On Wednesday 5/14/2025, fixed maintenance employees were working on reinforcing a stacking conveyor, when the stacker failed. This resulted in a conveyor beam striking an employee on the right leg. Diagnosed with right leg contusion/abrasion, was prescribed medication and placed on restricted duty.

The 167 KVA 7200V-480/240V transformer failed due to age. The transformer had been in service for over 30 years and there was no evidence of a fault external to the transformer itself. The fire was contained within the transformer containment. EMS was notified but had to wait until the power company could turn off power to extinguish the fire, causing it to go over 30 minutes.

An employee was pulling a rope to start an air compressor to get grease for the loader. The rope pulled out very fast, allowing EE's left hand to slip and strike a sign behind EE. The employee had gloves on, but the hand struck the sign on the side of the glove under the protective rubber. This resulted in a fractured metacarpal.

Employee became over heated while working outdoors and was taken to the hospital where EE received 1 bag of intravenous fluid and Tylenol for headache.

Dredge Operator accidentally slammed operators room metal door on EE's right thumb. Fracture to R. Distal phalanx.
Injured was struck by 4"" dia. x 4'long pipe as it was being suspended with crane. Chain rigging slid off pipe about 6' from ground. Pipe hit structure before striking Injured on Left foot. Injured was wearing steel toe boots. Need to be on crutches for two weeks for minor bone fracture to top of foot.
Injured ee removed screen guard on Dewater conveyor stacker to examine roller while conveyor was turning. As ee leaned in to get closer look adjacent top roller caught a tool in ee hand and ran forearm through roller. No fractured bones, no open wound, injured suffered contusion to right forearm.

Employee was greasing bearings with pneumatic grease gun when ee accidently shot chest with gun while putting gun away. Abrasion to chest wall from impact. Employee was released back to work with no loss time. Additional visits were later needed due to slow healing of wound. Employee is diabetic and restrictions are keep area clean and dry.

Injured jumped from one level to another while on work boat. Injured slipped twisting his left ankle. Injured loss no time from work and is working normal hours but in light duty area.
Injured employee was doing repairs on holes in vibrating screen deck. When Injured reached for torch, he stepped back into hole of screen causing laceration to Right lower leg. Injured was taken to doctor and given stitches to close up wound around calf. Injured was told to go home for two days and return back to work for light duty.

Injured was helping change conveyor through rollers and cut his left hand on sharp edge of roller as roller slipped from his grasp. Injured was wearing gloves and still cut palm of hand. He needed stitches to close laceration. He lost no time.
While hammering holes in conveyor belt, IE struck his left pinky finger on tip, with hammer. Tip of finger need stitches to close wound. IE went back to work without any loss time or loss wages and is doing his regular work duties of monitoring his plant.

Rather than getting a nearby ladder, Injured Employee (IE) was using a 55 gallon drum to stand on to reach a chain on cherry picker. IE fell off drum when trying to get down. IE struck his lower left side rib on edge of drum suffering mild rib fracture.
