Rock became lodged in lift arms of loader. Employee attempted to remove rock when it shifted and smashed EE's finger

Employee was walking back from river pump on road, stumbled and caught themselves resulting in them placing all their weight on left leg

Tech got into the back of the service truck to retrieve a hose. While stepping out of the truck, techs foot caught the tailgate of the truck bed and began to fall backwards. Techs hand slipped off the safety handle attached to the service bed and EE continued to fall backwards hitting the ground.

employee was standing between the front end loader access ladder and the front wheel fender checking for oil leaks. Another employee started the loader and the front end loader articulated, pinning the first employee between the ladder and the wheel fender

While working at the water pump area, employee received an insect bite.

Employee stepped out of the manlift basket when EE's right foot rolled under EE causing the ankle to twist.

Starting a water pump when snake surprised the employee causing EE to loose EE's footing hitting EE's right wrist on a nearby gas can.

While operating a dozer employee was sting multiple times by bees

Lifting oxygen/acetylene bottle (gas cylinder) from rack to the truck and strained their back

While chipping a weld, a piece of slag ricocheted off employee's forehead went behind their safety glasses and struck their left eye.

Employee was placing chain back into toolbox when toolbox lid fell cutting employee on head

Employee was working on the primary, ee foot was over the kick plate on man lift, other employee lifted the basket pinning foot between kick plate and belt

Working on primary crusher, removing belt clamp, belt clamp still under pressure, when the clamp came loose it pulled back knocking the ratchet from employee's right hand, wedging hand between belt clamp and support bar.

Rock hung in pre-crusher, employee putting a chain around stuck rock, another rock shifted, rolling into employee back

EMPLOYEE WAS REPLACING A BELT AT THE CRUSHER AND STRAINED HIS LOWER BACK.

EMPLOYEE WAS GREASING HIS FRONT END LOADER WHEN HE LOST HIS BALANCE AND FELL BACKWARDS HITTING THE LOADER BUCKET. HE RECEIVED A LACERATION NEAR THE ANAL ENTRY AREA REQUIRING STITCHES TO CLOSE THE WOUND.

EMPLOYEE WAS REPLACING A RETAINING BOLT ON A HAUL UNIT THAT HOLDS THE CAB TO THE FRAME. IN THE PROCESS, HE FRACTURED HIS LEFT INDEX FINGER.

EMPLOYEE WAS REMOVING THE CARRIER BEARING/PARKING BRAKE ASSEMBLY FROM UNIT 1580 WHEN IT FELL ON HIS RIGHT ANKLE.

EMPLOYEE CUT HIS LEFT LEG ON A JAGGED PIECE OF METAL WHILE WORKING ON THE SCALPER SCREEN. REPORTABLE DUE TO THE 4 STITCHES TAKEN TO CLOSE THE WOUND.

Employee hit his left elbow climbing out of a hopper. He requested to see a doctor on 01/26/2010, with condition of a contusion and returned to work. The condition did not improve, on 07/09/2010 surgery was completed with employee missing 8 days from work. He returned to work on 07/22/2010.

Employee slipped on loose gravel and injured his left knee. Employee continued to work until 09/28/2008 when he was scheduled. He is now on motion restriction. Medical report was not found until 10/06/2008.

While getting on the Cat Truck, his left knee popped, causing knee sprain.

This is an alleged accident. Loader operator stated that he injured his left wrist. He does NOT know the date of injury and he does NOT know the time of injury. He also does NOT know where the accident took place.

Employee was working on the lights on R40, slipped and hit left foot on U-joint.

Employee smashed right ring finger while replacing bering housing at the crusher.
