Over-exertion in pulling or pushing objects

107 & 106 conveyors had overloaded & tripped out. Therefore, choked up the chute that they were working on. The employee used a bar with a hook on it to move the clay around. In the process of getting the system back on line he hurt his right shoulder.

A PIECE OF SHEET METAL GOT CAUGHT UNDER THE 108/102 TRANSFER POINT. HE WAS PULLING THE METAL SHEET OUT AND IT CAUGHT ON THE TRANSFER POINT HIS HAND SLIPPED AND THE SHEET METAL CUT THROUGH HIS GLOVES.

REPLACING A FLEXIBLE CONDUIT ON #3 KILN VARIBLE SPEED DRIVE. EE WAS STANDING ON MOTOR AND EDGE OF TANK BASE. THE BASE HAD OIL AND WAS A BIT ICY. HE WENT TO MOVE AND SLIPPED, HE PUSHED HIMSELF AWAY FROMT HE TANK AND LANDED ONT HE GROUND 3.5 FEET BELOW. HE LANDED ON HIS ANKLE. HE STRETCHED A TENDON IN THE TOP OF HIS FOOT. X RAY WAS NEGATIVE.

EE STEPPED OFF OF BOTTOM STEP ONTO WET GROUND HIS FOOT SLIPPED ON THE WET GROUND & HE GRABBED THE HANDRAIL TO CATCH HIS BALANCE. ON 4/11/00 HEREPORTED HIS INJURY TO ANOTHER EE BUT CONTINUED WORKING WITH LOWER BACK PAIN. HIS FIRST DAY TO MISS WORK WAS 4/21/00
