EE was parked parallel to a Genie S85 man lift. Attempted to pull out from machine when he realized left rear wheels were choked. Exited the truck to remove wheel chocks, truck began to roll backwards. EE was struck/trapped between basket of Genie lift and the left front of truck. EE exited truck with engine running and transmission on neutral with parking brake partially engaged.

Employee was removing the clamp that held the stationary belt during the repair the clamp released suddenly and struck employee in the nose between the eyes causing a laceration and resulting in stitches.

EE WAS ENTERING THE PLANT TO REPORT FOR WORK AND REACHED IN FLOOR TO TRUCK TO PICK UP SOMETHING AND RAN IN A DITCH HITTING HIS HEAD. INJURY RESULTED IN STITCHES.
