EE was trying to remove an impact bed when it got hung up on the conveyor frame. While using a rock bar to gain leverage and free the impact bed, the rock bar slipped and struck the employee on the right shoulder causing a contusion.

Employee was washing out underneath truck scale when experienced a rash on lower arms. It was not determined to be work related until 9-2-16.

Employee was installing a bucket pin on a loader when the arm that the pin goes into slipped, catching employee's left middle finger tip. The finger tip was amputated.

EMPLOYEE WAS TROUBLE SHOOTING IN AN ELECTRICAL PANEL-BOX WITH A VOLT/AMP METER PROBE. THE PROBEMAY HAVE COME IN CONTACT WITH A HOT WIRE AND GROUND WIRE AT THE SAME TIME, CAUSING SPARKS AND FLASHES.
