Struck by falling object

Screen #4 side entry portal broke at the hinges while two employees were closing the port. The door fell on one employee right hand, resulting in a fracture to the right index finger.

An employee's left hand became lodged between the rotating pulley and the gear box shaft on radial stacker

Employee was changing out a screen at the Wet Station and loss EE's balance and fell causing lacerations to the back of the head. Employee was placed on restrictions and received three staples making this incident an MTI.

EE was working on closing a lid on a VSI Crusher when it became stuck on descent. EE states they placed hand between lid slot and ear for the lid as they were asking the operator to raise stop and raise the lid. The lid released unexpectedly causing the EE's left middle and ring fingertips to be severed.

Employee was making a cut on a 36"" wide piece of conveyor belt when the knife slipped out of the belt and cut his right hand below the thumb

Employee was helping change out jaw dies in the # 1 jaw. They were in the process of taking out the cheek plate when the pad eye that was welded to the plate broke out and struck employee on his right cheek.

Flat bar, employee was attempting to weld, shifted away from him and employee jerked back and hit his left elbow on T-Bar.

Employee was welding in shop, he laid down T-Bar on floor, turned around and tripped over T-Bar and fell onto left elbow.

WORKING ON GRIZZLY BAR #2 JAW, LAID GRIZZLY BAR ON CATWALK & WORKER WENT TO PLACE ANGLE IRON UNDERNEATH THE BAR. AT THAT TIME CHAIN GAVE WAY & HIS FINGER GOT CAUGHT BETWEEN GRIZZLY BAR & IRON.

THE INJURED TURN TO LIFT A FIRE EXTINGUISHER OFF BRACKET WITH RIGHT HAND AND FELT A PULL IN HIS CHEST. HE THEN STARTED COMPLAINING OF CHEST PAINS. HE TOLD SUPERVISOR AND EMS WAS CALLED IMMEDIA TELY CAUSE THEY THOUGHT INJURED PARTY WAS HAVING A HEART ATTACK.

EMPLOYEE WAS UNDER BIN LOADING 1"" MATERIAL. HE WAS HOLDING ON TO I-BEAM AS HE PULLED LEVER TO LOAD TRUCK. THE WEIGHT OF MATERIAL CAUSED TRUCK BED TO LEAN CAUSING HIS HAND TO GET PINNED BETWEEN BED AND I-BEAM. HIS MIDDLE FINGER ON HIS LEFT HAND WAS CUT.
