While scaling in 33 West, a rock fell from the back and struck the scaler boom jolting the cab causing the operator to hit the windshield with EE's forehead and right shoulder. The operator was wearing a seat belt. EE as a mild concussion and a strained A/C shoulder joint. Scaler equipped with a lap belt style seat belt.

EE was cleaning on top of the kiln silos. As they went to straighten up to lift the wheelbarrow they heard and felt a pop in their back and was in a lot of pain. Dr. released them with no restrictions, went to Ortho on 4/13/26 due to continued pain. Dr. gave restriction and EE is now off work.
Employee was following wires in the engine compartment for a light that was not working on a piece of equipment when their fingers contacted the fan blade of the engine cutting 2 fingers. They received 7 stiches in one finger and was put on restricted duty for 10 days.

Employee was installing the toggle seat in the jaw crusher when one of the come-a-longs hooks came out of the eyelet when EE was prying the opposite side into place. The loose end of the toggle seat swung down and over crushing the maintenance employees' arm against the side of crusher. No fractures, but there was damage to soft tissue, ligaments, & tendons in EE's arm.

Employee was straightening a piece of pipe by hand. The pipe kicked back striking right thumb. Went to Dr. 6/12 and was released to full duty. Went back to Dr. on 6/20 and was released to work with restrictions and a brace with possible torn ligament in right thumb.

Putting a track back on an excavator when the come-a-long broke causing the chain to whip back hitting EE's left hand. EE has a hairline fracture of the left thumb. EE's left hand was splinted and returned to work.
Employee was sweeping the floor of the kiln firing floor when they felt something pop in lower back. Dr. reported they had a lumbar strain and placed them on restricted duty.
Employee was welding inside screen, when the wind blew something into left eye. Doctor removed a small sliver of metal from the eye and prescribed antibiotic drops. No time missed or work restrictions.
EE stepped on a slick spot resulting in a jerking motion in EE's back. At the time of reporting the incident, the discomfort was not bad, but increased over the next few days. When EE came into work on 9/7/21, EE requested to see a doctor that the pain had gotten worse and wanted checked out. EE was seen at the doctor on 9/8/21 and placed on restricted duty.
Prying a sleeve from a shaft of a rotary air lock at the hydrate plant when the bar slipped and EE's weight shifted back causing EE to roll ankle, resulting in a fracture to the right ankle.
Miner was filling face drill with water. When they turned the ball valve it broke in half causing the valve to come loose and striking them in the left knee. The high pressure water knocked them back against the rib and floor of the mine causing bruising to side, back and shoulder.

Employee was climbing down a ladder from the head box of a conveyor, when EE stepped off the ladder EE twisted EE's knee. EE did not seek medical attention until EE reported to work on 11/19/20. EE was taken to a doctor and was wrote out of work with a brace on EE's knee and no walking or climbing until follow-up on 11/24/20

Employee was moving a pump hose when employee felt something pop in shoulder. Employee was seen by a doctor and returned to work and was able to perform normal duties. Employee worked normal job until 11/2/2020, when employee was scheduled for surgery on 11/3/2020 causing employee to miss work.

Employee mashed finger when lowering a bearing assembly to the press table. A block slipped from under the assembly mashing EE's finger between the assembly and the stand. Diagnosis was a fractured distal phalange (fingertip) no restrictions or treatment. EE was wearing leather work gloves at the time of the injury.
Employee was holding a small hydraulic pump (approx. 2 lbs) with arm extended for an extended time. EE complained of shoulder pain, went to doctor returned to work full duty. Worked until 2/7/19 when EE went to doctor. EE was placed out of work and referred for rotator cuff surgery from a strain.
Employee stated ee may have hurt shoulder pulling on a basket the shift before off days. The basket weighs approx. 23 pounds and is placed in the top of a tanker truck to catch oversized material. Employee was off 7 days for scheduled time off and did not report the incident until the day ee was scheduled to return.

Individual was hand scaling the roof from a man lift. While prying on a rock that was hung on a roof bolt the scale bar slipped and the force from pulling the bar and their weight caused them to roll their ankle causing a fracture to the right distal fibula.

Employee strained right shoulder while using a 3 foot pipe wrench to remove a 4 inch ball valve in the road rock tunnel.
Employe got lime dust in left eye while measuring Kiln silos lowered tape measure into measuring port causing dust to blow up in face and under glasses.
Employee strained right elbow while installing water pump. Employee was pushing/pulling during installation.
Employee suffered a strain to shoulder while pulling a rubber hose water line.
Employee bent over to pick up a sample screen in the lab and felt a burning sensation in lower back.
Employee was shoveling lime on the kiln firing floor and suffered a dislocated elbow from awkward movement.

Employee felt he had been exposed to muriatic acid fumes while cleaning lime from a radiator. He was nauseous and was throwing up. He was outside in good ventilation.
Employee was attempting to unload a chute from a screen and lime fell onto his safety glasses. As he removed his safety glasses, lime got into his eye.
Employee was helping co worker remove screw flights from a screw trough in the sizing system weighing approximately 150 lbs. He felt a pain in his right lower back and hip.
Operator was walking through the bottom floor of the product silos to adjust feeder controls and dry lime got into his left boot due to holes from wear. Socks were wet due to walking in water earlier in the day. Employee did have a new pair of boots at home, but didn't wear them. Did not become reportable until 4/20/15

Employee was sizing a metal patch for a fan housing. His finger got sucked into the hole and cut the tip of his left, middle finger at the joint.
Employee was leaning forward to pass under a screw conveyor and felt a strain in his lower back. Conveyor is approximately 5 feet off the floor.
Employee was tightening a rail car puller cable clevis attachment under a loading point. Lime dust from the conveyor above fell and got into employees eye. He was wearing RX safety glasses with side shields. The dust was embedded and he received a prescription.
EE was Walking under the kiln bag house on his way to stone tunnel. His hard hat had fallen off and as he stood up while under the steps hit the top of his head on a sharp metal end of angle that's supports the steps. EE received two staples at the ER.

Employee clocked and drove to kiln employee parking area and the proceeded to walk to the Kiln control room. As he stepped onto the first catwalk located under the Unit train conveyor # 2 just outside Unit train stone tunnel his right foot landed wrong on the walkway turning his right ankle.
EE was making repairs to the bend pulley. After unlocking the belt, and then verifying that the belt conveyor was running correctly, he gave instruction for to turn the belt off so he could put the guard back on. EE was picking up a hammer, which got caught by the belt and EE's right arm was caught between the shaft of the bend pulley and the conveyor truss.
On 7/24/2012 at about 11:45 pm an employee and injured employee were replacing idlers in the 5A belt. IE was pulling on the old idler to free it when it came loose all at once and fell and hit IE on his right foot.

EE pulled his truck to the side of the rib so the driller could walk backwards. As he stepped out of the truck his left foot landed on a small rock and the rock rolled from under his foot into a hole where a rock had been pulled out by the track hoe when it was scaled.
Employee was rewiring a water pump on a floating pontoon. When we tried to ""whip"" the slack out of the cable (large diameter), he felt a pull in his left elbow. Employee was referred to an orthopedic and discovered a tendon tear at the bicep. Surgery is being schedule and will cause lost time.

80 year old man reportedly, suffering from dementia, drove into the UG mine sometime between 3/20/08 and 3/24/2008. He became stuck and exited his van. Apparently he attempted to exit the mine with a flashlight. He apparently lost the batteries power, could not get out and was found dead the morning of 3/24/2008.

2 EE's were Roof bolting on the 5 west ramp. They proceeded to install a bolt in the area to the right of 10 previously installed bolts. Using a jackleg drill they started to collar a hole noticing the rock looked suspect. They started to move when a slab fell striking them with debris. Fall was arrested by fall protection. Both men sustained mutiple injuries.

Somewhere between 1:30 PM and 3:30 PM approximately 50 tons of rock fell from the roof impeding the main haulage way. The rock fell at 5 West daylight a.k.a Roof Grid C-3. When the fall was found the area was barracaded from entry. During the night more rock fell bringing the total to 85-100 tons.

EE was carrying 4 foot tubes of resin into the roof bolter. One of the tubes got hung and EE twisted causing pain in his right side down his right leg. EE did not loose any time until 7/5/05. EE is going in for further testing and might lose time again.

APPROXIMATELY 300-500 TONS OF SHALEY LIMESTONE FELL BLOCKING THE HAULAGEWAY OF B-LEVEL RAMP AT APPROXIMATELY MINE EAST 14,100. THE ROCK FELL SOMETIME BETWEEN 6:00 PM ON 8/23/03 & 5:20 AM ON 8 /25/03. NO ONE WAS WORKING IN THE MINE WHEN THE ROCK FELL. THE FALL WAS DISCOVERED DURING A PRESHIFT INSPECTION OF WORKING AREAS BY JERRY LYNCH. THE ROCK IS BLOCKING ACCESS TO ALL WORKING FACE

EE WAS PUTTING ON A HARNESS WHEN HIS FOOT SLIPPED OFF A ROCK TWISTING HIS LEFT KNEE. AN MRI WAS SCHEDULED & HE WAS RETURNED WITH NO RESTRICTIONS BY THE DOCTOR TO COME BACK TO WORK. AFTER THE M RI WAS DONE IT WAS FOUND THAT THE EE HAD A TORN ACL & WILL HAVE SURGERY.
EE AND COWORKER WERE IN 5 WEST TO 6 HAND SCALING ROOF. THEY WERE APPROXIMATELY 60' IN THE AIR NEAR THE ROOF. THEY HAD SOUNDED THE ROOF PREVIOUSLY AND IT SOUNDED SOLID. EE WAS REACHING TO SCALE A ROCK ABOUT 4' FROM BASKET WHEN THE ROCK ABOVE HIM FELL. FOUR ROCKS FELL, SOME STRIKING HIM, RANGING FROM 100 TO 500 LBS. COWORKER WAS NOT HIT BY ANY ROCK.
EE WAS LOADING BOLTING RACK FROM THE FRONT BASKET ON THE BOLTER. THE TIE WRAP ON THE BOLTS BROKE AND HE PULLED UP CAUSING PAIN IN HIS BACK. IT WAS NOT REPORTED BEFORE NOW BECAUSE HE WAS ABLE T O DO HIS JOB UNTIL TODAY (7/30/02).
EE COMPLAINED THAT HIS SHOULDER WAS HURTING FROM HIS ACCIDENT ON 10-4-01. HE WENT TO SEE DR AND IS ON RESTRICTED DUTY.
EE WAS STARTING A DIESEL PUMP. HE PULLED ON THE START ROPE AND HAD A CATCH IN HIS SHOULDER.
A SECTION OF ROOF FELL INT HE 4 WEST HAULAGE WAY IMPEDING PASSAGE TO THE WORKING FACES. NO ONE WAS UNDERGROUND WHEN THIS INCIDENT OCCURRED.
LOADING BOLTS ONTO THE ROOF BOLTER WHEN HE FELT A STING IN HIS BACK. IT STARTED HURTING LATER.

EE WAS POURING ANTIFREEZE INTO TRACKHOE WHEN HE LOST HIS BALANCE AND FELL TO THE GROUND. HE WAS UNCONSCIOUS FOR 3 TO 4 MINUTES.
INDIVIDUAL WAS CLEANING UP BOXES AND PALLETS AND STEPPED ON A ROCK AND TURNED HIS ANKLE AND KNEE.
THE ROOF AT E 13,650 N 12,000 (THE S-CURVE ON THE WAY TO 5 WEST) FELL. THE MINE SUPERINTENDANT HEARD THE FALL AND WENT TO INVESTIGATE. APPROXIMATELY 200 TO 400 TON OF ROCK FELL IN THE ROAD ALM OST BLOCKING THE ROADWAY. THE MEN THAT WERE PASTTHE BLOCKED OFF SECTION WERE BROUGHT OUT USING THE ESCAPE LADDER BETWEEN C & B LEVELS. THE ROADWAY WAS CONED OFF. NO ONE WAS INJURED.
