An employee was fatally injured when EE was struck by a third-party truck driver who failed to stop at a posted, lighted, STOP sign and made a sudden turn off the primary travel way. Local law enforcement arrested the third-party truck driver and charged the driver in connection with the accident.
EE was cutting skirt rubber with a utility knife and slipped cutting EE's right thigh.
Employee was working on a core drill rig when EE began experiencing minor heat stress symptoms. EE received conservative care on site and worked the rest of EE's shift without incident. However, heat stress symptoms persisted after the shift was over and EE was taken to a local hospital, where EE was admitted, given IV fluids, and treated for acute kidney injury.
EE was performing maintenance work with a colleague at the primary plant. Using a hammer, the colleague intended to strike a hammer wrench held by the EE but missed, bouncing off the feeder wall and striking the EE's right ring finger.
EE was performing a maintenance job at the primary part of the plant. The shaft for a conveyor pulley shifted downward and made contact with EE's left thumb.
Team Members were doing railcar repairs. The rail track was locked out with a de-railer & blue flag. The task was started around 9am. When the task was completed the LOTO were to be removed. The TM reached down to unlock the de-railer and was bitten by a copperhead snake on the left hand ring finger. TM was taken to the hospital for treatment. Work gloves worn during this task.
EE just completed handing a poly-screen to EE #2. EE #1 turned on the inclined walkway & placed right foot on the downhill side of the decline. This caused more weight to be transferred to the downhill side of EE's body. EE's right ankle twisted resulting in a sprain to the right ankle. EE was wearing lace up, steel toe boots at time incident. EE was placed on restricted duty.
A fire was discovered burning on a pole behind the sand plant. The plant was shut down and power turned off on the site. The fire was extinguished at approximately 3:30pm. The fire began from dust and windy conditions that happened when a storm blew in.
On 5/31/18, EE reported experiencing pain from previous shift. EE stated while operating a Front End Loader, EE approached a 6-8"" drainage ditch to position for loading a customer truck. As EE drove slowly over the drainage ditch, the right front tire dipped causing the EE to receive a contusion to the coccyx (tailbone). EE was released to restricted duty.
EE was attempting to cut a Tie Strap from the frame of the glass on the wing window (not emergency exit window) of a CAT 992G Front End Loader with a pocket knife. As EE began to cut, EE's thumb touched the lock on the knife, causing the blade to close on hand. EE received 5 stitches and was released to full duty with no restrictions.
While attempting to break a plug on a wheel loader at work, the employee slammed hand between the ratchet wrench and the inside of a steel rim detaching the whole fingernail and causing a laceration to the left pinky finger.
EE was removing a bushing. A chisel was used to create a gap between the cross shaft and bushing. EE pulled the bushing (chisel still wedged in assembly) off the cross shaft. EE tossed the bushing 4'6"" to the side. The chisel dislodged from bushing & projected upward 7'6"" at a 45 degree angle back towards EE. The chisel struck EE in the lip under the right nostril.
EE was driving the 834B rail car mover to the main parking area. At the point the employee crossed under a conveyor, a piece of limestone buildup fell from the conveyor frame above and landed against the front windshield. The windshield broke in one piece and folded in half striking left hand. EE suffered a laceration to the left 5th digit and left forearm.
EE was carrying a basket as she approached the scale house door. She began to enter the scale house by stepping onto the man-made boot wiper, pushed the basket between the scale house wall and her hip, pulled the door open and stepped forward. She lost her balance and fell forward on her right side striking the ground. The EE sustained a dislocated elbow and is on light duty.
EE began to trouble shoot an electrical problem at the plant & after visual inspection determined troubleshooting in MCC was necessary. He then opened the transformer for visual inspection. Using a T5-600V multi-meter to test voltage, the meter exploded causing a flash incident. The transformer is 4160V AC. He received 2nd and 3rd degree burns to both hands and forearms.
The employee was changing oil while standing on a catwalk approximately 10FT above ground, employee turned and foot slipped causing the employee to fall on the catwalk. The employee struck against the electric motor causing a strain to middle back and rib cage. The employee is seeking medical attention.
EE was replacing bad bolts in the SB-920. He was using an air impact to tighten new bolts, while in the process of tightening a bolt an old bolt popped loose hitting him on his right ear. When the bolt hit him on the ear he lost his footing and tried to catch himself with his left arm. He said when he caught himself he twisted his left arm and wrist.
The employee was doing normal housekeeping on an elevated catwalk. While clearing 3""-5"" rock from the catwalk a gust of wind blew some debris (dust/fines) over the top of his safety glasses into his eye. After attempting to flush his eye at a wash station he was taken to the ER for examination. The ER applied numbing eye drops to examine, the eye had been scratched.
Maintenance crew ee and I were attempting to pull the counter weight off the timing side of a Telsmith screen shaker. We had a puller and two come a longs on it as well as heating it. it started coming off as we cranked on the come a long when all of a sudden it broke loose and flew off resulting in smashing his hand between the counter weight and I beam.
The employee was using a pocket knife to trim the excess off of a zip-tie that was securing a grease hose to the framework of a conveyor. He took a step forward and lost his balance, this caused him to fall forward. The employee reached up with both hands to break his fall. While doing this the knife blade came into contact with his left thumb.
Employee was walking down a flight of stairs and thought he had reached the bottom. He took a flat step but there were actually 2 more steps. This caused the employee to lose his balance and fall forward. He reached out and grabbed the top rail of the hand rail to break his fall. This caused his lower half to twist resulting in a burning sensation in his lower back.
Employee using cutting torch. There was slag on the end of the torch and employee knocked slag off with hand. His glove had a hole on the left hand index finger and when he hit torch with left hand acetylene flared and burned his left hand and index finger.
Employee was doing routine maintenance on a shaker screen, changing screen media. Temperature was in excess of 100 degrees. When the employee finished the task he entered the motor control to cool down and remove his locks, he became light headed and shaky. He immediately called for assistance and laid down.
Employee was installing a new feeder above a tunnel belt. While attempting to lift the feeder onto the conveyor belt, he felt a pain in his lower lumbar.
The employee was tearing old 1 1/2"" conduit off the ceiling of the load out tunnel, after cutting the conduit into pieces he went back to cut the straps that were securing the conduit. When he cut the first strap, the conduit fell, bounced off the conveyor belt and cut his forehead above the left eye. This resulted in a 3in laceration that required 27 stitches to close.
The ee was doing a pre-op inspection in his area when he slipped on a section of ice. The ee's feet came out from under him and he landed on his back. He was checked at the er where they determined he had strained a muscle in his back, no restrictions given. When doing a follow up 1 wk later the ee was given the restrictions of not carrying objects more than 20lbs w/his right arm.
Employee was performing preventative maintenance around 920 scrubber, got foreign object in left eye.
Employee was removing v belt guard off C11 head pulley, hit his right elbow on hand rail. Incident happened on 10/6/10 but employee didn't go to the doctor until 11/3/10.
The employee was performing maintenance on the drive system of a conveyor belt. While using a chisel and hammer, a piece of metal came off one of the tools and lodged in his left thumb.
Employee was changing out a punch plate screen , screen fell across upper left arm causing left shoulder to seperate
The employee was using a utility knife to trim the edge of a urethane screen so he could get it set into place. While in the process of trimming this screen, the utility knife slipped off the screen and cut through his glove into the bottom of his thumb. This resulted in a 2 inch laceration that required 10 staples to close.
Struck finger with sledge hammer resulting in one inch cut and broken finger.
Employee was using a pry bar clearing a chute out on conveyor belt that was stopped up with material. After he cooled down his lower back started to hurt.
EE was attempting to hook up to a 4' X 6' utility trailer to remove some old wiring. The EE needed about 1 inch to set the trailer to the receiver hitch; EE lifted the trailer by hand to the receiver hitch when incident occurred.
An employee was attemping to tension a chain and place it onto a hook. During this process the employee got his finger pinched between the hook and chain. The tip of the finger was cut and the finger nail was torn off. The employee received a tetnus shot, antibiotic shot and seven stitches.
EE strained his back while lifting up on a pry bar.
Employee was getting a striker out of tool box on service truck when the wind blew the lid closed on his left hand's little finger, causing a small fracture and needing stitches.
Three workers were measuring stock piles at the sand plant. They were using a hundred foot vinyl measuring tape with an inch and half nut secured on the end for weight. The weight is slung up on top of the stock pile to measure the slope. While slinging the tape, employee heard and felt a tear in his right shoulder.
Employee was changing out a 5 gal bottle of water at the break room. The pull seal broke off of the water bottle cap so he used his pocket knife to pry off the remaining cap and cut his right index finger.
Employee ran it to a plate of metal, cutting his right side of his nose and right eye ball.
Employee was using a pocket knife to cut plastic zip-ties, he cut his left index fingers knuckle and received six stitches.
employee stepped off concrete slab onto some rocks and twisted his ankle.
Belting broke on a stock pile conveyor while loaded. Maintenance man was examining what to do when the belt moved and allowed some rocks to fall and hit him on the left arm.
Employee was holding one end of a wire rope that is used to pull belting on convyors. The other end was attached to a fork lift and as the forks were raised it pulled on his arm.
A hand chain hoist was being used to raise the bottom side of belting on a conveyor that return idlers were being added. While placing his left hand under the belt, as to receive one side of the idler, the hoist slipped causing the belt to come down on his hand. He received a contusion / sprain to his left wrist and hand area.
This individual was on a nylon webbing ladder, inside a rail road car. As he attempted to grab a shovel located out side of the car, the ladder spun, he lost his balance and fell about three feet to the bottom of the car, landing on his rear end and left hand.
Mechanic was lifting a vise out of the back of his pickup. He heard a pop in his arm, and had severe pain in left arm. Pulled a muscle in left arm.
While walking, employee stepped on a unstable area of rocks and water; the rocks shifted causing his right ankle to twist.
Employee was exiting a transportation van. He received a laceration to his right hand from one of the side doors as he was opening it.
A morning dew was covering the hand rail and platform where the employee was performing a job. After completing the task, she started to exit the landing when her right foot slipped and she landed on her back end.
EMPLOYEE WAS PULLING ON A ROPE THAT WAS ATTACHED TO A HARD SUCTION LINE. ROPE BROKE CAUSING EMPLOYEE TO FALL TO GROUND STRIKING HIS RIGHT ELBOW. EMPLOYEE WENT ON LIGHT DUITY FOR 3 DAYS AND THE PLANT WAS SHUT DOWN FROM THE 4TH TO THE 9TH.
Mechanic was removing bolts from the connection of the mast and boom on the Drill. Thumb was pinched between the plate of the mast and trunion.
Two employees were working at the C-10 conveyor. One was cleaning debris under the conveyor with the water hose. As he threw the hose over the belt, the valve on the hose hit the belt and turned on causing the hose to release the water pressure. The pressure caused the hose to swing around and the hose hit him in the head causing injury.
This employee felt some discomfort to his left knee. He went to see the Dr and the Dr put him on some anti-inflamatories and tylenol; no light duity needed. This employee had surgery done on 6/21/07 to his knee but he has been on vacation since. This employee has had this time scheduled off since the first of the year. He has come back to work on light duity as of 7-2-07.
On February 16, 2007, EE was driving a wedge in between the shaft and the round roller that is being used to roll up belts. The round roller is made up of pressed wood and as EE had the wedge in position, a piece of the pressed wood chipped off and debris came into EE's right eye. EE was wearing his safety glasses with side sheilds at the time of injury.
Employee was attempting to straighten out the grizzly bars in the primary crusher (feeder #2). As this task was being done, the grizzly bar rolled over onto the employees left arm causing a deep laceration to the bicep.
Employee was working on the classifier at the sand plant and he was coming down the catwalk and slipped and fell into the handrail striking his left side of his chest area. The tempture was freezing and it is possible some ice had developed on walkway.
Employee was walking on catwalk when he turned his right ankle.
Was washing out Scrubber and slipped. Harness caught him and strained his left groin.
EE was welding a guard into place and struck an arc with his hood up then placed his hood down
Employee was cutting a piece of belting with a utility knife and the knife slipped cutting his left forearm. Employee recieved 3 stitches.
The employee was shoveling out a railcar and said he felt pain in his upper back. He contacted his supervisor and the supervisor took him to the doctor.
Fell off fuel truck while fueling diesel pumps. Fractured one vertebrae in his neck.
Employee was in the process of cleaning out a chute plug with a pry bar. When the product broke free it struck the pry bar which in turn caused the pry bar to strike the employee in the jaw. The employee went to the Doctor and was taking off of work due to a fracture to the jaw.
EE WAS WASHING AT THE SAND PLANT AND GOT SLURRY WATER SPLASHED IN BOTH EYES. EE DID HAVE ALL PPE ON AT THIS TIME. THE SLURRY WATER SCRATCHED HIS EYES. DR GAVE 4 DAYS RESTRICTED DUTY.
Employee was bent over cutting packing away from baring and felt sharp pain in his lower back.
Employee was bending down to pick up water hose and he felt pain in his lower back. He was taken to Dr. and Diagnosis was lower back strain.
He was entering his loader and some dust blew in under his safety glasses. We atempted to wash it out of his left eye and were unable to. We went to the dr and the dr put some drops in his left eye and removed foreign body from his left eye. Dr told him he wanted to give him some drops for his eye and then Dr told us that he could go back to work with no restrictions.
Employee was holding a wrench on the back side of bolt while it was being tightened from the other side and the wrench caught his left little finger between the wrench and belt frame.
EE was cleaning up after a job and stated that he felt somthing in his left eye he washed his eye 3 times and said that it still felt like somthing was in his eye he was taken to the Dr. the Dr. stated that he got what ever was in out when he washed it out but his eye had a scratch and the Dr. gave him prescription of eye drops. Note he stated he had all PPE on.
While walking between two screens employee stepped wrong and twisted his left ankle.
While changing out rollers Employee was climbing back on to a cat-walk and sliped hiting his right knee on a hand rail.
EE was working on a feeder when he felt something get in his right eye. He washed it X4 with no help. He went to the Dr and the Dr removed foreign body from his right eye. EE had on all his PPE at the time. Conditions that contributed to this was the wind.
He was closeing a Valve on a slurry line and tore his bicep in his right arm
Employee was moving an acetylene bottle and lost his grip letting it slip catching his right third digit between the bottle and a trailer.
Ee was removing a choker from the back of a loader that look to not have any pressure and it sprang out and hit him just above the right eye causing a small lac. to the forhead that took 11 stiches not he had on his safety glasses and hardhat at the time.
Employee was cleaning out empty rail cars when it was struck by additonal empty rail cars causing a strain to the back secondary to the jaring.
Employee was cleaning out a head pully shut with a prybar when the prybar sliped hitting his tooth and chiping it.
Employee states that while he was runing the clean-up loader his back started to get stiff and sore. He also stated that he was sore the next morning. After seeing the doctor the doctor stated he had a straind muscle in his lower back. EE states he can't remember doing anything other than operating the loader that could have injured him.
He was operating a bobcat and he stated that it locked up jaring him around causing him strain his right shoulder
Employee was sitting in haul truck being loaded by a loader he states that the rock shook the truck and caused him to strain his lower back
Employee was using a come-along to move a motor into place when it sliped causing him to lose his balance and strick his head on a pice of angle iron straining his neck
Employee was in the process of replacing troughing rollers when the roller fell over ontop of his right thumb. This caused the employee to receive a smash causing laceration that required stiches.
EE WAS OPENING DOOR WHEN THE WIND CAUGHT THE DOOR SLAMING IT ON HIS FINGERS ON HIS RIGHT HAND.
EE WAS IN THE PROCESS OF HELPING REPLACE 3""VALVE ON THE DISCHARGE SIDE OF SLURY PUMP HE WAS HOLDING THE VALVE WHILE IT WAS BEING WELDED. EE RECEIVED ARC FLASH BURNS TO THE EYES.
EMPLOYEE WAS SETTING 48"" TROUGHING IDLER UPON CATWALK TO BE PUT ON AFTER SETTING IDLERS DOWN. EMPLOYEE UNHOOKED CHAIN FROM IDLERS ONE OF THE IDLERS STARTED TO FALL OFF THE CATWALK. EMPLOYEE RE ACHED FOR IDLER TO PREVENT IT FROM FALLING. HIS MIDDLE FINGER WAS CAUGHT BETWEEN IDLER AND CATWALK HANDRAIL. EMPLOYEE WAS TAKEN TO DOCTOR. EMPLOYEE RECEIVED STITCHES IN MIDDLE FINGER.
EE WAS DISMOUNTING FROM THE DRILL RIG. AS HE STEPPED TO THE GROUND AND PUT HIS WEIGHT ON THE FOOT THAT WAS ON THE GROUND, THE ROCK HE WAS STEPPING ONTO SHIFTED, CAUSING HIM TO FALL, ROLLING HI S RIGHT ANKLE.
EE WAS SPLICING CONVEYOR BELT WHEN WORK WAS COMPLETED EE WAS GATHERING TOOLS WHEN THE BELT CLAMPS FELL OVER ONTO HIS RIGHT FOOT. EE WAS TAKEN TODR FOR XRAYS & TREATMENT ON 11/10/03. EE WAS PLA CED ON RESTRICTED DUTY TILL RELEASED BY DR.
EE WAS CHANGING TROUGHING IDLERS OUT ON A CONVEYOR WHILE THE EE WAS PULLING THE OLD IDLER OUT HEATTEMPTED TO DROP THE OLD IDLER TO THE GROUND ITHIT EE'S RIGHT FOOT. EE WAS TAKEN TO DR TO HAVE IT XRAYED. EE WAS PUT ON RESTRICTED DUTY UNTIL FOLLOW UP BY DR
EE ALLEGED THAT WHILE BEING LOADED WITH BOULDERS THE LOADER OPERATOR DUMPED THE ROCKS TO HARD CAUSING THE TRUCK TO SHAKE AND INJURING THE EE'S BACK. THIS INCIDENT HAPPENED ON 10-23-03 SHIFT BU T HAPPENED ON THE 10-24-03. THE EE DID NOT REPORT THIS INCIDENT TIL THE NEXT DAY. EE WAS TAKEN TO LOCAL HOSPITAL, EE WAS BUT ON TWO DAYS OF RESTRICTED DUTY.
EE WAS ATTEMPTING TO ENTER A SCRUBBER TO CLEAR OUT A SLURRY LINE WHEN HE SLIPPED LOSING HIS FOOTING & HE FELL ABOUT 31 FT DOWN THE DISCHARGE CHUTE. EE WAS TAKEN TO LOCAL HOSPITAL FOR TREATMENT . RECEIVED 13 STITCHES TO THE BACK OF HIS HEAD & SEVERAL BRUISES & SMALL CUTS & SCRAPES TO HIS BODY. EE WAS KEPT OVER NIGHT FOR OBSERVATION.
EE WAS REPLACING WIRING THAT HAD BURNED INTO. THE EE HAD CHECKED THE POWER TO SEE IF IT WAS SAFE TO WORK ONT HE BREAKER BOX THAT MORNING. HE AND OTHER EE WERE REPLAICNG THE WIRING AND CONDUIT ON FP-30 THIS WAS GOING ON ALL DAY. AROUND 6:00PM THE EE WAS ATTEMPTING TO LOOSEN A BOLT WHEN HE RECEIVED FLASH BURNS TO BOTH HANDS EE WAS TAKEN TO HOSPITAL FOR TREATMENT OF 2ND AND 3RD DEGREE
EE WAS AT C-11 CONVEYOR LIFTING IDLERS UP TO THE HEAD PULLEY AREA WITH A CRANE. AFTER THE IDLERS WERE LIFTED TO THE CATWALK THE EE UNHOOK THEM FROM THE CHAIN THE IDLERS FELL OVER CATCHING HIS HAND BETWEEN THE IDLERS AND HAND RAIL. EE WAS TAKEN DR TO HAVE X RAYS NO BONES WHERE BROKEN. EE RECEIVED 3 STITICHES BETWEEN THUMB AND INDEX FINGER. EE WAS SCHEDULED OFF TIL 8-19-03.
EE WAS TAKEN A SIGN OF THE PRINTER HOUSE DOOR. WHEN EE STEPPED DOWN, EE TWISTED LEFT ANKLE. EE WAS TAKEN TO DR ON 8-4-03. EE WAS FITTED WITH BRACE WAS INSTRUCTED TO WEAR BRACE TILL RELEASED BY DR ON 8-11-03. EE WAS RELEASED TO FULL DUTY ON 8-4-03.
EE WAS HELPING INSTALL A 54: RETURN IDLER ON A FIELD CONVEYOR. THE EE HAD ONE END PUT IN AND WAS ATTEMPTING TO PUT THE OTHER END IN WHEN THE CHAIN THEY WERE USING TO HELP LIFT THE IDLER IN PLA CE SLIPPED CAUSE THE IDLER TO FALL STRIKING THE EE IN THE FOREHEAD. THE EE RECEIVED A CUT ON HIS FOREHEAD, EE WAS TAKEN TO THE DR. EE RECEIVED FOUR STITCHES TO CLOSE THE CUT. EE WAS RELEASED
EE WAS WORKING AT THE PUMP BARGE TRYING TO RESTART THE PUMP FOR PLANTS TO START UP. STORMS HAD MOVED IN EARLY THAT MORNING KNOCKING THE POWER OUT FOR A SHORT TIME, EE HAD STARTED THE PUMPS AND WAS WALKING BACK TO HIS TRUCK WHEN HE TRIPPED OVER A PILE OF MATERIAL CAUSING HIM TO FALL AND STRAIN HIS LOWER BACK. 5/24/03 EE DIDN'T GO TO DR. UNTIL 5/28/03 EE WAS ON LIMITED DUTY- 6/4/03
EE WAS FUELING THE PIECE OF EQUIPMENT HE WAS OPERATING. WHEN HE INADVERTENTLY TRIPPED ON THE FUEL HOSE CAUSING HIM TO FALL STRIKING HIS FACE ON A CONTAINMENT BERM. THE EE WAS TAKEN TO THE DOC TOR, EE RECEIVED TWO STITCHES IN HIS LOWER LIP AND BROKEN TOOTH.
ON 10-31-02 EE WAS WASHING OUT FROM UNDER A TAILPULLEY TO REMOVE MATERIAL THAT HAD BULIT UP CAUSED BY RAINY WEATHER. EE GOT A PIECE OF FOREIGN MATERIAL IN HIS RIGHT EYE. EE DID NOT REPORT IT T IL 11-1-02 EE WAS TAKEN TO DR ON 11-2-02 TO HAVETHE FOREIGN OBJECT REMOVED. EE WAS TO FULL DUTY ON 11-1-02 WITHOUT ANY RESTRICTIONS.
EE WAS OPERATING MAN-LIFT CHANGING LIGHTS THROUGHOUT THE PLANT & TERRAIN WAS ROUGH. EE WAS EXPERIENCING PROBLEMS AROUND 11:00 AM, DID NOT REPORT IT UNTIL 3:00 PM THAT DAY. EE WAS RELEASED TO F ULL DUTY THE SAME DAY. EE HAD FOLLOW UP ON 10/1/02. EE PUT ON LIMITED DUTY UNTIL MORE TESTS COULD BE PERFORMED TO DETERMINE WHAT IS CAUSING DISCOMFORT IN HIS ANKLE.
THE EE WAS DOING A PRE-SHIFT ON HIS LOADER. WHENHE FINISHED HIS WALK AROUND INSPECTING, HE CLIMBED UP ON HIS LOADER TO START IT UP TO WARM IT UP, HE FELT A SHARP PAIN IN HIS LEFT ARM. EE WAS TAKEN TO DR. ON 7-11-02. IT WAS DETERMINED THAT HE PULLED A MUSCLE IN HIS ARM.
EE WAS CHANGING A PUNCH PLATE SCREEN ON A METSO SCREEN TOP DECK. HE AND ANOTHER WERE LETTING THE SCREEN DOWN TO THE CATWALK WHEN IT GOT HUNG UP ON THE SIDE OF THE SCREEN. WHILE TRYING TO FREE IT, THE SCREEN FELL CAUSING HIM TO SMASH HIS RIGHT INDEX FINGER BETWEEN THE SCREEN AND GUARD.
EE WAS ATTEMPTING TO MOVE A HOOD ON THE DISCHARGE END OF A SHAKER SCREEN TO GET INSIDE THE SCREEN TO DO SOME WORK. THE EE WAS USING A COMEALONG TO MOVE THE HOOD BACK. HE HAD A SMALL CHAIN TO H OLD THE COMEALONG IN PLACE TO MOVE THE HOOD BACK THE SMALL CHAIN BROKE STRIKING THE EE IN THE LOWER RIGHT LEG. THE EE WAS TAKEN TO THE LOCAL HOSPITAL FOR TREATMENT. EE WAS PLACED ON LT. DUTY
EE WAS REPLAICNG FRAME PIPE IN A SHAKER SCREEN. EE WAS USING COME A LONGS AND A PRY BAR TO MOVE THE PIPE IN PLACE TO BE WELDED THE PRY BAR HE WAS USING SLIPPED CAUSING HIM TO STRAIN HIS LOWER BACK MUSCLES. THERE IS LIMITED SPACING IN THE SCREEN.
THE EMPLOYEE WAS HELPING CHANGE OUT A SLURRY LINE. HE WAS ON ONE END OF THE PIPE, WELDING A FLANGE ON THE END. AS HE WAS CHANGING SIDES HIS LEFT BOOT GOT STUCK IN THE MUD. WHEN HE TRIED TO FRE E IT HE COULDN'T ON THE FIRST ATTEMPT. HE THEN PULLED HARDER. THE NEXT TIME HE FREED HIS BOOT BUT IN THE PROCESS INJURED HIS LOWER BACK. HE DIDN'T REPORT IT UNTIL 4/19 & SEE DOCTOR 4/22.
EE WAS HAULING BOULDERS TO THE LEVEE IN THE EASTPIT, THEY HAD BEEN WORKING ON FOR A FEW MONTHS. THE LOADER OPERATOR HAD PUT ONE BUCKET IN THE BED OF THE HAUL UNIT FOR CUSHION WHEN THE OPERAT OR WAS ATTEMPTING TO PUT THE SECOND BUCKET ON THE TRUCK A BOULDER WEDGED IN THE BUCKET. THE BOULDER CAME FREE AND BOUNCED THE OPERATOR INSIDE THE CAB. EE WENT TO DR.4-8-02 & RELEASED TO WORK.
EE WAS REPLACING A 52"" BREAK OVER PULLEY. WHILE STANDING ON THE TOE-BOARD HE WAS REACHING FOR A CHIPPING HAMMER THAT WAS LYING ON THE BELT. HIS FOOT SLIPPED OFF TOE-BOARD CAUSING HIM TO FALL A GAINST THE INSIDE HANDRAIL, BRUISING HIS RIBS ONTHE RIGHT SIDE.
EE FELL WALKING UP STAIRS, FALLING ON BOTH KNEES& LT. HAND ON 2/15/02. EE DIDN'T WANT TO GO TO A DR. TILL 3/28/02, X-RAYS WERE TAKEN & NO DAMAGE WAS FOUND. DR. SAID POSSIBLE TENDONITIS. AFTER SEVERAL FOLLOW-UP VISITS, AN MRI WAS DONE & SHOWED A TORN MENISCUS CARTILLAGE, OUT-PATIENT SURGERY WAS DONE ON 10/10/02. EE WAS RELEASED WITH NO RESTRICTIONS.
EE WAS HELPING REPLACE AN OLD BELT AND PUTTING ON SOME NEW IDLER'S. AFTER THEY HAD PUT THE IDLER ON THE FRAME OF THE BELT WAY EE WAS ATTEMPTING TO LINE THE IDLER IN PLACE WHEN THE END SLIPPED OFF THE FRAME CAUSING THE IDLER TO TWIST UPWAID CATCHING EE HAND BETWEEN THE IDLER AND CONVEYOR FRAME CUTTING EE RIGHT HAND PALM AREA.
EE WAS CLEANING MATERIAL OFF A CATWALK THE MATERIAL WAS 8"" TO 12"" ROCK THAT HAD BEEN SPILLED OVER ONTO THE CATWALK, BY THE DISCHARGE CHUTE STOPPING UP. THE EE HAD MOVE A ROCK WHEN ANOTHER ROCK MOVED CATCHING EE S FINGER IN BETWEEN BOTH ROCKS. SMASHING HIS RIGHT MIDDLE FINGER.
EMPLOYEE WAS LOADING TABLES IN A TRAILER. HIS FOOT WENT THROUGH A HOLE IN THE FLOOR AND HE SPRAINED HIS BACK. X-RAYS WERE NEGATIVE. HE WAS PLACED ON A 25 LB MAX LIFTING RESTRICTION UNTIL 10/29 /01.
EE WAS WORKING ON A PUMP. THE IMPELLER WAS FULL OF MATERIAL. EE WAS USING A PIPE WRENCH TO WORK THE PUMP BACK & FORTH TO FREE IMPELLER. EE DIDN'T REPORT THIS UNTIL 10/22/01. SAW CHIROPRACTOR.
EMPLOYEE WAS TAKING SAMPLES OFF OF 4 DIFFERENT WITHT HE HELP OF OTHER EMPLOYEES. AFTER GETTING SAMPLES EE AND ANOTHER WERE WORKING ON REPLACINGSIDE BOARDS ON SS-14 SCREEN. EMPLOYEE WENT HOME AROUND 4:30PM WHEN HE GOT HOME HE TOOK A COLD SHOWER TO COOL DOWN, IT DIDN'T HELP. WENT TO HOSPITAL AND DIAGNOSED WITH HEAT EXHAUSTION.
EMPLOYEE WAS RUNNING MATERIAL THRU THE LOADING TIPPLE. EE WAS HAVING PROBLEMS GETTING CLODS IN FEEDER CAUSING IT TO STOP UP. EE WENT DOWN THE THE TUNNEL TO SEE WHAT PROBLEM WAS. EE STUCK RIGHT HAND UP IN FEEDER TO REMOVE CLODS HE HIT THE KILL CABLE KILLING THE BELT CAUSING FEEDER GATE TO CLOSE ON HIS WRIST. EMPLOYEE FAILED TO LOCK-OUT BELT BEFORE WORKING ON FEEDER.
EMPLOYEE WAS CLEANING ROAD ENTRANCE ON VULCAN SIDE SCALEHOUSE. ENTRANCE ROAD RIGHT OF WAY WAS BLOCKING VIEW OF TRAFFIC LEAVING THE PLANT. EE REPORTED THIS ON 6-28-01, WENT TO DOCTOR ON 6-28-01 , RECEIVED A SHOT. EMPLOYEE HAS NO RESTRICTIONS.
EMPLOYEE WAS WALKING TO HIS WORK VEHICLE, WHILE TALKING TO ANOTHER EE, FROM TALKING TO WITNESSES, EE TRIPPED OVER HIS OWN FEET, EE STARTED TO FALL, EE WALKING WITH HIM REACHED OUT TO KEEP HIM FROM FALLING TO THE GROUND, AROUND EE'S NECK, CAUSING EMPLOYEE TO STRAIN MUSCLE IN NECK. EE DIDN'T REPORT THIS UNTIL 6-24-01, WENT TO DOCTOR 6-26-01.
EMPLOYEE WAS INSIDE VSI CRUSHER CHANGING SHOES OUT. SHOES HAVE TO BE LOWERED IN FROM THE TOP BY A SPACE IN CRUSHER, YOU CAN NOT STEP WHILE TURNING YOUR BODY, ALL YOU CAN DO IS TWIST YOUE TORSO . EE DIDN'T REPORT THIS UNTIL 6-26-01.
EMPLOYEE WAS ATTEMPTING TO HELP OTHER EMPLOYEES CHANGE A SCREEN OUT ON 110 SHAKER SCREEN. EMPLOYEE WAS IN A HURRY. WHEN EMPLOYEE STEPPED FROM PLATFORM TO SCREEN WHILE IN PROCESS, EMPLOYEE SLIP PED, TWISTING LEFT KNEE. ACCIDENT HAPPENED 5-16-01, EMPLOYEE DIDN'T REPORT UNTIL 5-18-01.
THE EE WAS ASKED BY ANOTHER EE TO HELP INSTALL SOME TROUGHING IDLERS ON C-5 BELT. THE EES WERE USING A CRANE TO HELP LIFT THE ROLLERS IN PLACE. THESE IDLERS WEIGH ABOUT 200# EACH. ONCE THE IDL ERS ARE IN PLACE THE EES WOULD SLIDE THE IDLERS IN PLACE ON FRAME. THE IDLER SLIPPED ON THE FRAME SMASHING THE RING FINGER ON EE'S HAND.
EE ALLEGED HE WAS CLEANING RAIL CARS BEHIND WAREHOUSE, WIND WAS BLOWING. THERE WAS SOME POISON IVY IN THE AREA(AT THE TIME IT WAS NOT KNOWN). THE POISON IVY WAS IN BLOOM. THE EE SAID HE DIDN'T TOUCH THE IVY BUT THE WIND HAD BLOWN SOME STUFF ON HIM.
EMPLOYEE WAS AT WAREHOUSE TO PICK UP A PULLEY TO PUT ON A BELT. PULLEY WAS STANDING UP ON ONE END. (THE PULLEY STANDS ABOUT 58 INCHES TALL) EMPLOYEE CLIMBED ON TOP OF PULLEY TO PUT A CHAIN ON IT. EMPLOYEE GOT DOWN TO GROUND AND FELT SOME BACK PAIN. ACCIDENT HAPPENED 4-7-01 WAS NOT REPORTED TILL 4-17-01.
EE WAS ATTEMPTING TO OPEN DOOR ON GREASE HOUSE TO GET OIL OUT TO SERVICE PLANT. DOOR WAS STUCK SHUT. EE WAS ATTEMPTING TO OPEN DOOR WHEN EE PULL HARD ON DOOR, THE DOOR HIT EE'S LEFT THUMB.
EE WAS CLEANING RAILCARS USING A VACUUM HOSE.EE WAS ATTEMPTING TO GRAB HOSE FROM ANOTHER EE. WHEN THE EE HANDED IT TO HIM, EMPLOYEE'S LEFT HAND BENT BACKWARDS. EE WAS INJURED ON 3-17-01. EE DI D NTO REPORT THIS UNTIL 3-19-01. WENT TO DR. 3-19-01.
EE WAS WALKING TO C-19 AND C-20 CONTROL PANEL TO START PLANT UP. EE WAS RETURNING BACK TO VECHILE, HE WAS ATTEMPTING TO STEP ACROSS A WATER PUDDLE WHEN HE PUT HIS LEFT FOOT DOWN AND IT SLIPPED CAUSING HIM TO TWIST HIS LEFT ANKLE.
EMPLOYEE WENT TO SEE DOCTOR WITH NUMBNESS IN HI SLEFT HAND, AFTER TEST WERE DONE THE DOCTOR SAID HE HAD CARPAL TUNNEL SYNDROME IN HIS HAND. THE DOCTOR ALSO SAID HIS RIGHT HAND IS EXPERIENCING SOME SYMPTOMS. DOCTOR SAID IT WAS FROM REPETITIVE MOTION OF OPERATING A LOADER FOR SEVERAL YEARS.
EE WAS CLIMBING OUT OF GONDOLA RAIL CAR OVER THE END. HIS FOOTING SLIPPED. HE FELL ABOUNT 4' TO THE GROUND RECEIVING BRUISES TO BOTH LEGS. THE DR PUT A SPLINT ON ONE SIDE OF LEFT LEG AND ASKED HIM TO RETURN TO DR 9/28/00.
INJURED EMPLOYEE NORMALLY WORKS IN SCALEHOUSE. HE WAS ASKED TO RUN A LOADER IN PLANT OT HELP LOAD RIAL CARS. HE DID THIS FOR TWO HOURS AND ALLEGED HIS BACK WAS SORE 7-14-00. HE SAID HE FELL IN HIS YARD 7-14-00, WENT TO ER DR 7-14. HE WAS REFERRED TO ANTOHER DOCTOR ON 7-25-00.
EMPLOYEES WERE INSTALLING KNIFE VALVE IN SLURRY LINE. ONE BOLT INSTALLED IN ONE SIDE, ANOTHER SHOULD HAVE BEEN ON OPPOSITE SIDE. INJURED PUT HISFINGER IN A HOLE TO REMOVE SAND, VALVE SHIFTED, CAUGHT FINGER AND CUT IT.
EE, EQUIPMENT OPERATOR, WAS DOING PRE-SHIFT INSPECTION ON AN END DUMP HAUL TRUCK. HE TRIPPED, BROKE HIS FALL WITH HIS RIGHT HAND. HE COMPLAINED TO SUPERVISOR AT 9:30 PM THAT HIS R SHOULDER WAS HURTING. HE WAS TAKEN TO DR AND RELEASED WITH RIGHT ARM IN SLING. A RETURN VISIT WAS SCHEDULED TWO WEEKS LATER.
EE WAS REPLACING SPIRAL BOLTS IN SCURBBER ON DISCHARGE END A PIECE OF HOT METAL FELL AGAINST THETORCH HOSE, BURNING THE ACETYLENE HOSE CAUSING A SMALL FIRE. EE GRABBED THE HOSE TO PINCH IT TH E GAS OFF. IN THE PROCESS THE OXYGEN SIDE HOSE BROKE CAUSING A BIGGER FIRE BURNING THE EE'S RIGHT THUMB
EMPLOYEE WAS REMOVING A COVER OFF THE SUCTION LINE ON THE DREDGE, TO REMOVE SOME ROCKS THAT HAD PICKED UP DURING DREDGING OPERATIONS. THE COVER TO BE REMOVED IS UNDER THE OPERATOR'S CAB. EMPLO YEE HAD TO BEND OVER, LIFT WITH ARMS EXTENDED AND TWIST BODY. COVER WEIGHS APPROX 70 LBS.
EMPLOYEE WAS WELDING ON FEEDER PLATE FOR A SHAKER SCEEN. HOT METAL GOT INTO R. EAR. IT BURNED FOR ABOUT 3 HOURS AND STOPPED. HE REPORTED IT ON 4-19-00. DR STATED INFECFTION FROM BURN CAUSE D EAR TO CLOSE UP.
EMPLOYEE WAS USING HAND HELD GRINDER TO SMOOTH UP EDGE OF METAL ON 3-30-00. HE WAS NOT AWARE OF ANYTHING IN EYE. LATER HE USED EYE DROPS THINKING EYE WOULD GET BETTER. A FEW DAYS LATER HIS VIS ION WAS BLURRY. DOCTOR FOUND STEEL IN EYE AND REMOVED IT.
