EE was using a pry bar on a reaction arm, the reaction arm came loose and pinched EE's finger between the pry bar and mid-rollers on the roller frame. This caused an open fracture of the distal phalanx of right ring finger.

Employee was pushing john boat off the bank when EE lost footing and dove into the boat so EE wouldn't fall in the water. EE struck left shin on the bow of the boat, requiring stitches.

Employee was helping to tighten a bolt on a loader when the adapter broke. The employee slipped and fell to the ground twisting his knee. The employee reported it to the office. After the Thanksgiving holidays he came back to work and said he needed to go to the doctor about his knee. He was taken to the doctor and they recommended he have physical therapy.

Employee was by log washer and was going to wash the radial rock stacker tail pulley, crossed over the 10"" pipe and he planted his left foot and his knee gave out and he sprained his left knee.

Employee was operating tail winch on D6D Dozer, operator engaged winch & got out of cab in order to guide winch cable into the correct position onto the winch drum. He did this by using his left foot as a guide while operating the winch with his right hand. This caused his foot to become caught in the winch resulting in a cracked bone in his left foot.

ON MONDAY, MARCH 18, 2002 WE WERE NOTIFIED THAT EE WAS NOT COMING IN BECAUSE HE HAD HAD SURGERY OVER THE WEEKEND. HE HAD TOLD A THIRD PARTY THAT A CABLE HAD HIT HIS HAND AT WORK ON WEDNESDAY. HE DID NOT REPORT THIS OR ANY INJURY TO US. WE ASKED THE EMPLOYEES WORKING IN THE AREA WITH HIM AND NO ONE HAD ANY KNOWLEDGE ABOUT THE INCIDENT.
