Employee was accessing conveyor to clear rocks from the belt and slipped and landed on catwalk on EE's left shoulder and hip. EE was utilizing fall protection at the time of the incident.

Employee reported to supervisor on 1/4/2025 that employee allegedly hurt wrist on 12/4/2024, approximately one month prior, during a maintenance project while using a hammer. Employee sought medical treatment on 1/4/2025, and advised management of being prescribed a wrist brace and prescription.

Employee reported that they were removing rocks from a catwalk in November and later alleged soreness in their left hamstring. Employee thought they would be fine, but approximately a month later, was still feeling pain in the left hamstring and sought medical attention on 12/09/2024. On 12/09/2024, management was made aware of employee being prescribed physical therapy.

Employee pulled away from office in service truck and was struck in passenger door by customer truck. On 12/12/2024, employee sought medical attention where management became aware EE was given physical therapy order.

Employee was cutting a water hose toward the barbed end of the nozzle when the knife slipped and cut the top of EE's left hand between the thumb and index finger.

Employee was in the process of reinstalling a roller frame on a dozer. As the employee was sliding the frame back on the dozer a bogie slid and fell onto the employee's left hand.

Dozer became stuck in wet material. After employee hooked cable to dozer ee turned and stepped away to go back to operator's cab ee felt a pop and sharp pain to the front side of right knee.

Employee was climbing up ladder on 773 Haul Truck and felt a pop in right knee. Employee asked to seek medical treatment on 02/07/18 and was diagnosed with a strain.

3rd Part Contractor working on roof and fell. Contractor has applied for ID, but has not received it yet

Tire contractor was raising tailgate on service truck when Vulcan employee attempted to assist and right thumb was caught in tailgate hinge

Injured employee was climbing up the steps of the crane ladder when his lower leg popped.

A precautionary immediate report was made because of the potential for the inundation of water into the pit. A creek adjacent to the pit overflowed during a significant rain event resulting in water overflowing the creek and going into the pit. There were no injuries and no health concerns.

Injured employee stepped out of service truck and injured employee was struck by the passenger side door causing a contusion to the right knee.

Injured employee had the deck washed off and was removing a 990 shank and tooth loader from the platform when he lost his grip and dropped shank and tooth loader on his right foot. Resulted in a contusion.

The employee was sitting on the service truck with another employee discussing service issues on the plant. The injured employee started having symptoms of a heart attack. The co-worker transported him to the office where he was later transported to hospital where he was prounounced dead on arrival. Heart attack

Employee was laying out cast boosters and began to feel a skin irritation. As day progressed, hands swelled and fingers began to blister.

CRANE OPERATOR LOST CONTROL OF MOVING CRANE OR INCLINE JUMPED FROM CRANE ONTO ROCKS.

WAS DRILLING HOLES IN CONVEYOR SECTION WHEN DRILLBIT GOT BOUND IN HOLE, CAUSING DRILL MOTOR TO TWIST OUT OF HAND. DRILL HIT BACK OF HAND.

WAS INSTALLING SHUT OFF VALVE IN SPRAY BAR, WAS STEPPING FROM CONVEYER ONTO CATWALK, WHEN HIS FOOT CAUGHT ON PULL CORD, CAUSING HIM TO FALL.

LIFTING AND CARRYING REBAR ALL DAY CAUSED BACK STRAIN.

EE WAS WALKING DOWN CATWALK WHEN HE TOUCHED THE HANDRAIL WITH HIS LT. HAND. THE HANDRAIL HAD JUST BEEN WELDED AND WAS HOT. THIS BURNED HIS LT. HAND.

EE WAS ERECTING STEEL FOR FEEDER SUPPORT. HE WAS TRYING TO SET A KNEE BRACE UNDER AN I-BEAM BY HAND. HE TRIED TO HOLD IT WITH HIS SHOULDER AND BOLT IT UP AT THE SAME TIME. BY PUSHING ON THE KNEE BRACE WITH HIS SHOULDER. HE FELT PAIN IN BACK RIGHT SIDE.

STEPPED OFF CEMENT FOUNDATION ONTO STEEL LAYING ON GROUND. THE STEEL SHIFTED. HE FELL BACKWARDS ONTO HIS RIGHT FOOT AND LEFT HAND.

SLIPPED ON STAKES LAYING ON GROUND, FELL AND PENCIL ROD WENT INTO LEFT THIGH, MINOR LACERATION.
