Struck by flying object

Employee was working on getting anvils out of the anvil ring of a VSI Crusher. EE was using a soft blow hammer to hit the anvils out of the ring when a piece of the harden steel broke off. The chipped piece of steel flew and pierced through EE's leather glove cutting the back of EE's right hand just above the ring finger knuckle.

Fall from machine

EE was climbing the ladder of the shipping loader to go to work. When climbing the shipping loader, EE missed the step and fell backwards. EE landed on the ground on EE's right shoulder and back then hit the upper back portion of EE's head.

The employee was doing a repair on the PC#3 conveyor belt. While doing so, EE cut a V-notch on each end of the splice. While EE was cutting the belt notch with EE's left hand pulling toward self the blade passed the belt and EE cut EE's right middle finger causing a small laceration needing a few stitches.

Struck against a moving object

Employee was operating a front end loader in the Pit and was instructed to re-handle boulders. Employee was trying to dislodge a boulder at a working face when the rear of the loader lifted up and then dropped back down to the pit floor once the rock released, resulting in an injury to the employees back.

Over-exertion in pulling or pushing objects

The EE was removing the drain plug from a 775F with a rachet. The plug was on too tight and would not loosen. The EE felt pain in shoulder as EE tried to break loose the plug.

Caught in, under or between a moving and a stationary object

The EE was working to remove a bearing housing from the Wobbler Feeder, located at the primary. Upon removing the bearing housing from the shaft, the housing slipped, catching the employee's hand on the way down, catching the employee's index finger between the housing and the feeder structure, causing a rupture wound to the finger that required 19 sutures to close.

A large rock fell off a conveyor onto a catwalk then to the ground striking the employee on the lower right leg. Bruise.

As employee was removing sheet metal from a building, a gust of wind caused the metal to strike the employee on his right wrist. The incident resulted in a laceration.

Crew was trying to get engine on drill to start. They pulled driveline to pump box and at same time pulled guard that covers driveline & alternater. For some reason, inj EE reached down to check tension on alt belt at which time engine turned over, catching his finger between belt & pulley.

Employee strained his upper back while climbimg stairs. On 04/18/07 employee's back was still hurting. He went to the doctor and received steroid shots in his back.

EE WAS CLEANING OUT STOPPED UP DRILL PIPE. WHEN HE ABOUT HAD IT UNPLUGGED THE AIR PRESSURE THAT REMAINED BLEW THE REMAINING DIR AND GRAVEL OUT AND SKINNED HIS LEFT HAND.
