEG. EMPLOYEE'S LEG WAS BRUISED AND SCRAPED.

EMPLOYEE WAS LOOKING UP AT CONSTRUCTION WHILE WALKING. HE STEPPED ON A ROCK WHICH TURNED AND ROLLED HIS ANKLE.
EE WAS CLEANING INLET O2 ANALYZER & FORGOT TO TURN OFF AIR WHEN THE BLASTER WENT OFF AND THE HOSE HIT HIM IN THE MOUTH. IT CAUSED A LACERATION TO LIP AND A BROKEN TOOTH.
EMPLOYEE WAS USING THE WATER BLASTER WHEN A CHUNK FELL AND HIT THE NOZZLE CAUSING THE WATER BLASTER LANCE TO CATCH UNDER EMPLOYEE'S ARM.
WHILE CHANGING SIDE CASTINGS, DRAFT PICKED UP DUST DEPOSITED IN LEFT EYE.
HAD AN INSPECT DOOR OPEN ON INCLINE SCREW BELOW #5 GCT TO CHANGE THE TAIL BEARING ON THE SCREW. HOT DUST FROM #5 GCT BLEW OUT THE INSPECTION DOOR.

CLOSING DOOR ON RAIL CAR WITH A STEEL BAR. THE BAR TWISTED AND HE FELL ON THE BAR WITH HIS RIGHT SIDE.
EE STATES THAT HE REMOVED A KNIFE THEN SET ASIDE TO REMOVE A ROCK. THE KNIFE FELL OVER ONTO HIS FOOT.
KNOCKED COVER OFF HOIST WHILE MOVING A LADDER. THE COVER STRUCK THE EE ON TOP OF THE HEAD.
SEVERE SKIN RASHES ON LEFT CHEST, LEFT BACKARM AND RIGHT LEG WHILE WORKING IN RECLAIM TUNNEL. DUST WAS FALLING ON HIS CLOTHES AND SKIN.
WAS GETTING OUT OF A PICKUP AND FELT PAIN IN HIS LOWER BACK.
WAS LIFTING PLASTIC TO RUN BULL NOSE. FELT PAIN IN RIGHT SHOULDER AND COLLAR BONE.
