Rock became lodged in lift arms of loader. Employee attempted to remove rock when it shifted and smashed EE's finger

Fall from machine

Tech got into the back of the service truck to retrieve a hose. While stepping out of the truck, techs foot caught the tailgate of the truck bed and began to fall backwards. Techs hand slipped off the safety handle attached to the service bed and EE continued to fall backwards hitting the ground.

employee was standing between the front end loader access ladder and the front wheel fender checking for oil leaks. Another employee started the loader and the front end loader articulated, pinning the first employee between the ladder and the wheel fender

EMPLOYEE WAS GREASING HIS FRONT END LOADER WHEN HE LOST HIS BALANCE AND FELL BACKWARDS HITTING THE LOADER BUCKET. HE RECEIVED A LACERATION NEAR THE ANAL ENTRY AREA REQUIRING STITCHES TO CLOSE THE WOUND.

Employee hit his left elbow climbing out of a hopper. He requested to see a doctor on 01/26/2010, with condition of a contusion and returned to work. The condition did not improve, on 07/09/2010 surgery was completed with employee missing 8 days from work. He returned to work on 07/22/2010.
