Struck by flying object

While brush cutting with tractor and 15 foot brush cutter operator hit an unseen rock which was propelled from the cutter through the back glass of tractor striking the operator on the right side of head causing a laceration. Operator was transported to clinic where laceration was stapled shut.

Accident type, without injuries

Power went out and electrician was sent to troubleshoot. Grass fire was found near track 140. Train traffic was stopped and no movement issued. Fire dept called and the fire was extinguished in 1 hr 15 min. No injuries or equipment damage. Area was monitored with no further activity.

A maintenance employee experienced a possible heart attack while working in the shop at the mine. MSHA 1800 hotline was called to report the event. After further investigation the maintenance employee was diagnosed with a personal medical illness in which was not work related.

Accident type, without injuries

A slide was noticed at 7:40 am in the A2 North pit. The spoil slide encroachment across the pit was minimal, but several large cracks appeared along a 600' to 700' long area of the spoil. Mining activity in the pit was stopped and employees were removed. MSHA was notified at 8:34 am.

The north highwall sloughed causing loose material to fall into the pit. Employees were removed the pit, MSHA was contacted. Mine supervision and Safety viewed the slough where it was discovered that the South highwall had sloughed after removal of equipment and personnel.

Employee sustained a small puncture wound to the left hand between the thumb and index finger requiring two stiches.

Dirt contacted the front windshield of a 992 CAT loader causing small glass shards to contact employee face. The employee was transported to the doctor to have a small piece of glass removed. The doctor prescribed anti-bacterial ointment.

Employee was approximately 3.5 hours into shift and bent to pick up a water hose while cleaning.

Fall to the walkway or working surface

The injured was picking up rebar close to the base of a concrete pile when she slipped on a loose piece of concrete and lost her balance and fell, catching herself with her left hand causing a fracture to her left wrist.

ATTEMPTING TO ADJUST THE FORKS ON A ZOOM-BOOM. THE FORK SLID AND PINCHED EE'S THUMB.

Employee started up ramp with loaded haul truck when the truck shifted from 1 to 2 gear, 3-4 times. Employee didn't seek medical attention at the time of the incident but did ask to see a doctor on 6/09/14. The hard shifting resulted in the employee suffering from cervical strain/spasm. He was issued restrictions on 6/10/14 from a follow up visit, making this a recordable injury

A fire was noticed burning in a wooded area that had been reclaimed. The local fire department and forestry service responded to the fire. The fire was contained by 1845 HRS that evening, but there was still many areas smoldering inside the fire break. The fire was monitored over the weekend and was completely extinguished on May 5th. The fire burned approximately 5 acres.

A fire was noticed burning in a roll off trash container near the remote shop. The fire burned for approximately 35 minutes before the local fire department was able to extinguish it. The trash container received some minor damage due to the fire.

Employee was utilizing a pick to clean ballast from beneath the rail for anchor installation. The bottom side of the pick head struck the base of the rail causing a small piece of shrapnel from the rail to strike the employee in the right thigh.

On March 4, 2014, the electrician reported that he strained his back the previous day. He did not wish to seek medical attention at that time. On March 11th, he asked to see a physician. The physician diagnosed him with a lumbar strain with sciatica. The employee was given restricted duty and referred for physical therapy.

The mine operator withdrew miners and equipment from the AII Railroad Pit when heavy rain began to fall. Approximately 1.28 inches of rain fell over a period of thirty minutes. A berm was constructed at the top of the ramp to prevent more water from entering the pit. The Safety Specialist called the local MSHA Supervisor who advised him to call the MSHA hotline.

A mechanic was removing the left side final drive of a D10T Dozer when the male end of the 3/4"" drive slipped out of the magnetic plug. The employee's left thumb struck the dozer's blade arm causing a fracture to the thumb.

The mechanic ruptured or detached the proximal tendon of his right bicep muscle while performing a bucket changeout on a Marion 8750 Dragline. The mechanic did not report the injury for four days. On February 10th, an orthopedic surgeon issued restrictions which made this a recordable injury. Physical therapy has also been approved and will be scheduled.

The employee was setting a pump when he twisted his left knee. He reported hearing a snap and feeling pain in the knee. He refused medical treatment at that time. On Jan 17, 2014, he asked to see a physician. He was diagnosed with a knee strain. After several doctor visits, his pain persisted. On Feb 21, 2014, physical therapy was prescribed, making this injury reportable.

Over-exertion in pulling or pushing objects

Employee was pulling a roll of flex line on an elevated bed of a pump truck with a 4"" strap. Another employee was pushing the roll. As the roll broke over the edge, the employee stopped it briefly and then let it go but attempted to slow the momentum of the fall and therefore the weight of the roll jerked his wrist from the weight causing pain.

Employee was operating 38077 Heavy Haul Truck when it skidded and jack knifed the trailer and striking the bump stop causing employee to bump his head in the cab.

Employee was performing maintenance on a D9T Dozer. His wrench slipped as the nut released causing him to lose his balance. His momentum propelled him from the track bumping his tail bone and striking his left knee. He was taken to a medical clinic for treatment. Left knee was braced pending an MRI and evaluation. No climbing, squatting, working at elevation and ladders.

An electrical pole with a cracked insulator located at the top of the pole shorted out when a bird landed on it. The birds feathers caught fire and fell to the ground catching the grass on fire.

In the A2 Area (Pit #45) there was a dirt spoil slide that disrupted the mining process for more than 1 hour.

While preparing to perform a drag rope re-socket job at D-33 Dragline, the employee placed a three foot pry bar into the drag chain socket wedge. He then turned to move to another location when he felt some pain in his lower back.

Employee was assisting in installing a dragline drag chain repair link when the employee's left hand ring finger was caught between the drag chain and repair link.

A railroad mechanic was repairing a hitch cushioning device on a caboose when stored energy was released causing the mechanic's wrist to contact an axle. The mechanic was diagnosed with an avulsion fracture of a bone in his left wrist.

Employee reported that he had been having first minor chest and right arm pains and then they began to get more painful. Employee was transported to the ER where he was diagnosed with a degenerative neck disorder which caused the pain. Employee was given restrictions which caused this to be a recordable on 8/30/2012.

An employee received a back injury while operating a Cat 777 end dump. The employee sustained back spasms. The Cat 777 seat suspension ride was found defective and replaced. The employee did not initially receive restrictions but it was determined on a follow up visit on 07/20/2012 that he would need physical therapy.

Fall down stairs

Employee was walking down steep stairs from Dragline. Employee tripped on on last stair, losing his balance. Employee twisted left knee on the ground.

Over-exertion in pulling or pushing objects

The mechanic was installing a serpentine drive belt on a 3/4 ton Chevrolet pickup. As he pulled on a 1/2"" drive, 16"" long ratchet to loosen the belt tension, he felt a sharp pain in his left rib cage. He reported the injury the following Monday. He was taken to a local clinic where he was diagnosed with a left rib contusion and was placed on restricted duty.

Employee was starting a pump during a rain event. He was walking back to the truck when his boot got stuck in the mud. When he pulled up on his boot to take the next step he lost his balance over the suction hose and fell. This caused the employee to suffer a sprained ankle.

A forklift was being used to install a pin in a dragline bucket's left drag hitch extension. The forklift operator was using the forklift tines to put side pressure on the pin to seat it. The tines slipped off the pin and contacted a scaffold that one employee was standing on. The jarring motion caused pain in the employee's lower back.

Working on pre-op-checking oil & water level in dump truck. Somehow caught right hand (at base of thumb) on a piece of metal causing cut. He had glove on hand that was cut.

The mechanic was adjusting the drag brakes in preparation for lowering the dragline boom. The employee was using a one inch drive impact wrench with a 2.25 inch socket to compress the brakes spring. As he was tightening the spring nut, the socket broke apart causing a laceration to the employee's upper right arm. Six sutures were required to close the wound.

Mechanic supervisor was hepling trouble shoot circuit breaker problem on D10R dozer. He was in an awkward position twisting bent over. When he had finished he dismounted dozer and was walking to his pickup when he experinced lower back muscle spasm.

Employee was tightening a bolt on a front end loader when the wrench slipped causing him to hit the back side of his hand. At the time of the incident no medical attention was needed. On 9/26/13 the employee said he had a knot on his right wrist from the incident. Dr. done surgery to remove the knot on 4/7/14. So this injury went recordable today.

Employee was riding in shift change bus when it hit two large holes in the roadway. The injured employee was sitting near the rear of the bus. The incident caused him to hit his head on the ceiling and be thrown to the floor causing pain in his lower back and an abrasion to his shin. Work restrictions were issued.

Employee was confirmed to have a standard threshold shift in his left ear. Retest was conducted on Feb 17, 2011 and results were received on Mar 2, 2011.

The EE was welding on a side cutter of a spare dozer blade n the maintenance shop. He was lying on his right side. A BB fell behind his welding hood and hit his right eye. A doctor used a cotton tip applicator to remove the metallic fragment. A (sample) eye drop medication was given to the EE.

Mechanic was removing small belly pan from Cat D6T XL dozer for cleaning. After removing all retaining bolts, the pan fell. The mechanic's right ring finger was pinched between the pan and a floor jack causing a laceration requiring sutures.

Employee was separating shim packs for upper bearing on main rotator of swing case #4 of a Marion 8750 Dragline. After heating the laminated shims, he was using a putty knife to work the shims apart. The putty knife caught and pushed the shim pack, slicing though his gloved hand causing a laceration to his left thumb and a cut tendon. *A 7% whole person impairment was issued.

At 10:45 PM, a large flame shooting out of the vent on top of a salt water tank on a gas well pad was discovered. The gas well company (XTO) was contacted and the fire department was dispatched. The fire department responded to the scene and extinguished the fire 12:15 AM. It was determined that a lightning strike caused the venting gas to ignite.

Employee was welding under the walking cam while another employee was machining on the bore of the walking cam. Employee felt something hitting him on the back and reached around to see what it was and lacerated his right index finger with a shaving.

The employee was cutting a pump flex-line with a utility knife. As he was cutting towards himself, the knife slipped and cut the inside of his right leg near the knee. He was taken to a local emergency room where steri-strips were used to close the small laceration.

The EE was closing coupling covers on a dragline swingcase. He bent down to close one when he became dizzy and fell from the case, bumping his head and separating his right shoulder. The ER doctor determined his dizziness was from sinusitis. CT scans were negative. He had surgery to repair his right shoulder. *A 5% whole person impairment rating was issued.

Hot water wash #1 was locked out by cross cleaning solution EE, #2 hot wash unit was being utilized by Benchmark EE's to wash equipment. Benchmark EE's took a 15 min break. When they returned, one of the Benchmark EE's turned on switch for unit #2, a propane gas ignition occurred blowing the four 3""x4' doors off the cabinet of #2 and displacing bent stack.

Employee fell in hole trying to relocate discharge line on a pump straining medial collateral ligament of right knee. Physician did not issue any restrictions until 10-13-2009. Restricted to no climbing or descending equipment. Cannot operate heavy equipment.

Employee stated that he stepped into a coal crevice created by a ripper dozer. He further stated that he was stuck at about waist deep and was unable to free himself without help for more than 30 minutes.

Employee was assisting with removing old teeth on the shovel bucket. When a fellow employee tapped on one of the teeth with a sledge hammer, a sliver of metal came off the tooth and struck employee in the left forearm requiring stitches.

On 8-25-09 employee exerted extreme force on a long handle wrench to remove a filter. Employee felt slight pain in shoulder not reported until 8-26-09. On 9/09/2009 at 6:30 am employee told supervisor his shoulder still had a slight pain. Physician did MRI and no restrictions on 09/10/2009. Second Physician visit recommended physical therapy that started on 10/06/2009.

Employee was changing tires using a tire machine. He had changed several tires and developed pain and swelling in his right knee.

Employee was handling suction line at the barge pump when the suction line float slipped out of his hands and landed just behind the steel-toe of his left boot causing a fractured big toe.

Employee dismounted his dozer and was walking to the portable toilet when he slipped on the muddy conditions and fell. He caught his left wrist in trying to break the fall and sprained his left wrist. **Follow-up Dr. appt. on 3-23-09 placed employee back on restricted duty.

Employee was changing the teeth on the backhoe bucket and struck the keeper pin with an 8 lb. sledge hammer. A sliver of metal flew off and struck the employee just above the left knee, requiring removal of the imbedded metal and 4 stitches.

Employee was installing a race on a dragline boom point sheave. As the race was being tapped in place by a co-worker, injured placed his fingers under the sheave to check the alignment and the race dropped in place thus pinching employee's fingers.

When removing existing power poles, sandy soil along with poor placement of crane caused rear outrigger to giveway when pole was removed. When the outrigger pushed into the pole hole the crane became unlevel & overturned. The operator was attempting to get clear of the overturning crane when he fell from the crane hitting ground causing sprains to neck/back/shoulder

Employee was walking to a dragline junction box and stumbled on the loose ground and fell, striking his left ring finger into the ground. Resulting in a fracture to the middle phalanx and joint.

Employee stated that he was laying on his back installing cap bolts on dragline boom when he felt a burning sensation. Dr. diagnosis was an umbilical hernia and assigned employee to restricted duty.

EE was working on a broken dump door under a railroad coal car. The EE cut the chain holding the broken door up to begin repairs and the door swung down, stricking the EE in the chest. The EE received multiple broken ribs and his right lung collapsed. This injury has resulted in lost time, beginning Friday, 2/29/08.

EE's were changing out crossties on the rail. EE set spike on a new tie and as he struck it with the spike maul to drive it in, the spike popped up out of the tie and struck the EE in the mouth chipping a tooth.

Employee was removing the drag motor coupling cover on D-32 Dragline when the cover fell over mashing his right little finger between the cover and the motor support gusset requiring 3 stitches to close the wound.

Employee was removing a plastic tie-wrap from a bad order tag, when his personal knife slipped and he cut himself on the underside of his left forearm, causing a laceration that required 9 stitches.

EE was using a rail pick to clean out ballast from between ties under the rail to remove the tie plate. While doing so the ee accidently hit the rail with pick causing it to chip and a piece of the pick entered the ee's left cheek just under the skin.

Employee was working off a scaffold. He reached down to obtain a welding machine rheostat, lost his balance and fell approximately 8 feet. He received a small non-sutured cut above his left eye, bruised shoulder and sore back. **Per call from Lonnie Green: add comment that fall aggravated EE's prior kidney transplant.

Employee was in the process of driving t-posts with a t-post driver. When he picked up the driver over the top of the post, he picked it up to far and missed the post with the driver, causing his finger to become caught between the handle and the top of the post. This caused a laceration requiring 5 sutures

Employee was removing a water pump discharge line in the A-4 pit, when the line flew back and struck him on the chin, resulting in a laceration the required several stitches to close. No restrictive duty was issued by the emergency room doctor.

Employee felt pain in his right wrist while dismounting a coal haul truck at the AII Area crusher. Doctor diagnosed tendonitis in his right wrist.

While tightening the nut on a pitman arm with a wrench, the EE strained his right shoulder. The MRI done on 5/1 revealed inflamation in the shoulder area. It was decided that physical therapy may relieve this condition. The EE began therapy 5/10 which makes this injury recordable.

Employee was disconnecting air hoses and when he disconnected them the pressure in the hose whipped around and hit left hand index finger. Left index finger was lacerated requiring five stitches.

Employee was descending from a hauler. He lost his grip with his left hand and all of his weight shifted to his right hand. This caused him to swing away from the ladder with all of his weight on his right arm and something pulled in his right arm and it started to swell. He was taken to the hospital and his first Restricted Duty Day was 11/30/06.

Employee was retrieving a dragline bucket tooth pin from inside of D32 dragline bucket. He slipped on the teflon coated bucket liner and fell backward. He tried to catch himself with his extended right hand. He jammed his right shoulder. On 11/28/06 the doctor approved him for physical therapy. No restrictions were issued. He still has some soreness in shoulder.

Employee was picking up a 5 gallon pail of oil when it slipped out of his hands and the edge of the pail landed on top his right foot just behind where the steel toe stops. This caused him to break the 4th and 5th toe on his right foot. Bones were not misplaced so he was put in a walking shoe while the injured toes healed. He was allowed to return to work.

A supervisor walked out of the building and saw in the distance what he initially thought was smoke or a mist which was close to ground level. It was several hundred feet outback of the building. Upon closer investigation, it was determined to be gas spewing up through the ground.

A train was assigned to pick up some hot lignite out of A1 silo at the Beckville mine. When train operator began to fill the first car the lignite dust flashed and then went out. The operator continued to load until he had emptied the silo (five cars). The hot lignite was taken to the power plant where it was used.

The employee was repairing a drag chain on D31 Dragline and as he was positioning the drag repair link, he moved it with a bar and also by hand. As he was rising from a kneeling position, he realized that he had strained his back. On 10/20/06, I was informed that W/C has approved him for Physical Therapy.

The employee was wrapping hydraulic hose inside of a portable coal screener. He slipped and tried to break his fall with his extended left hand. He heard a pop, and doctors determined that he had fractured his 5th metacarpal bone on his left hand.

Employee was in process of cutting a rope with a pocket knife with the sharp side of blade toward his leg. When the knife cut completely through the rope it inadvertly cut his leg below his knee causing a cut that required immediate attention.

Around 2:30pm, an environmental specialist noticed smoke in a remote location and went to the area to investigate. Upon arrival, he noticed that there was a fire in a cut over timber area. No one was working in this area. About fifty acres of cut over timber burned.

The employee was using a come-a-long to install an upper fairlead sheave on D31 Dragline. As he was pulling the come-a-long, the pin traveled farther than it normally would and the employee hit his right elbow on the bushing between the elaborate seal & the sheave. On 8/14/06 the doctor diagnosed it as Bursitis.

Employee was changing out a feeder crank disc and while he was using a wrench to break the bolts loose, he felt pain in his left shoulder. An MRI was taken on 7/20/06 and he was told the results on 7/28/06. He was told that the MRI showed some Bursitis in his shoulder and he is now scheduled for Physical Therapy.

Employee was descending from the drag rope platform on D32 Dragline. His foot slipped off the step on # 4 drag bull gear pinion. This caused the employee to fall and skin the shins on both his right and left leg on the step that is welded to # 4 drag drum access route.

Over-exertion in lifting objects

Mechanic was using a 1 1/2"" impact wrench and a 4 5/8"" socket to remove 3"" rod bolts from a hoist case on D33 Dragline. As he lifted the impact up a little over head high, he felt some pain in his upper back and it was sore the next day. Chiropractic treatment started on 5/19/2006

EE was operating a motor grader in a pit which had not been cleaned. He stated pit was rough so he was assigned to blade it before hauling equip arrived. EE called supervisor 4/25/06 that his back hurt and arm numb. Drs. had difficult time determining pre-exist or new. 8/30/06 independent medical examiner diagnosed it as an aggravation of old injury with some new damage.

Employee's foot slipped while dismounting from a wet dozer track. He fell onto the concrete floor. His first doctor's visit was on 3/30/06, bruised right arm & shoulder. On his follow up visit (4/14/06), his condition was diagnosed as Tendonitis in his right arm.

Flash burns (welding)

Two mechanics were welding on a backhoe bucket with a shield between them. However, during this process, one of them received flash burns to both of his eyes. The mechanic was examined by a doctor (1/19/06) and received a prescription for antibiotics. No restrictions were issued.

EE has been diagnosed with Carpal Tunnel Syndrome in both wrists due to operating equipment over the years. He is a hauler operator. His medical statement was dated 1/11/06; we were not notified of the diagnosis on 1/17/06 when a statement from the doctor was faxed to us. EE had surgery on left wrist on 8/4/06. Missed work days & was on light duty. Released full duty 9/5.

Contact with electrical current

AN ELECTRICAIN RECEIVED AN ELECTRICAL SHOCK WHILE WORKING IN CLOSE PROXIMITY TO ENERGIZED FLOOR MOUNTED LIGHTENING ARRESTORS WHICH WERE LOCATED ON D33 MARION DRAGLINE.

EMPLOYEE WAS HOOKING UP A DISCHARGE LINE ON A BARGE PUMP. HE HAD MUD ON HIS BOOTS. HE SLIPPED AS HE WAS STEPPING OFF OF THE PUMP BASE AND FELL ONTO HIS BACK AND BOTH ELBOWS. THE INCIDENT OCCURRED ON 7/11/05 BUT IT WAS NOT REPORTED UNTIL THE EVENING OF 7/26/05. AT THAT TIME THE EMPLOYEE WAS IN ER. HE WAS DISCHARGED FROM HOSPITAL ON 7/29/05 & RTW ON 8/1/05 MISSING 3 DAYS.

EE WAS CARRYING 5 GALLON BUCKETS OF OIL TO TOP OF THE SILO. HE STARTED HAVING PAIN IN RIGHT KNEE WHILE PERFORMING THIS TASK. FIRST DOCTOR VISIT WAS 7/18/05. AN MRI WAS TAKEN OF 7/28/05. EE WAS INSTALLING A SHOE RETURN CABLE ON DRAGLINE. THE CABLE HAD BEEN THREADED THROUGH SHEAVE & A STRAP WAS ATTACHED TO A LOADER T0 STRETCH THE CABLE BEFORE TIGHTENING THE CLAMPS. THE CLAMP WAS ON THE END OF THE CABLE & IT SLIPPED. A BOLT FROM THE CLAMP HIT THE EE IN THE MOUTH. IT FRACTURED ONE TOOTH & ANOTHER WILL NEED A CROWN. ONE OF THE TEETH HAD TO BE EXTRACTED.
