Employee was pulling the chain on a come-a-long when EE stepped back to gain more momentum, employee felt a pop in EE's lower right leg.

Employee was walking into the main office, tripped on the front steps/rug and hit knee on the concrete platform.

Employee squatted down to pick up a screw, injuring their left knee. Prescribed physical therapy on 9/14/2023.

Contractor was sitting in service truck with door open when EE blacked out and fell to the ground striking head on the ground. EE drank coffee, chocked on coffee then passed out and fell out of the truck.

Employee was reaching for a bucket with leg and felt a pop in right knee.

Mechanic was pulling on prybar when bar slipped hitting him in the mouth.

Employee was performing maintenance at Jaw Crusher and was using a pocket knife to cut a plastic tie strap, when the knife cut through the strap and cut the top of his left hand through his glove. Employee received medical treatment consisting of 2 stitches and a tetanus shot. No restrictions.

Employee was working under the hood of the Cat 988F loader when the lower support bolt that holds the safety support cylinder sheared off allowing the hood to fall striking employee in the back. Injury is bruise on back and is currently on restricted duty.

Employee was cleaning stuck material from hopper and 6"" diameter chunk of compacted dust hit his leg causing a bruise.

Employee strained muscles in right shoulder lifting screen cloth.

Strained back carrying screens up tower.

Employee's right hand struck by cone crusher feeder plate when a welded lifting eye broke.

WHILE PERFORMING WORKING PLACE EXAMINATION, EMPLOYEE BENT OVER TO MOVE SOME WEIGHTS ON THE CRUSHER DECK. HE HANDLED THEM INCORRECTLY, RESULTING IN A LOW BACK STRAIN, WHICH DID NOT BOTHER HIM UNTIL ABOUT 20 MINUTES LATER.

EE SUSTAINED ELECTRICAL SHOCK WHILE CLEANING UP WITH WATER HOSE AT PRIMARY CRUSHER.
