Caught in, under or between a moving and a stationary object

Employee was installing v-belts on the head pulley at a feed hopper. Right hand ring finger tip was fractured when the finger tip was caught between the v-belt and pulley. Employee was provided immediate medical attention, and returned back to work next scheduled shift.

EE was attempting to remove screen covers to screw located below screen to remove broken flights when screen got caught on EE's shirt, causing the screen to twist, catching the EE's left ear lobe with jagged edge of screen cloth causing a laceration that required 5 sutures to close.

EE was splicing a belt and was pushing the belt over with a piece of wood to line it up. While EE was pushing, EE was using the handrail as leverage. As EE was doing this, the handrail broke causing EE to land on the metal catwalk EE was standing on twisting knee.

Employee was hammering down poly deck screens. EE struck the middle finger of the hand holding down the material, fracturing the tip.

Employee was cutting the top off a tube of Loctite with a pocket knife. The knife cut the top of the thumb that was holding the tube, requiring stitches.

Contact with electrical current

A crane operator dragging belt material to make a patch raised the boom to keep the belt out of the mud. In so doing EE contacted a power line causing a ground employee to be shocked. EE lowered the boom and broke the connection. The ground employee was able to walk away.

Employee was walking between hopper and skid belt when the material under his feet gave way and he fell onto his right shoulder. Employee sought medical treatment the same day and the initial diagnosis was a bruised arm/shoulder. The shoulder did not get better and on 5/13 the employee was re-examined and placed on restricted duty.

Tightening a clamp using a wrench when the bolt sheared off and smashing finger between wrench & clamp.

Juan was hitting a bearing with a hammer and a small sliver of steel was embedded into his left wrist.

EE was unhooking the engine from the rail cars. He could not reach the air valve on the engine from the side he was on so he crawled up on the railcar hitch so he could get to the valve. When he did, he slipped. He used his right arm to catch himself. That is when he hurt his wrist.

Fall from machine

Employee was dumping a bucket of dirt & grease off the back of the dragline. He slipped on some grease and fell off the back of the dragline. He fell about eight feet and landed on his shoulder. He sprained his left shoulder and back.

Fall from scaffolds, walkways, platforms

Employee was installing a chute 10' off the ground. He was tied off to the wire rope sling attached to the piece he was installing. The wire rope slipped dropping the chute and pulling the employee down with it. The employee suffered a ""transverse process"" fracture of the vertebra and will possibly miss a week of work.

EE was changing return rollers on the field belts. He strained the muscles in his back resulting in a back injury.

Employee was tightening bolts on the east crusher with right foot on crusher spring base about 16"" off of crusher floor, left leg lifted off of floor at about same height as right foot, right foot slipped off of spring base scraping shin on spring base.

Employee was unhooking anchor pipe and ties off of the cherrypicker boom. When the operator lowered a pipe to the ground, the hook came loose and the pipe fell down on another piece of pipe lying on the ground. This caused the pipe to bounce off and strike the employee on his lower right leg, causing a sprain.

Employee was standing on a small scaffold, cutting on a dragline bucket. He attemped to get down by stepping on a stool. He stepped on the edge of the stool, his foot slipped and he fell to the floor, striking the back of his head. He received eight stitches in a laceration on the back of his head and has some muscle strain.

Employee cut his right knee when walking past some equipment with a piece of metal sticking out of it. His knee hit the metal and the laceration required eight stitches. He is unable to squat or kneel.

Caught in, under or between a moving and a stationary object

Employee was carrying a board up some steps. He attempted to throw the board to the walkway but it wouldn't reach. The employee attempted to catch the board as it was sliding down the steps when his finger was pinched between the board and a step. He received a laceration on his right index finger requiring four stitches.

EE WAS STARTING SWITCH AT HEAD SECTION. AS HE STEPPED ON A PIECE OF ANGLE IRON THE ANGLE IRON BROKE CAUSING EE TO FALL, TWISTING HIS KNEE.
