EE was on 3 person team changing V-Belts at Impact Crusher. Hand rolled under pulleys while pulling belt. Right thumb fractured from pinch point.

Over-exertion in lifting objects

Employee worked on multiple jobs this day. One of them involved replacing a gearbox on a screw conveyor. Employee noticed EE's back was sore after carrying the gearbox up the screw conveyor.

Employee was changing brake pads on a haul truck. As the employee lifted the caliper up EE attempted to slide a jackstand under it. EE's hand slipped off of the caliper and it dropped catching right ring finger between the jackstand and caliper. Employee suffered a broken right ring finger.

Employee was stretching back in a chair and felt a pain in their back. Missed one shift due to doctor restrictions.

Over-exertion in lifting objects

Employee bent down to pick up 50lb weight to calibrate the warehouse scale. They picked the weight up about half way before they felt a sharp pain in groin area and it radiated down the leg. The employee suffered a strained groin and was put on to restricted duty after seeing a doctor.

Employee was coming down the stairs of DCL#5 at the 75 Loadout. As EE stepped down on to the asphalt off the stairs EE's left foot gave out and rolled as EE planted it. The ankle swelled up immediately. The employee sought medical attention the following day and received restrictions to not be on feet for more than 4 hours at a time.

Over-exertion in lifting objects

Employee was stacking finished material bags onto pallets off the conveyor. As EE picked up a bag and turned to place the bag on the pallet, EE felt a pop and sharp pain in EE's right chest/ribs. EE immediately fell to the floor and called for help. The doctor diagnosed EE with a muscle strain in EE's right thorax wall in the ribs.

Struck by flying object

EE was removing a hydraulic line on the crusher rock breaker. When the fitting was on the last thread it whipped and struck the employee on the left calf causing a laceration that required two stitches.

EE was walking from the breakroom to the West warehouse. When EE walked across an area that had been leveled with 2"" rock EE lost balance. When EE fell and caught self with hands and suffered a contusion to left palm.

An employee was shoveling material around the outer wall of the 70x200 building, when the employee felt a pull in the middle of lower back. The employee strained their back.

Employee was lowering a ladder at loadout and felt a pulled stomach muscle; medical treatment was declined by employee. On 12/26/17 the employee had non-work related surgery, when the surgeon noted and repaired an unknown hernia. On 2/2/18 the employee notified management the hernia repair was a work-related injury. The injury is under investigation to determine work-relatedness.

EE was cleaning grease on the floor of Ball Mill 1, slipped and struck guard

EE was connecting a portable air compressor to pickup truck trailer. EE pulled and lifted on the trailer tongue and the trailer rolled forward, the trailer tongue stuck the EEs hand that was positioned on the pickup truck bumper.

EE was prepping railcar for loading. EE strained back when opening railcar hatchlid. EE declined medical treatment at the time of the incident, however, sought professional medical treatemnt on 8/24/2016 and was assigned restricted work activities.

EE was cleaning off # 3 belt at the primary crusher when EE noted left leg began to cramp. EE sought professional medical attention on 6/19/2016. EE was assigned restricted duty and prescribed meds.

Fall down stairs

EE had sample bags in left hand and right hand was on the handrail while walking down stairs. Right hand slipped off the handrail and EE fell down six or seven stairs.

EE was discarding worn out belt idlers when EE turned and felt a twinge in the lower right side of their back. EE did not report to immediate Supervisor until 5/17/2016

EE states on May 9th was working in the bag house stacking 50lb bags onto a pallet. EE noticed a small pain lower back, yet continued to work and finished the shift. EE reproted severe pain on May 12, 2016 to Supervisor.

EE was replacing bags in the r/mill baghouse. Task began upon start time (7:00 AM). EE states lunch break @ 12-12:30 PM. Completed task appr. 2:15 PM and noticed tightness in mid to upper middle portion of back. EE cleaned up used bags and tools, and clocked out @ 3:30 PM.

EE was cleaning the crusher out and strained left shoulder.

Contract EE was standing near a moving conveyor belt. EE placed right hand on the moving belt. The conveyor caught the EEs glove and pulled right index and middle finger into the 1/4"" space.

Struck by falling object

EE was operating the East Side Crusher. Several rocks had come to rest on the jaw of the Crusher. EE was attempting to remove them by hand when a rock had come loose from the feeder/shaker (crusher was not operating) and struck his right index finger.

EE was digging in the hard rock portion of the muck pile when the CAT 988F loader teeth slipped out of muck pile jolting machine and the EE.

EE was assigned the task of removing tool boxes off of old Maintenance truck that is being taken out of service. EE was using large timber (appr.4' portion of railroad tie) to slide under the tool box (before he began to cut it off of the bed) and it slipped out his hands pinning the tip of his left ring finger on the concrete floor of shop.

EE was assisting supervisor replacing a diesel fuel nozzle (@ the West Shop diesel tank). Upon disconnection of the hose from the nozzle, residual diesel came out of hose and sprayed onto the EEs hardhat, which dripped into the EEs right eye. EEs eye was flushed by co-worker. EE sought medical treatment and given a prescription for eye meds to treat afterwards.

On 03/06/2015, EE was trying to maneuver around belt skirting and had to twist to access 3/8"" bolts underneath the skirting. After completing task, EE noted a 'twinge' in the lower right side of his back. EE did not request medical. On 3/23/2015 EE reported to work and requested medical treatment. Medical issued prescription for pain.

Struck against stationary object

EE was climbing 8X12 access ladder (fixed) when he struck his right knee cap on a rung. EE was offered medical at the time incident was reported and refused. Appr. 9pm that evening EE sought professional medical treatment. EE was released with no restrictions and returned to work on the next scheduled work day. On 02/17/2015 EE had Rx filled.

Employee was pushing on pry bar at dyer discharge screw, when the pry bar slipped and caught employee's right thumb between pry bar and metal. Employee received 14 stitches.

Fall down stairs

Trucking company would not release personal information. A Non employee / contractor fell when descending from stairs at scalehouse. Injuries that occurred from fall were ankle sprain, Hip / thigh strain, Leg / knee sprain.

Employee was changing Oil Drum 50 gal. Drum slipped and caught employees right hand between drum and forklift. Cuts to index, middle and ring fingers.

Fall from machine

At around 7:00pm control room operator was replacing them belt on a screw. She was positioned on top of the cover for another screw. While opening the guard she lost her footing. When she made impact with the surface below approximately 2 feet she struck her right knee on an old pipe that is no longer in service. Currently at home waiting to return to the dr. for an MRI.

Employee was pulling O2/Acetlyne hose from his truck to work destination (approx. 30ft) and felt a sharp burning pain in his left bicep.

Employee was descending from rear of VOLVO L350F Front End Loader and slipped and fell to the ground landing on his feet causing trauma to his upper back.

EE was clearing out a clogged pipe using a 4ft piece of all-thread on an electric drill. EE made a bend to the end of the all-thread using a cutting torch. As EE finished heating and bending the end of the all-thread a small fire was started as a result of the cutting torch. EE suffered 2nd degree burns to the upper right arm and neck area.

Team member was standing on a water line/pipe approx. 8""-10"" off ground assisting in replacing a bearing housing on a shaft. As he was maneuvering the bearing housing to align the bolt holes his feet slipped off the pipe and fell striking his left foot/heel on the water pipe.

Maintenance repairs/replacing hammers on crusher. A 16 lb. sledge hammer was being used to hammer out a broken hammer bolt. EE was taking a measurement to see how much more hammering was required. Team member assisting in repairs did not notice EE's hand in the way as he continued to hammer on the broken bolt striking EE's left pinky with the sledge hammer.

Employee was cleaning the dust buildup on the rotary dryer with air hose to prepare it for maintenance repairs and came into contact with the outer surface of the dryer drum with his left arm which was still at a very hot temperature level.

Fall from machine

Employee was in the process of conducting a pre-shift inspection on a 988F front-end loader. He stepped onto the left rear tire from the ladder to access the rear engine compartment. As he was stepping across onto the tire his right foot slipped off of the tire and fell to ground landing on his right hand.

Employee was dismounting from the RB V20 Excavator (long hoe) after completing maintenance repairs. The employee stepped onto the lower step of the equipment and his foot slipped due to the mud build up on the equipment step. The slip caused the employee to fall and land awkwardly, injuring his right leg (knee).

Ballmill door was to be opened for repairs. the door is sealed so that material does not excape. Wedge are driven into the door flange to break the seal and then a come-along is used to lift the door to an open position. EE croched next to the wedge to remove it and it dislodged striking ee on the knee. No equipment damamge.

EE welding on top of storage tank when ignition occurred burning his left arm. storage tank integrity may be in question.

Fumes in 20,000 gal. #4 oil storage tank ignited when a welding spark reached the fumes. Resulting flash explosion knocked employee from on top of tank.

EE WAS CLEARING THE WALKWAYS AROUND THE CRUSHER PRIOR TO STARTING CRUSHER. HE WAS TOSSING LARGE PIECES OF ROCK OFF THE WALKWAY AND CAUGHT THUMB BETWEEN THE ROCK AND THE HAND RAIL.
