Employee was getting meter reading from a truck. As he turned and stepped away from the truck, he tripped and fell, catching himself with his left hand. He sustained a servere contusion to his wrist. He was placed on modified duty on 2/29/08 after receiving medical attention.

EE WAS DIGGING OUT A TRENCH WITH THE EXCAVATOR. HE WAS MOVING THE EXCAVATOR BACKWARDS WITH THE TURNTABLE TO HIS LEFT, WHICH WAS THE WRONG POSITION. THE WEIGHT OF THE EXCAVATOR CAUSED THE TRACKS TO FALL INTO THE TRENCH ON ITS SIDE. EE IS HAVING BACK PAIN. (EXCAVATOR ENDED UP ON ITS SIDE; EE WAS NOT EJECTED FROM CAB - WAS WEARING SEAT BELT.)

EE WAS LIFTING AN AUGER INTO THE RACK OF THE DRILL RIG. HE WAS LIFTING WITH ONE ARM. ANOTHER EE HAD THE OTHER END OF THE AUGER. AS HE LIFTED IT TO THE MAXIMUM HEIGHT, HE FELT PAIN IN HIS RIGHT SHOULDER. (THE AUGER WEIGHS APPROXIMATELY 80 LBS).

EE WAS DISCONNECTING THE HYDRAULIC HOSES TO THE SCRAPER PANS CONNECTED TO THE JOHN DEERE TRACTOR HE STEPPED OFF THE TRACTOR LANDING ON A HYDRAULIC HOSE CAUSING HIM TO TWIST HIS LEFT ANKLE.
