Fall from machine

An operator slipped and fell while exiting the 242D3 skid steer. Employee fell on right side with their right hand taking most of the impact.

Struck by falling object

The employee was manually clearing material out of the cone crusher (choked), as employee was doing it a rock fell and struck employee's right forearm. The employee received 9 stitches.

Employee was exiting a light duty vehicle when their foot slipped in-between the railing and the truck which resulted in a fracture to the left ankle.

Caught in, under or between a moving and a stationary object

Employee was trying to remove a loader bucket tooth from the 136 conveyor belt. The employee stopped the belt and the tooth was under the belt magnet. When they grabbed the tooth, the magnet pulled their hand up with the tooth and smashed a finger.

Employee states EE was working on the ball mill trying to fix an oil leak. EE went to leave and lost EE's balance and grabbed the handrail, EE then felt burning and sharp pain in EE's left shoulder area.

Employee fell while washing out under a silo with a water hose. When the hose was turned on, the pressure made employee take a step backwards and employee tripped, landing on a step and felt pain in lower back.

An employee smashed their finger while installing rebar with a 4lb hammer. As the employee swung the hammer EE missed the rebar and EE's finger hit the side of the form board.

Struck against stationary object

Technician was removing a cone seal from the engine of 33010 Truck. When the seal broke loose, technician's hand nicked the corner of the block and cut EE's knuckle.

Employee was performing routine maintenance of crusher and when walking down crusher stairs, foot got hung up causing employee to lose balance. In the process of catching self from falling, employee jammed/twisted back.

The injured employee was overseeing a trailer being unloaded. The chain snapped and caught IE's ear. IE was wearing all of their required PPE. The injury was minor and required just a few stitches.

While removing bucket off loader employee struck dog bone pin with pipe, the pin slipped out and the pipe moved, resulting in pinching fingers between pipe and bucket. Employee crushed 3rd and 4th digit finger and distal fracture of ring finger.

Struck by flying object

EE set up a hand pump press with one end against the left loader frame and the other end against the right lift cylinder pin. EE had hand pump engaged with pressure on the pin. The pin did not move. EE climbed down off the machine to get a drink and as he walked toward his truck, he heard a pop noise and he turned around to see what the noise was the pin ejected out, hit his head.

While bending down injured stated that his knee started hurting, he finished his shift and went to seek medical treatment the next day, diagnosed with strained MCL right knee.

EE was replacing belt clips on discharge conveyor, when dried mud fell onto his face. When EE tried to wipe the material off of his face, a small piece fell behind his safety glasses and into his eye. Debris was removed and an antibiotic ointment given.

EE was stepping off of a ladder, when his foot slipped on the edge of a trough in the floor, causing him to fall and strain his left knee.

Employee was working in the lab. He broke a burett containing Nitric Acid. The acid splashed on his chest resulting in minor burns. Employee was treated at a medical facility and released.
