Contractor was sitting in service truck with door open when EE blacked out and fell to the ground striking head on the ground. EE drank coffee, chocked on coffee then passed out and fell out of the truck.

Employee was performing maintenance at Jaw Crusher and was using a pocket knife to cut a plastic tie strap, when the knife cut through the strap and cut the top of his left hand through his glove. Employee received medical treatment consisting of 2 stitches and a tetanus shot. No restrictions.

Employee was working under the hood of the Cat 988F loader when the lower support bolt that holds the safety support cylinder sheared off allowing the hood to fall striking employee in the back. Injury is bruise on back and is currently on restricted duty.

WHILE PERFORMING WORKING PLACE EXAMINATION, EMPLOYEE BENT OVER TO MOVE SOME WEIGHTS ON THE CRUSHER DECK. HE HANDLED THEM INCORRECTLY, RESULTING IN A LOW BACK STRAIN, WHICH DID NOT BOTHER HIM UNTIL ABOUT 20 MINUTES LATER.
