EE was installing a new 4 wide drive belt on the drive belt on the Dryer Air Supply Fan for the Furnace. Belt was on driven pulley and they were rotating non driven pulley to install belt. When the belt jumped into place EE finger was between the belt and pulley.

Two employees were using a metal bar and forklift to free a compacted bucket elevator. The injured employee was in the line of fire when a bucket broke over causing the bar to jolt upward. Injured employee was struck by the metal bar causing a laceration requiring stitches on the left hand palm side thumb at the second knuckle.
Employee using impact driver installing screws into a fan shroud. Screw slipped sideways and the employees hand moved forward hitting the edge of metal that was left sharp from the manufacturer.

As employee was getting off ladder of hopper car, set foot down on the edge of concrete walkway twisting left ankle
While opening an inspection door to a hammer mill, Employee got finger smashed between plate door and dump hopper sitting close by.

Employees was climbing steps to access top floor of milling area, while making a turn on second level platform EE's right knee buckled. EE stated the right knee was bothering EE on 5/21 and was seen by a doctor on 5/22. Doctor put EE on light duty and a follow up appointment was scheduled.

The employee was performing service work on a dozer while standing on the dozers tracks at 36"" from the ground. The employee was trying to break a bolt loose when the wrench slipped causing him to fall to the ground. When tried to catch himself when hit the ground he broke his right wrist.

Employee was adjusting the tip with his left hand he accidently compressed the trigger with his right hand, causing the high pressure water to cut his finger and the water was injected into his index finger on his left hand.
The employees was removing one of the two remaining truck tires off the fork of the forklift. As he began to roll the first tire away the second tire fell striking him about the knee knocking him to the ground.

Employee stepped on a rock, his right knee gave way. He went to the doctor and was returned to work. His knee was still giving him some trouble. A return visit was set up and a MRI was ran. Surgery was scheduled. He was off work the day of the surgery and was off Monday the 7th. Employee's work status is light duty and a return visit is set up for 11/18/05.

EE was struck in the head by another employee. He was standing in the mill office talking too another employee, when another employee entered the room and struck His in the head. He was taken to the Martin Hospital Emergency room where he recieved stitches. Jim was then transported to Nashville where he went thru sugery on 9/26/04.
Employee was using a lathe and had dropped a chuck into the catch basin of the metal shavings that are left in the bottom pit on the machine. As he attempted to get the part that had fallen, his finger slid across the long slinder shavings and sliced the middle finger on his right hand
EE was cleaning a bag storage area and was throwing trash into a dumpster that was about shoulder high in the top. While doing so the wind blew some of the finer material back into his face and got into his right eye.
EE BENT DOWN, GOT A HOLD OF THE BOTTOM OF THE DOOR WHILE USING PROPER LIFTING TECHNIQUE, STRAIGHTENED AND PROCEEDED TO RAISE THE DOOR THE FULL LENGTH OF HIS REACH. AS HE DID SO PAIN ACCURED IN HIS LOWER BACK, AGGRAVATING AN INCIDENT HE HAD ON 9/10/03. HE HAD BEEN INSTRUCTED PREVIOUSLY ON PROPER LIFTING TECHNIQUES AND HAD RETRAINING IN JAN. 2004. HE SAID HE HAD USED PROPER LIFTING.

A PALLET CONTAINING ELECTRIC MOTORS WAS STORED ON A SHELF ABOVE FLOOR LEVEL. THE PALLET HAD BROKEN SPILLING THE MOTORS ON THE SHELF. AFTER THE MOTORS WERE REMOVED EE CLIMBED ONTO THE SHELF TO REMOVE THE PALLET. AS HE STEPPED OFF A PIECE OF PALLET BROKE OFF UNDER HIS FEET. HE GRABBED ONTO THE SHELF TO BREAK HIS FALL AND FELT PAIN IN HIS RIGHT SHOULDER.
EE WAS MOVING A BOX OF METAL PARTS FROM THE CLAY SHREDDER EST. AT 50 LBS. AT THE BOTTOM OF THE SET OF STEPS HE TURNED TO SET THE BOX DOWN AND STRAINED THE LOWER RIGHT SIDE OF HIS BACK.

EE ATTEMPTED TO ROLL A COUNTERWEIGHT OVER TO LIFT WITH A FORKLIFT TO RE-INSTALL ON FRONT END LOAD. WHILE ATTEMPTING TO ROLL HE FELT PULL IN RIGHT HAM STRING.
EE WAS CLEANING UP SPILT CLAY FROM UNDER 66 SHREDDER WITH A FLOOR SCRAPE. PULLING CLAY OUT FORM SHREDDER.
EE WAS LIFTING STAND FOR SUPER SACK TO PLACE ON SCALE. AS HE WAS SLIDING STAND ON SCALE HIS FINGER GOT CAUGHT BETWEEN THE EDGE OF STAND AND SCALE.
