Contact with hot objects or substances

Employee was working on a 300# steam line to remove a skillet blind from the flange when residual condensate sprayed out onto the employees left mid section.

Absorption of radiations, caustics, toxic and noxious substances

Employee was climbing out of sump, when the top of hard hat made contact on top of sump. This resulted in the hard hat contacting the chemical goggles, repositioning them and exposing eyes as the hard hat fell into a puddle of caustic. When the hard hat fell into the caustic, it caused the caustic to splash up and into the employee's left eye.

On 7/12/2016 at approximately 11:15 am, EE (Area Lead) was working the recirculating valve on #3 Deep Bed Washer when the wheel wrench ee was using, slipped off of the valve and smashed right ring finger between the wrench and an adjacent flange. EE immediately went to Medical where ee was treated and released.

Employee was hosing down inside 10 settler, for approximately 40-45 minutes with 1"" hose down hose, and had PPE on. Employee called to have hose shut off, walked to the man-way. Employee suffered from dehydration from lack of fluid intake.

Employee was walking and tripped on grating. Sustained a fractured left patella. Employee required surgery to repair injured body part.

EE knocked a drain valve off piping at 2 SOF pump, liquor contacted lower leg area.

Employee was moving valves in order to retrieve the required valve that he was moving rotated over and caught his right pinky finger between the two valves.

Employee dropped bottom of Precipitation Tank that was plugged, hitting the barrel with a sledge hammer and when plug broke loose, liquor and hydrate engulfed the employee.

EE reported he got sprayed in the right eye when portable pump 4"" hose ruptured. EE reported that he took his goggles off to clean them when he got sprayed in the eye. EE washed eyes for about 20 minutes at safety shower then was taken to medical for treatment. EE received corneal abrasion requiring medical attention.

EE was overhauling 3-1 sweetner transfer pump. EE pulled bowl off of pump using carry deck. As the bowl was being placed on the ground in a vertical position the liner fell out striking EE on right foot. EE rec'd a fractured right toe.

Employee working at #8 Combine floor. Condensate hose came loose and sprayed him on the left shoulder and arm, causing a 2nd degree burn.

Employee was checking 15 and 16 precipitator, after checking 15 and 16 precipitator employee crossed over pipes to close 3 fill header, employee tripped over pipes and fell on his back.

EE approached the sump to the north and perched himself onto a fixed concert support base and attempted to use a shovel to clean-off the grating. While cleaning off the grating, the EE repositioned himself which caused his right foot to slip off of the base and submerge 3-feet into the hot slurry. The hot slurry overflowed into the EE's boot immediately.

Flash burns (welding)

Employee was setting up the welding leads and was checking to make sure ground was hooked properly. He struck an arc at when injured employee walked up with his hood up and received welders flash.

EE was clearing restriction in PO 1-3 #1 GSC when hot alumina sprayed back causing 2nd. degree burns to the employees' face, nose, mouth, arm and shoulder. Employee was wearing required PPE including gloves, FR clothing and goggles. Preliminary investigation suggests that the employee was not wearing a faceshield at the time of the incident.

Employee slipped in unit 26 as he broke is fall his goggles came off causing caustic to get into Eyes. Right Eye was ok and required no treatment. Left Eye required prescription medication.

Employee was running in lukie on 3-2 DFP lukie with a nut runner while squatting. He lost his balance and fell forward, hitting his head on 90 degree pipe.

An employee was using sledge hammer to clear a plug on the bottom of #61 PPT. Employee was working off a stand, and lost control of the sledge hammer, striking injured employees' on top of his left foot with the sledge hammer.

Employee was working cleaning chutes at top side 24 in Rod mills. Employee started feeling like he was having an asthma attack. Employee went to control room where he lost consciousness. Employee was stabilized and transported to the hospital for further examination.

Setting up drip pan on 2-16 heater. One end of the 6"" PVC drain pipe fell off the jack stand causing the other end to rise pinching EE left thumb against the drip pan. EE was treated at medical with a steri-strip.

Employee was using a wheel wrench to turn the 126 wheel valve when the wrench slipped out of the wheel valve. Employee's left right finger was crushed between the wheel wrench and angle iron (guarding) on the platform.

EE was loading tools and material on to the back of a truck. There was a 4 inch hose and pump running through the lay down and fab area where he was loading truck. His right foot got caught under the hose causing a fracture in his ankle. Employee was taken to doctor then Given a post accident UA in which the ee failed. He was then terminated for violating company policy.

EE fell into a storm drain that was covered in water during a rain storm. Bruised backside.

Employee was using a condensate hose to wash down the pad under #4 hydrate tank. When EE turned the main valve back on the pressure from the condensate caused the ball valve on the other end of the hose to turn on, hitting EE on the chest and spraying hot condensate on EE's arm, chest and head.

Employee was using a hammer wrench to tighten up some structural bolts. While swinging a sledge hammer, he made contact with a nearby overhead beam that caused the hammer to ricochet and hit employee's left index finger causing a fracture.

Employee was hosing down in a washer when hose water splashed back and entered his eye. Employee immediately flushed his eyes and reported to the medical facility for evaluation and treatment. Employee returned to the medical facility the following shift for evaluation and was released to full duty.

Struck by flying object

A hose ruptured spraying hose water into the employee's eyes. The employee's eyes were irrigated and the employee was evaluated the following day at the onsite medical facility and released to full duty.

Employee was riding a bike when contractor walked out right in front of the bike causing the employee on the bike to swerve in order to avoid striking the pedestrian and ended up losing control of the bike and falling on the roadway.

Employee strained his left shoulder while closing a valve. The treating physician placed the employee on restrictions as of 4-15-15; back dating the restrictions to 3-25-15.

PMIC EE left work and after getting home felt irritation on his left calf. EE cleaned up the area and went to sleep. When he woke up, the area was worse, so he went to the ER and had it treated. EE was told he could RTW in 2 days. EE did not report incident until 3/7/2015.

Employee was taking care of a water pump at 3-3 cooling tower basin. Employee noticed a small oil leak and started to look for it and at the same time clean up the oil. He was using several rags and he placed his hand too close to the radiator fan. The radiator fan then sucked up the rags and his hand into the fan causing a laceration between two of his fingers on his right hand.

Employee was descaling a washer when caustic liquor seeped under his goggles and into his eye. The employee flushed his eye with water and reported to the Medical facility for evaluation and treatment. The employee returned to medical the following shift for a follow-up and was released to full duty.

EE was in the process of replacing the hose down hose on 39 press. EE was unscrewing the old hose. The ball valve (two piece valve) separated at the ball retainer ring instead of the piping, allowing the blocked liquor to release and spray EE. The liquor entered from the bottom of the EE goggles.

The EE was walking from the main road to get on #3 WOF tank pad to perform housekeeping duties when he misjudged his step getting over the curb. The EE attempted to step over the curb when his left heel caught the curb causing him to lose his balance. EE reported that his left ankle twisted causing sprained ankle.

Moving Drill Mast from 4-18 heater to 4-11 heater. Employee was trying to help direct the mast between the middle and inboard body of 4-18. EE's left ring finger got caught between the mounting foot of the drill mast and a small flange on top of the 4-18 middle body dome causing a laceration and fracture to the end of said finger.

Employee was attempting to place a compressor on to a vehicle hitch. The compressor's chocks had been removed so that the compressor could be rolled on to the hitch, causing the compressor to roll forward and catch the employee's hand between hitch and compressor. The employee received a fracture on the right hand metacarpal.

Employee slipped off 3rd rung from the bottom of a scaffolding ladder, falling to the ground. Fractured Right ankle.

EE walked out of the elevator and down the stairs to move the hose on the hose tamer 12/31/14. He reached down and moved the hose tamer and when he backed up to see if the hose was spraying in the right direction he stepped on a rock and twisted his (R) ankle. EE did not report incident until 1/2/2015 when ankle 'stayed' swollen. Fractured right ankle.

EE was cleaning the top of #5 Washer when he bumped his head on a metal beam. He was going under the beam and raised his head before he cleared the beam. Neck Strain

Employee was opening suction valve to 5-1 slurry booster with sledge hammer employee felt pain in his right shoulder area. Shoulder strain.

Employee was cleaning under the east cross conveyor and he stooped down to clean and when he stood up he struck the hand rail as he stood back up-lower back strain. Employee did not become reportable until 12/22/14 when work restriction were given.

EE was trying to pull apart two hoses (Chicago style) using a pipe wrench. EE stated that he was holding the hose down on the floor with his left leg/foot and pulling the pipe wrench towards himself when he felt pain in his left groin area. On 12/17/2014: EE was placed on work restrictions.

Employee was operating a nut runner when it slipped out of his hands and hit employee in the left shin. EE reported incident on 12/14/14--incident happened on 12/10/14 no report made on 12/10/14.

EE was in the process of depressurize 5-2 DFP mud blocks when he opened drain valve that spray on face. EE washed off under safety shower. EE states he had his goggles on and that when he was washing off his goggles fell off and wash water entered his eyes. EE received prescription medication for his eyes.

EE fell through 2nd deck to 3rd deck (10ft Fall) inside 4 washer while descaling inside tank. EE was transported to ER. as a per caution. EE was released with restrictions for one day.

Employee was working on a Cytec issue at 5 Diluter. While swinging a hammer (8 lb with a long handle), his right pinky finger got caught between the hammer handle and piping/equipment resulting in a fractured right pinky finger.

He shoveled two scoops into the wheel barrel and on the second one some dust billowed out of the wheel barrel and got into his right eye. He was not wearing his goggles. Employee sustained a corneal abrasion requiring antibiotics for his eye.

EE was walking into Bldg35 north west door, lost footing and fell on right knee. EE was taken to the medical department and diagnosed with a contusion and laceration to right kneecap. EE received sutures.

EE was assisting fellow EE with closing the discharge lukie on 5-8 heater using a nut runner. As the valve was almost closed the nut runner torqued up, the handle swung around and hit EE on the right wrist and right collar bone. AC Joint Separation.

Employee slipped and fell on his back. At initial visit to the clinic, all x-rays were negative and the EE was given a full duty release. During a follow up on 11/11/14, new x-rays showed a fractured left 2nd and 3rd rib. The EE was placed on restrictions.

Employee was about to hose down on the pad at 5 Hytank but when he tried his hose plugged off. He worked the valve to see if it would pass through and it did. EE was struck by a port a power on his shoulder on 9-9-14, taken to the clinic for initial treatment on 9/10/14 and released to full duty. At the EE's 9/18/14 follow up he was referred to an ortho and again released to full duty. EE was seen by an ortho on 9/25/14 where he was diagnosed with a torn rotator cuff and prescribed physical therapy.

While working on 1-2 slurry booster pump, EE grabbed the impeller to rotate the shaft. The sharp leading edge, of the impeller vein, cut through his leather palm glove and resulted in a laceration to the area between the thumb and first finger, of his right hand.

Security officer called the back gate and stated that she was feeling sick. She stated that she was dizzy and light headed. She was on the North E-crane working security detail. Dehydration.

Cleaning inside of 8 washer liquor tank on the 4th deck with a water hose. Caustic mud splashed back on EE while cleaning the tank (all appropriate PPE was employed). EE began cleaning hard hat and face shield while still donning hard hat and shield when he removed goggles to clean face, at this time mud fell into eye.

When EE reached the last set of steps he noticed some piping right in front of the stairway along with a 4"" toe plate right at the bottom of the stairway to the left. When he stepped over them and immediately felt pain on his left leg (calf).

Employee was climbing ladder to reach #2 condensate steam valve to isolate steam. While climbing the employee states he felt a pop but didn't experience hardly any pain. After lunch employee stood up and realized pain in his right knee. Employee reported incident to supervisor who then sent employee to medical dept. for evaluation. Right knee strain

Employee was walking into the plant when he was struck by a scooter on his right side. He was working unrestricted at his regular job (training electricians) since 3/11/14. He had surgery to his right rotator cuff on 5/7/14.

Opening feed Lukie on 3-8 flash tank. EE was opening the feed Lukie on 3-8 flash tank using a ""nut runner"" and two multipliers. EE states that when he actuated the nut runner the scaffold he was standing on shifted slightly. The shift caused the nut runner to move where he had it braced with his leg and allowed it to pull on his right shoulder. Shoulder Strain

Employee was adding a water sump relay tank to control the level. Employee was opening the suction valve of the second pump when the relay tank overflowed. Employee received burns to his back.

Employee was completing his Pre-Operational Inspection on Broderson #51.As he was descending from the deck he lost his balance causing a minor abrasion to his nose and pulled his left groin.

While walking east on the main roadway between Fac. 45 and Fac. 50 a gust of wind blew particles in EE's right eye.

EE had energized a 4160VAC contactor and was walking away from it when an arc flash occurred. EE was transported to a local hospital where he was treated and admitted with 2nd and 3rd degree burns to his right arm and 2nd degree burn to his right ear. EE released from the hospital on 10/11/13 but remains off work.

While reaching to operate the condensate conductivity block valve, on the north end of 4 digester unit, the EE's shirt snagged on a nearby ball valve handle. When the EE backed away, the ball valve opened, resulting in hot condensate spraying him in the abdomen area. Abdomen burns.

Employees were sailing a ship out of 90 dock when the lines got caught between the ship and the pilings. As they got the line on the dock the ships crew member tightened the instead of giving slack line causing the line to hit and flip another employee off his feet. EE was installing a Level Control Probe J-Box at 45-6 Sump Pump. EE was pushing and adjusting J-Box and got his left ring finger caught between hand rail, unistrut and J-box. On 9/26/13, X-ray revealed a fracture of the left ring finger.

He squatted down to verify the meter was the correct meter, started to move the meter with his left hand/arm and lost his grip, slipping and fell backwards. I picked up a 5 gallon can of oil and proceeded to carry it to the upper level of the 6 A/C/ The area is hot and steamy. After I added oil to 6 A/C, I proceeded to come down. While using an 8# sledge hammer to operate valves felt pain in his right elbow. EE stated that the pain got worse when he took a break and rested his arm. EE was closing the suction valve of a pump using an 8 lb sledgehammer. As EE struck valve handle, the valve handle broke, causing the EE to follow thru with his swing. EE experiencing pain in his right shoulder. EE placed on restricted duty on 7/9/2013.

On 05/15/13, EE brought document from physician proclaiming that the hernia was work related.

EE was struck by a scooter as the scooter was backing up to latch onto a welding machine. EE's supervisor put EE on sedentary duty on 02/04/2013.

EE was replacing the valve handle on feed pump. As EE was walking away from the pump he struck his head on an overhead support beam causing him to jam his neck. On 01/22/13, EE was prescribed a muscle relaxant and 2 weeks of physical therapy, making this a reportable injury. No lost work days or restricted duty.

Employee has been de-scaling a line with a rivet buster. He set the rivet buster on a pipe in order to take a break. It slipped off of the pipe and landed on the employee's left hand, fracturing his ring finger.

Employee was cleaning scale from a platform when a section of the expanded metal between the piping and the beam gave way causing the employee's leg to fall through. The section of expanded metal had been previously cut and re-tacked in place. The employee injured his shoulder during this event.

EE was using a 3/4"" air impact with a ""wobble"" joint on a 1 13/16"" socket. EE placed his left hand around the wobble and socket assembly to stabilize it. EE's left index finger at the tip got pinched by the wobble joint. EE was treated at medical and placed on restricted duty. EE RTW full duty on 9/17/12.

While using a rivet gun to conduct clean up under #5 Digester Unit. The 18"" rivet gun bit broke, fell forward and struck the EE on the top of his right foot.
